Hipoacusia e zumbido associada à otosifilis / Hypacusis and tinnitus associated to otosyphilis

RESUMO

Introdução: Tanto a sífilis congênita quanto a adquirida pode causar disfunção cocleovestibular. Na literatura, perda auditiva neurossensorial, zumbido e desequilíbrio têm sido descritos na sífilis secundaria e terciária. Objetivo: O objetivo do trabalho foi enfatizar a necessidade de identificação etiológica para tratamento e controle dos indivíduos com sintomas cocleovestibulares. Relato do Caso Descrevemos o caso de uma paciente de 64 anos, negra, com queixas de diminuição da acuidade auditiva lenta e progressiva desde há cerca de 12 anos, associada a zumbido e desequilíbrios ocasionais. Na avaliação laboratorial e audiométrica, após exclusão de outras causas relacionadas, chegamos ao diagnóstico adicional de otosífilis. Considerações Finais A sífilis deve ser sempre considerada em pacientes com manifestações cocleovestibulares independentemente da faixa etária e de pertencerem ou não a grupos de risco, e do número de parceiros de cada paciente. O diagnóstico etiológico é de suma importância no controle e tratamento desses pacientes.

ABSTRACT

Introduction: Both congenital and acquired syphilis may cause cochleovestibular dysfunction. In the literature, the neurosensorial hearing loss, tinnitus and unbalance have been described in the secondary and tertiary syphilis. Objective: The objective of this work was to emphasize the need for etiologic identification for treatment and control of individuals with cochleovestibular symptoms. Case Report We described the case of a patient, aged 64 years, black, with complaints of slow and progressive diminishing of auditory accuracy since about 12 years ago, associated with tinnitus and occasional unbalances. In the laboratorial and audiometric evaluation, after exclusion of other related causes, we came to the additional diagnosis of otosyphilis. Final Comments: Syphilis must always be taken into account in patients with cochleovestibular complications, irrespectively of the age range, whether or not belonging to risk groups, and the number of partners of each patient. The etiological diagnosis is very important for the control and treatment of such patients.
