Nephron Sparing Surgery Using Expanded Polytetrafluoroethylene / 대한비뇨기과학회지

ABSTRACT

PURPOSE:

Small renal masses and complex renal cysts are considered to have a nephron sparing surgery concomitant with that of a frozen biopsy. Nephron sparing surgery is accompanied by postoperative complications, even though techniques for a partial nephrectomy have been developed. Here, our experiences of a repairing method for a renal parenchymal defect after nephron sparing surgery for renal tumorous conditions, is reported. MATERIALS AND

METHODS:

14 patients underwent nephron sparing surgeries for suspicious malignant renal solid and cystic masses. As a nephron sparing surgery, expanded polytetrafluoroethylene was performed in five renal cell carcinoma cases, six benign cystic lesions and one case each of the angiomyolipoma, renal arterial aneurysm and renal caliceal diverticulum. The operative merits of our technique in nephron sparing surgery and complications during postoperative follow-up were evaluated.

RESULTS:

The mean age of the patients and follow-up duration were 49.3 years, ranging from 35 to 66 years and 38.6 months, ranging from 22 to 53 months, respectively. This technique allowed a short kidney ischemic time, which resulted in well-preserved kidneys during follow-up. There was no local or regional recurrence of malignancy in renal cell carcinoma cases. No complications were observed in relation to the nephron sparing surgery or the use of expanded polytetrafluoroethylene.

CONCLUSIONS:

The use of expanded polytetrafluoroethylene to bolster the closure of a parenchymal defect is believed to allow a short ischemic time and a tight and hemostatic closure. Therefore, this technique can be primarily considered in nephron sparing surgery.
