Clinical analysis of 69 patients with primary gastric diffuse large B-cell lymphoma / 肿瘤

ABSTRACT

Objective:

To analyze the diagnosis, treatment and prognosis of primary gastric diffuse large B-cell lymphoma (PGDLBCL).

Methods:

Retrospective analysis was conducted in 69 cases of PGDLBCL between January 2007 and December 2009.The data of clinical presentations, diagnosis, treatment and prognosis were collected, and the association of clinicopathological features with treatment outcomes and prognosis was analyzed.

Results:

PGDLBCL comprised about 10.44% (69/661) of all lymphomas, including 29 males and 40 females; the median age was 54 years old (range 21-78). The major clinical presentations of PGDLBCL were abdominal pain, abdominal distension and melena. ALL patients were treated with chemotherapy-based comprehensive treatment. In this cohort, the complete remission rate was 37.7% (26/69), the partial remission rate was 46.4% (32/69), and the overall response rate was 81.4% (58/69). The univariate analysis revealed that the patients with increased level of serum lactate dehydrogenase (LDH) had poor response (P<0.05). The one-, two- and three-year survival rates were 89.2%, 84.2% and 81.3%%, respectively. The univariate analysis revealed that the age, international prognostic index (IPI) and serum LDH had significant effects on the prognosis; COX regression multivariate analysis showed that only serum LDH had significant effect on the prognosis.

Conclusion:

PGDLBCL is more common in female and treated mainly with chemotherapy-based comprehensive treatment. The prevention and treatment of gastrointestinal bleeding and perforation should be emphasized. Serum LDH was an important prognostic factor for PGDLBCL. Copyright© 2011 by TUMOR.
