RESUMEN
Con el objetivo de determinar validez pronóstica de signos predictores de intubación, se realizó este estudio observacional, longitudinal y descriptivo que incluyó 90 pacientes en cirugías programadas y de urgencia a los que se les realizó anestesia general en el H.O. # 2 “CNS”. A cada paciente se le realizaron preoperatoriamente 4 pruebas Mallampati, Distancia Tiromentoniana, Esternomentoniana y Extensión de la Articulación Atlantooccipital; luego de la inducción anestésica de CormackLehane. Con los resultados obtenidos se clasificó a los pacientes como posibles intubaciones fáciles o difíciles, para después de intubado el paciente comparar si aquellos pacientes que según cada prueba serían fáciles de intubar fueron realmente fáciles, y si aquellos que aparentaban ser difíciles de intubar fueron realmente difíciles. De los 90 pacientes, 77 (85.5%) se intubaron fácilmente, 10 (11.1%) presentaron cierta dificultad pero sin llegar a cumplir con la definición de intubación difícil, 3 (3.3%) fueron difíciles de intubar y no hubo ningún paciente que no se pudiera intubar por el método convencional. Luego de realizado el análisis de discriminación diagnóstica se determinó la sensibilidad, especificidad, valor predictivo positivo y valor predictivo negativo para cada prueba. La valoración de CormackLehane fue la más sensible ya que detectó a todos los pacientes que tendrían una intubación difícil, su especificidad también fue alta; pero como esta prueba se realiza después de inducido el paciente, no nos es muy útil. Las mediciones de las Distancias Esternomentoniana y Tiromentoniana tuvieron las sensibilidades y especificidades más altas después de la valoración de CormackLehane, además de ser valoraciones que se realizan sencillamente y antes de la inducción; por lo que según nuestro estudio son bastante útiles y aunque no detecten a todos los pacientes difíciles de intubar o den muchos falsos positivos, se deben tomar precauciones en aquellos pacientes que presenten mediciones pequeñas de estas distancias. Las pruebas de Mallampati y de la Extensión de la Articulación Atlantooccipital, resultaron tener sensibilidades y especificidades muy bajas para ser buenas pruebas predictorias de intubación.
ABSTRACT
In order to determine the prognosis validity of predictable signs of intubation, we made a study including 90 patients, who were supposed to have programmed surgeries and emergency assistance. With general anaesthesia in H. O. # 2 CNS. Each patient took 4 tests before operation Mallampati, Tyromenthonian Distance, Sternummenthonian and Atlantooccipital Joint Extension, after CormackLeane anaesthetics induction. With results, patients were classified according to possible easy or hard intubations in order to compare them. Those patients after each test would be easy to intube were really easy to do. Those who apparently were hard to intube, in fact they were really hard to do. From the 90 patients, 77 (85.5%) were easy to intube, 10 (11.1%) showed little difficulty but they were not classified as hard intubation. Three (3.3%) of them were hard to intube and there was any patient not being intubated through the conventional method. After carrying out the diagnostic discrimination test, it was set the sensitivity, specificity, positive predictable value and negative predictable value for each test. The CormackLehane valuation was the most sensitive since it could detect all the patients that would have a hard intubation. Its specificity was also high, but as this test is done after inducing the patient, it is not useful to us. Measures of Sternummenthonian and Tyromentians Distances had the highest sensitivity and specificity after CormackLeane valuation. Furthermore, they are simple valuations to do before induction. From our study they are very useful although they don’t detect all patients hard to intube or they give lot of falsepositives answers. Precautions must be taken for those patients who show little measurement of these distances. Mallampati tests and the Atlantooccipital Joint Extension, showed to have very low sensitivity and specificity to be good predictable tests of intubation.
