ABSTRACT
In this study, a new visualized procedure for the local injection of the prostate with antibiotics via the transurethral route by using a special needle was adopted. The study was conducted on 31 patients suffering from chronic bacterial prostatitis, which were resistant to multiple courses of antimicrobial therapy. The patients were divided into 2 groups according to the antibiotic used. In group 1, thiamphenicol was used. In group 2, the choice of antibiotic depended on the culture and sensitivity readings of EPS. Urethrocystoscopy was initially done to exclude any associated pathology. A special fine malleable needle with sharp tip was then introduced through the catheterizing bridge and the antibiotic was injected in different sites of the prostate as well as in the prostatic sinus alongside the verumontanum. Four patients were excluded from the study because of endoscopic evidence of associated pathology. Eleven patients [40.7%] of the remaining 27 patients required a second injection. All patients were reviewed 2-3 weeks after injection and then once monthly for at least 4 months. The average cure rate was 50% in group 1 and 61.5% in group 2 after the first injection. The results improved significantly after the second injection [57% in group 1 and 75% in group 2]
