Introducción:
El síndrome febril sin foco (SFSF) es una patología frecuente en niños, siendo su principal etiología infecciones virales. El uso de terapia antibiótica en estos pacientes sigue siendo un tema de debate.Objetivos:
Evaluar el uso de antibióticos (ATB) en la población estudiada y según predictores de infección bacteriana invasiva (IBI). Pacientes ymétodo:
Estudio descriptivo retrospectivo realizado en 147 pacientes entre 3 y 36 meses de edad, hospitalizados en el servicio de pediatría del Hospital Claudio Vicuña, con diagnóstico de SFSF durante el período de Enero 2011 y Mayo 2013.Resultados:
Se administró terapia ATB en 81 porciento de los pacientes. Del tota éstos, se diagnosticó con etiología viral a un 39,5 porciento, etiología bacteriana a un 40,1 porciento y en un 19,7 porciento no hubo diagnóstico etiológico. Al análisis, según las pautas de riesgo de IBI, se obtuvieron diferencias significativas en el análisis de leucocitosis y PCR. En los datos observados destaca el alto índice de uso de ATB en pacientes con etiología viral, en los cuales se atribuye su uso a la sospecha de sobreinfección bacteriana y a la ansiedad de los padres.Conclusiones:
Existe una alta prevalencia de uso de ATB en los pacientes hospitalizados, en el Hospital Claudio Vicuña, con diagnóstico de SFSF y los valores de exámenes de laboratorio, en especial leucocitosis y PCR, juegan un papel importante al momento de decidir el inicio de terapia ATB empírica, teniendo principal importante la sospecha de sobreinfección bacteriana en cuadros virales comprobados mediante exámenes de laboratorio.ABSTRACT

A Retrospective,descriptive study applied to 147 patients between 3 and 36 months of age, hospitalized in the pediatric service of the Hospital Claudio Vicuña, diagnosed with SFSF in the period from January 2011 to May 2013.Results:
Antibiotic therapy was administered to 81 percent of patients. Of these, 39.5 percent were diagnosed with a viral etiology, 40,1 percent with a bacterial etiology and 19.7 percent had no etiologic diagnosis. In the analysis by IBI risk patterns, there were statistically significant difference in the criteria of leukocytosis and CRP. High rate of use of antibiotic therapy in patients with viral etiology, in which their use is attributed to suspected bacterial infection and parental anxiety.Conclusions:
There is a high prevalence of the use of ATB in hospitalizedpatients in Claudio Vicuña Hospital diagnosed with SFSF and values of laboratory tests, especially leukocytosis and CRP, playing a relevant role in deciding when to start the empirical therapy, being the most important the suspicion of bacterial superinfection in viral conditions proven by laboratory tests.
