RESUMO
Objetivo: analisar os achados tomográficos dos pacientes vítimas de traumatismo cranioencefálico (TCE), atendidos no Hospital Metropolitano de Urgência e Emergência (HMUE) Método: estudo transversal, pela análise sistemática de 140 exames de Tomografia Computadorizada (TC) multislice de crânio de vítimas de TCE atendidos no HMUE, entre março e julho de 2008. Para análise da significância foi utilizado o teste Qui-Quadrado, com nível ?=0,05. Resultados: observou-se a presença de alterações tomográficas relacionadas ao TCE em mais da metade dos pacientes (53,6%). Dentre as alterações verificadas, as mais freqüentes foram o hematoma subgaleal e subpalpebral (44,3%), fraturas (14,2%), sendo a do tipo complexa a mais presente (70,0%) e a hemorragia intraparenquimatosa com 11,4%. Constatou-se hematoma subdural em 10,0% dos exames; contusão cerebral esteve presente em 8,6%; hemorragia subaracnóidea, desvio da linha media e herniação de estruturas equivaleram-se, todas com 6,4%; edema cerebral difuso em 5,7% e hematomas extradurais em 4,3%. A presença de projéteis/estilhaços e apagamento/assimetria de cisternas basais foi de 1,4% e 0,7% respectivamente. Não houve constatação de hemorragia intraventricular em nenhum dos exames analisados. Conclusão: a prevalência de alterações tomográficas no TCE está associada não somente à gravidade do trauma, porém também na conduta de triagem dos pacientes que não apresentam indicação clinica para a realização do exame.
ABSTRACT
Objective: to analyze the CT findings in patients suffering from traumatic brain injury (TBI) treated at ?Hospital Metropolitano de Urgência e Emergência? (HMUE) Method: A survey by the systematic analysis of 140 multislice CT studies of TBI victims treated at HMUE between March and July 2008. For analysis of significance was used chi-square test with level ? = 0.05. Results: it was observed the presence of CT changes related to trauma in little more than half of the patients (53.6%). Among the changes, the most frequent were the hematoma and subgaleal subpalpebral (44.3%), fractures (14.2%), being the complex type the most present (70.0%) and intraparenchymal hemorrhage with 11.4%. There was subdural hematoma in 10.0% of patients. Cerebral contusion was present in 8.6%. Subarachnoid hemorrhage, midline deviation and herniation of structures were equivalent, all with 6.4%. There was brain swelling in 5.7% and extradural hematomas in 4.3%. The presence of shells and shrapnel and deletion / asymmetry of basal cisterns was 1.4% and 0.7% respectively. There was no finding of intraventricular hemorrhage in any of the tests analyzed. Conclusion: the prevalence of CT findings in TBI is associated not only the severity of trauma, but also the screening conduct of patients without clinical indication for CT scan.
