Anestesia Venosa Total para Cirurgia Ginecológica em Regime Ambulatorial. Associaçäo de Doses Variáveis de Alfentanil e Doses Fixas de Etomidato e Midazolam / Total Intravenous Anesthesia with an Association of Three Doses of Alfentanil to Fixed Doses of Etomidate and Midazolam for Outpatient Gynecological Surgery

ABSTRACT
Total intrevenous anesthesia using a combination of alfentanil, etomidate and midazolam was assessed in 45 female patients undergoing outpatients were divided into three gropus of 15 each and received fixed doses of etomidate (0.15 mg.Kg-1) and midazolam (0.1 mg.Kg-1). Alfentanil was administered at variable doses - 10,20 and 30 ug.Kg-1. Groups 10,20 1nd 30 respectively. Time to indution of and recovery from anesthesia was measured and side-effects were recorded. Heart rate, arterial blood pressure and respiratory rates were registered 6 times before and after induction of anesthesia. Results showed short times to induction of and recovery from anesthesia. Cardiovascular variable measured were stable and respiratory rate decreased in all patients. Six patients from Gropu 30 required ventilatory support because of a respiratory rate less than 10, and 3 patients from the same group due to thoracic rigidity. In conclusion, this combination of intravenous drugs produce anesthesia of good quality for outpatient gynecologic procedures. However, alfentanil must be administered at a maximum dose of 20 ug.Kg-1 because higher doses will increase the incidence of intoward respiratory side-effects
