Clínica y psicoterapia de las psicosis / Clinic and psychoterapy of the psicosis

RESUMEN

Si bien Psiquiatría y Psicoanálisis comparten la idea de que la característica básica de la psicosis es el trastorno de la relación con la realidad, la diferencia reside en que para el psicoanálisis dicha relación es libidinal y esto significa que, primordialmente, se fundamenta en la relación de objeto. Sobre esta base el psicoanálisis plantea que los síntomas psicóticos tienen sentido más allá del déficit o disfunción, un significado a comprender por la historia del sujeto. Esta concepción de la realidad, que incluye tanto la realidad externa como la realidad psíquica, también determina un específico abordaje psicoterapéutico en tanto el objetivo no es la "verificación" de la realidad externa sino la contención y eventual análisis de la realidad psíquica. Se presentan ejemplos clínicos que ponen de manifiesto la falla primaria en la relación con la realidad y el uso de la Identificación Proyectiva para depositaria en el terapeuta.

ABSTRACT

Even though Psychiatry and Psychoanalysis share the idea that the basic characteristic of psychosis is a disorder of the relation with reality, the difference is that for psychoanalysis said relation is libidinal, which means that it is essentially based on the object relation. According to these grounds, psychoanalysis considers that psychotic symptoms make sense beyond the deficit and dysfunction, a meaning to be understood through the subject s history. This conception of reality, which includes both the external reality and the psychic one, also determines a specific psychotherapeutic approach as long as the purpose is not "verifying" the external reality, but containing and eventually analysing the psychic reality. There are clinical examples that show the primary failure in the relation with reality and the use of Projective Identification to be deposited into the therapist.
