A research of elderly hip fracture patients: the association between preoperative fasting and postoperative delirium / 中国实用护理杂志

ABSTRACT
Objective To evaluate the association between preoperative fasting and postoperative delirium in elderly hip fracture patients. Methods A total of 120 hip fracture patients were divided into the experiment group and the control group with 60 cases each by random digits table method. The experiment group was recommended a 2-3 h preoperative fast for clear fluids and a 6-8 h fast for solids in elective patients. The control group received a 4 h preoperative fast for clear fluids and a 12 h fast for solids in elective patients.Adverse effects of fasting and variation of prevalence of postoperative delirium were compared. Results The incidence rate of hypoglycemi,hunger,anxiety were 18.3%(11/60),63.3% (38/60),16.7%(10/60)in the control group and 3.3%(2/60),36.7%(22/60),5.0%(3/60)in the experiment group, there were significant differences between the two groups(χ2=6.988,8.533, 4.227, P <0.05). There was no significant differences in thirst between the two groups(P>0.05). The incidence rate of postoperative delirium in the experiment group and the control group was 6.7%(4/60)and 21.7%(13/60), there was significant differences between the two groups(χ2=5.551,P<0.05).There was a linear correlation between fasting prohibition and postoperative delirium (r =0.215). Conclusions The new preoperative fasting method is safety and can reduce the incidence of delirium.
