Estudo prospectivo aleatorizado comparando a eficiência clínica dos métodos convencional ou piggyback na drenagem venosa do fígado transplantado / Prospective randomized trial comparing the clinical efficiency of conventional versus piggyback method in venous drainage of the transplanted liver

RESUMO

Foram comparados 15 pacientes submetidos a transplante de fígado pelo método convencional (CV) e 17 operados pelo método piggyback (PB). Após a revascularização do enxerto, a mediana do gradiente entre as pressões de veia hepática livre e venosa central (PVHL-PVC) foi 2 mm Hg (0–8 mm Hg) no grupo CV e 3 mm Hg (0–7 mm Hg) no PB (p=0.734). A creatinina global do 1º ao 28º dias foi maior no grupo CV (2.04  0.89 vs. 1.41  0.44 mg/dl; p=0.02). Os grupos foram semelhantes quanto ao desenvolvimento de ascite maciça e à sobrevida. No grupo PB, o gradiente PVHL-PVC foi menor nos casos em que o óstio da veia hepática direita do receptor foi incluído para implantação da veia cava inferior do enxerto (1.4  1.4 mm Hg vs. 3.9  1.7 mm Hg; p=0.005).

ABSTRACT

This study compares 15 patients submitted to liver transplantation by conventional (CV) method and 17 patients by piggyback (PB) method. After graft reperfusion, measurements of gradient between free hepatic venous pressure and central venous pressure (FHVP-CVP) were performed. The median of FHVP-CVP gradient was 2 mm Hg (0–8 mm Hg) in the CV group and 3 mm Hg (0–7 mm Hg) in the PB group (p=0.734). The overall Cr of 1º to 28º postoperative day was higher in CV group (2.04  0.89 vs. 1.41  0.44 mg/dl; p=0.02). Both groups were similar to regarding development of massive ascites and patient survival. In PB patients, FHVP-CVP gradient is lower when the ostium of the right hepatic vein of the recipient is included for implantation of the graft inferior vena cava (1.4  1.4 mm Hg vs. 3.9  1.7 mm Hg; p=0.005).
