RESUMO
Dentre a variedade de microrganismos comunitários e nosocomiais responsáveis por infecções, os mais relevantes são bactérias como as não-fermentadoras, especialmente Pseudomonas aeruginosa. Devido a sua facilidade de troca de informações genéticas, ela adquire determinantes de resistência importantes como, por exemplo, genes que codificam enzimas metalo-β-lactamases (MβLs). Essa família de enzimas, pertencente à classificação B de Ambler, é muito diversificada (VIM, IMP, SPM, GIM e SIM) e sua expressão resulta na inativação de drogas como os carbapenens que, geralmente são opção terapêutica em infecções por esta bactéria. Contudo, quando tal enzima está presente, estes antimicrobianos não tem ação. Diante disso, a terapia torna-se limitada, podendo ser utilizada a polimixina B ou colistina, extremamente tóxicas. A MβL mais freqüente no mundo é a VIM-2 e no Brasil a mais prevalente é a SPM-1. O CLSI (Clinical and Laboratory Standard Institute) ainda não padronizou a detecção de MβL e a pesquisa destaenzima vem sendo feita através de testes fenotípicos ou moleculares. Entre os ensaios fenotípicos destacam-se os de disco-aproximação com agentes quelantes como o EDTA ou 2-MPA juntamente com imipenem e ceftazidima. Outros testes vêm sendo propostos, como o EIM (EDTA-imipenem microbiological assay). Os testes moleculares são importantes, mas realizados em menor escala devidoà necessidade de investimento em equipamentos e pessoal treinado. A confirmação de MβL nos isolados de P. aeruginosa não é realizada rotineiramente, mas é importante para o conhecimento da epidemiologia local desta enzima.
ABSTRACT
Among the variety of nosocomial and community acquired microorganisms responsible for infections, the most relevant are bacterias like non-fermenting, especially Pseudomonas aeruginosa. Due to its ease-of-trade genetic information, it acquires some important resistance as, for example, genes that codify metallo-β-lactamases (MβL) enzyme. This enzyme family, which belongs to AmblerB classification, is very diversified (VIM, IMP, SPM, GIM e SIM) and its expression results in the inactivation of drugs as carbapenems, that usually are the therapeutic option in infections by this bacteria. However, when this enzyme is into the bacteria, theseantimicrobial dont have action. According to this, the therapy becomes limited to the use of polimixin B or colistin, extremely toxic drugs. The most common MβL in the world is VIM-2 and in Brazil the most prevalent is SPM-1. CLSI (Clinical and Laboratory StandardInstitute) havent standardized the detection of MβL and the research of this enzyme is done by phonotypical or molecular tests. Among the phenotypic tests it stands out the disc-approximation test with chelant agents as EDTA or 2-MPA together with imipenem and ceftazidim. Other tests are being proposed, as the EIM (EDTA-imipenem microbiological assay). The molecular tests are important, but are much less done due to its need of investment in equipment and trained personnel. The confirmation of MβL in P. aeruginosa isolated is not done in the routine, but its important to the knowledge of the local epidemiology of this enzyme.
