RESUMEN
El ictus isquémico constituye el 80 por ciento del total de las enfermedades cerebrovasculares y los fármacos antitrombóticos son una parte importante en la atención de pacientes con esta enfermedad. Por ser las Unidades de Ictus estructuras relativamente nuevas en los centros hospitalarios, no se conoce la frecuencia y el modo de empleo de estos medicamentos en ellas. Se sabe que la elección del tipo de fármaco a emplear, antiagregantes plaquetarios o anticoagulantes, depende de la identificación del mecanismo causal del ictus y de las particularidades de cada paciente. Se estudiaron 275 casos, de los cuales el 72 por ciento tenía un ictus isquémico, se emplearon fármacos antitrombóticos en el 83 por ciento de ellos. Se halló que la etiología fue cardioembólica en el 23 porciento de los ictus isquémicos y recibieron tratamiento con anticoagulantes; la warfarina fue la droga más empleada, y la fibrilación auricular, el principal motivo de indicación de estos fármacos. La mayoría de los pacientes con ictus isquémico de etiología no cardioembólica se trataron con antiagregantes plaquetarios, la aspirina resultó el más empleado.
ABSTRACT
80 percent of the total of cerebrovascular diseases are ischemic strokes, and the antithrombotic drugs play an important role in the attention to patients with this disease. As the Stroke Units are relatively new structures in the hospitals, the frequency and mode of use of these drugs are not known. It is known that the election of the type of drug to be used, platelet antiaggregants or anticoagulants, depends on the identification of the mechanism causing stroke and on the particularities of each patient. 275 cases, of whom 72 percent had ischemic stroke, were studied. Antithrombotic drugs were used in 83 percent of them. It was found that the aetiology was cardioembolic in 23 percent of the ischemic strokes and that they received treatment with anticoagulants. Warfarin was the most used drug, whereas auricular fibrillation was the main reason to indicate these drugs. Most of the patients with ischemic stroke of noncardioembolic aetiology were treated with platelet antiaggregants. Aspirin proved to be the most used.
