Heridas pene-escrotales por proyectil de arma de fuego: consideraciones para el manejo quirúrgico / Penis and scrotum wounds caused by firearm: considerations for surgical management

RESUMEN

Introducción: si bien la afectación traumática del pene y de las bolsas escrotales es rara, representan alrededor del 4% de los traumatismos en general y es común que resulten subevaluados. Objetivos: revisar consideraciones del tratamiento quirúrgico de las heridas por proyectil de arma de fuego pene-escrotales. Material y Métodos: revisión retrospectiva e iconográfica de 100 heridas por arma de fuego, entre Enero de 2010 y Diciembre de 2012. Resultados: de los pacientes que ingresaron al servicio con heridas por proyectil de arma de fuego, se identificaron 5 pacientes (2,5%) con heridas pene-escrotales. De los mismos, 2 accidentales, 2 en situaciones de violencia y uno indeterminado. Dos de los 5 casos tenían además compromiso musuclar del miembro inferior derecho. Dos tenían el proyectil alojado dentro de la bolsa escrotal. Conclusiones: la cirugía está indicada ante la evidencia de rotura parenquimatosa o albugínea y grandes hematoceles. Los traumatismos penetrantes del escroto ameritan conducta quirúrgica en la mayoría de los casos y su tratamiento dependerá del tipo de lesión.

ABSTRACT

Introduction: Although trauma to the penis and scrotal pouch is rare, they account for approximately 4% of injuries in general and they are usually underestimated. Objectives: To examine the considerations for surgical treatment of penis and scrotum wounds caused by firearms. Material and Methods: Iconographic and retrospective review of 100 patients with firearm wounds, between January 2010 and December 2012. Results: Of all the patients admitted to the Surgery Service with firearm wounds, five (2.5%) had suffered penis or scrotal wounds. Of those five, 2 were involved in accidents, 2 were caused in violent situations and one was undetermined. Two out of the five cases also presented right-lower limb muscular compromise. Two patients presented the bullet lodged in the scrotum. Conclusions: Surgery is indicated when there is evidence of parenchymal damage or lesion of albuginea tunic and large haematocele. Surgery is indicated in scrotum penetrating trauma in most cases and their treatment depends on the type of injury.
