Cistitis enfisematosa / Emphysematous cistitis

RESUMEN

Se reporta el caso de un marinero con diabetes mellitus descompensada e insuficiencia renal con una cistitis enfisematosa, evidente en un ultrasonido de la vejiga que se presentó con hematuria, disuria, dolor suprapúbico y síntomas urinarios obstructivos, tratada inicialmente en altamar por 48 horas, con antibióticos. Se mantuvo en excelente estado general y afebril, sin poder aislar una bacteria causal, por haberse iniciado antibioticoterapia temprana; fue tratado además con cirugía conservadora, con debridamiento y lavado de tejidos afectados con agua oxigenada y solución yodada, y con derivación de la orina con una cistostomía suprapúbica, para una excelente evolución y resultado final.

ABSTRACT

We herein report an uncommon case of a diabetic sailor with renal failure and emphysematous cystitis, who presentedwith hematuria, dysuria, suprapubic pain and obstructive urinary symptoms, initially treated at sea with antibiotics,with an ultrasound showing the presence of gas. This man remained in good general condition with no fever, but it was not possible to isolate bacteria in the cultures because ofearly use of antibiotics. He was also treated conservatively with surgical debridement of superficial tissues and bladder mucosa, washings with peroxide and iodine solutions andwith a suprapubic tube for urinary diversion. The outcome was excellent with bladder preservation and full recovery of his health.
