O presente estudo foi desenvolvido para testar a hipótese de que as variáveis dinâmicas de avaliação de pré-carga ventricular, assim como as estáticas, podem sofrer influência de diferentes parâmetros ventilatórios numa mesma condição volêmica. Métodos:
Dez porcos ventilados mecanicamente em ordem aleatória com VT 16ml/kg, PEEP 15cmH2O ou IE=21 durante normovolemia, após sangria e após reinfusão do sangue.Conclusões:
1 - PEEP elevada acarreta o aumento das pressões de enchimento (PAD e POAP), diminuição da SvO2 e aumento da ∆PP; 2 - o aumento do VT amplifica o valor da ∆PP independentemente da condição de volemia; 3 o VDFVD é a variável hemodinâmica que sofre menos influência da ventilação. A ∆PP, a SvO2 e o VDFVD identificam a condição de hipovolemia melhor que a PAD e a POAP independentemente dos parâmetros ventilatórios...ABSTRACT
This study tests the hypothesis that ventilatory settings may influence dynamic variables of preload evaluation, such as static ones, in a same volemic status. Methods:
Ten anaesthetized pigs were mechanically ventilated in random order with VT 16ml/kg, PEEP 15cmH2O or IE=21 in normovolemia, after withdraw of 20 per cent estimated volemia and after reinfusion of withdrew blood.Conclusions:
High PEEP causes increase of filling pressures values (RAP and PAOP), reduction of SvO2 and increase of ∆PP value; 2- VT causes great influence on ∆PP value; 3 - RVEDV is the hemodynamic variable that is less influenced by ventilatory settings. The ∆PP, SvO2 and RVEDV are better indicators of hypovolemia than RAP and PAOP independently of ventilatory settings...
