Apresentação incomum de ruptura esplênica / An unusual presentation of splenic rupture

RESUMO
Em vítimas de trauma abdominal fechado, o baço é o órgão mais comumente afetado, o mais susceptível a lesões graves, e está associado com significante morbidade e mortalidade. Permanece muita controvérsia no diagnóstico e tratamento da lesão esplênica. O diagnóstico deve ser baseado na clínica e exames de imagem, como tomografia (TC) e ultrassonografia (US) de abdome ou até mesmo suspeitado através de lavado peritoneal diagnóstico. As lesões esplênicas são classificadas anatomicamente pela Organ Injury Scaling Committee of the American Association for the Surgery of Trauma. O grau da lesão, assim como a situação clínica do paciente, influenciam ativamente na decisão por tratamento cirúrgico ou não. Apresenta-se o relato de caso de uma paciente com diagnóstico de ruptura esplênica por causa incomum, com tratamento e seguimento em nossa instituição.
ABSTRACT
In victims of closed abdominal trauma, the spleen is the most commonly affected organ, the most susceptible to serious injury, and this is associated with significant morbidity and mortality. There remains much controversy in the diagnosis and treatment of splenic injury. The diagnosis must be based on clinical and imaging studies such as computed tomography (CT) and ultrasonography (US) scans or even suspected by diagnostic peritoneal lavage. The splenic injuries are anatomically classified by the Organ Injury Scaling Committee of the American Association for the Surgery of Trauma. The degree of injury and the situation of the patient actively influence the treatment decision. Here we report the case of a female patient with splenic rupture due to an unusual cause, with treatment and follow up at our institution.
