Undescended testis: early forecasting cases candidate for surgery

ABSTRACT
To correlate the age of presentation primary site as well as pretreatment hormonal profile with the outcome of hormonal treatment would help to predict early the non-responders and hence offer them surgery from the start. We analysed 65 boys with undescended testes, 22 bilateral and 43 unilateral, their age range 3-12 years [6.5 +/- 1.2]. All of them were subjected to hormonal profile estimation [FSH, LH, testosterone and IGF-I] and sonographic evaluation for accurate localization of testes. Cases where there ivas failure to visualize testes were excluded from the study. We have found that 20 testes were high scrotal, 42 inguinal and the remainder [25] were located in the abdomen. Hormonal therapy [HCG] was given to all cases and the sonographic progress of testes was assessed 12 weeks after treatment. Thirty two testes moved distally with therapy [4 bilateral and 28 unilateral]. Discriminant equation shoivs 95% accuracy with the age and primary site being the most important predictors. We can conclude that the best the results of hormonal treatment are obtained the earlier the treatment is started and in the most distally located undescended testes
