Repensando a ação terapêutica / Rethinking therapeutic action

RESUMO

Como outros construtos psicanalíticos centrais, a teoria da ação terapêuticaestá atualmente em processo de mudança, com teóricos de diferentes orientações propondo mecanismos diferentes. Neste artigo, os autores procuram integrar desenvolvimentos de dentro e de fora da psicanálise para oferecer um modelo dinâmico dos multifacetados processos envolvidos na produção de mudança na psicanálise e psicoterapia psicanalítica. Uma teoria da ação terapêutica deve descrever tanto mudanças (objetivos do tratamento) quanto estratégias que serão provavelmente úteis em facilitar tais mudanças (técnica). Os autores crêem que teorias da ação terapêutica com um único mecanismo, independente de quão complexas forem, dificilmente se provarão úteis neste ponto, pois a variedade de metas de mudança e de métodos úteis para se efetuar a mudança nestas metas (tais como técnicas que objetivam alterar diferentes tipos de processos conscientes e inconscientes). As intervenções que facilitam a mudança podem ser classificadas em uma de trêscategorias aquelas que propiciam o insight, aquelas que fazem uso de váriosaspectos mutativos da relação terapêutica e uma variedade de estratégias secundárias que podem ser de tremenda importância. Os autores propõem que, em todas as formas de tratamento psicanalítico, seria mais exato falarmos em ações terapêuticas em vez de ação

ABSTRACT

Like other core psychoanalytic constructs, the theory of therapeutic action is currently in flux, as theorists of differing persuasions propose different mechanisms. In this article, the authors attempt to integrate developments within and without psychoanalysis to provide a working model of the multifaceted processes involved in producing change in psychoanalysis andpsychoanalytic psychotherapy. A theory of therapeutic action must describeboth what changes (the aims of treatment) and what strategies are likely to be useful in facilitating those changes (technique). The authors believe that singlemechanism theories of therapeutic action, no matter how complex, are unlikely to prove useful at this point because of the variety of targets of change and the variety of methods useful in effecting change in those targets (such as techniques aimed at altering different kinds of conscious and unconscious processes). Interventions that facilitate change may be classi. ed into one of three categories those that foster insight, those that make use of variousmutative aspects of the treatment relationship and a variety of secondarystrategies that can be of tremendous importance.
