RESUMEN
El hemangioendotelioma hepático infantil multifocal y sintomático es uno de los tumores vasculares primarios más frecuentes en el niño antes de los dos años. Se presentó un caso de un lactante de ocho meses de edad, procedente de un parto eutócico, con antecedentes patológicos de hemangiomatosis cutánea que ingresó en la Unidad de Cuidados Intensivos del Hospital Pediátrico Octavio de la Concepción y de la Pedraja de Holguín, con dificultad respiratoria severa, tumoración hepática palpable y cianosis, requirió asistencia respiratoria, esta terapéutica no fue suficiente y luego de una taquicardia el paciente falleció debido a insuficiencia respiratoria. Con el uso del rayo x simple de abdomen, el ultrasonido diagnóstico y la tomografía computarizada se describen las características de las lesiones hepáticas multifocales y bilobulares en relación con hemangioendoteliomas; valorando sus características es posible llegar al diagnóstico diferencial entre las lesiones benignas y malignas del hígado.
ABSTRACT
Multifocal, bilobular and symptomatic infantile hepatic haemangioenthelioma is one of the most frequent vascular tumors in the infant. An eight month baby from an normal mother´s pregnancy, with previous history of skin hemangiomatosis at the age of 6 moths was described in this article, who was admitted at Intensive Care Unit due to shorteness of breath, abdominal mass, poor respiratory effort and cyanosis. The patient required oxygen supply then was changed to mandatory ventilation due to respiratory failure. This therapy was unsuccessfully and after tachycardia the patient died. Abdominal X ray, diagnostic ultrasound and computed tomography, were used to describe the multifocal and bilobular lesions related to hemangioendoteliomas. The imagenological techniques is useful to determine the characteristics between malignant and benign lesion of the liver.
