ABSTRACT

Objective To analyze the work situation of the personnel in Beijing Centers for Disease Prevention and Control during the novel coronavirns disease 2019 (C0VID-19) epidemic,and to provide references for improving the construction of the capital5 s disease control and prevention system. Methods Cross-sectional survey and cluster sanpling methods were used. A total of 422 municipal-level and 664 district-level professional technicians from CDCs who were mainly involved in epidemic prevention and control in Beijing were included in the study. Self-designed questionnaires were used to collect the basic information, work intensity and satisfaction and other data. The statistical description and test analysis were carried out. Results Among professionals, 64. 36% had nornal workload, and 76. 89% had overload during the epidemic prevention and control period. The proportion of disease control personnel expressing dissatisfaction "with the usual salary level "was 54. 51%, and the satisfaction with the professional title promotion w-as mostly at the average level (45. 58%). The proportions of satisfaction with the prevention and control work arrangements and logistical support during the COVID-19 epidemic were 49. 08% and 54. 42%, respectively. Only 21. 73% professionals were satisfied with the temporar w-ork subsidy. From the perspective of population distribution, staffs at the municipal and district levels and in different job positions were mainly dissatisfied with the salar level (all P<0. 05). Most of staffs who undertook different prevention and control responsibilities were satisfied with the work arrangements and logistics support (all P<0. 05), but they w-ere dissatisfied with the temporar work subsidies (H = 27. 076, P = 0. 012). Among the survey respondents, 44.48% had thoughts of resigning. Regardless of the municipal and district levels, different professional titles or positions, the wdllingness to resign was generally high (all P>0. 05). The primar reason for wanting to leave was the low salary level, followed by difficulty in promotion of professional titles and poor development prospects which were also major considerations. Conclusion It is suggested to improve the stability of CDCs staffs and promote the high-quality and sustainable development of the disease control and prevention system by improving the personnel allocation, strengthening the interdisciplinary talent reserve, improving the salary system and optimizing the professional title appointment mechanism.

ABSTRACT

The prevalence of child maltreatment is quite high during the coronavirus disease 2019 pandemic in rural Hubei province, and children from vulnerable families are at greater risk of self-harm behaviours. The impact of lockdown measures in Wuhan, China during the coronavirus disease 2019 pandemic on child maltreatment remains unknown. The present study attempted to estimate the prevalence of child maltreatment during this period, to identify risk factors, and the influence of child maltreatment. A representative sample of 1,062 school-aged children in rural Hubei province was surveyed. Results indicated that the prevalence of family violence, physical violence, emotional abuse and neglect during the lockdown period were 13.9, 13.7, 20.2 and 7.3 percent, respectively, and that of lifetime prevalence were 17.0, 13.9, 14.6 and 6.9 percent, respectively. And most victims did not seek official help. Boys were more likely to experience physical violence. Children from separated/divorced families tended to report more emotional abuse. Those having family members with a history of drug abuse and mental illness were more likely to experience neglect during the lockdown period. All types of child maltreatment were positively associated with self-harm behaviours. These findings highlight the importance of identifying at-risk children immediately and implementing timely intervention programmes to prevent self-harm behaviours for social workers and health professionals.

ABSTRACT

To review systematically the progress of all domestic and foreign clinical trials since the emergence of the COVID-19 epidemic and the overview of human challenge study (HCS) in the past year, grasp relevant information of ethical review, and explores the feasibility of human challenge study and provide references for stake holders. Based on the Chinese Clinical Trial Registration Center and clinical trial. gov, clinical trials that were consistent with COVID-19 was searched, and the deadline was June 11, 2020.Basic information was extracted and the number of trials was selected from the research type, clinical stage, recruitment status, and ethical passing status. Literature search was conducted based on Pubmed between January 1, 2019 and June 11, 2020.According to the included literature, basic information was extracted and opinions were reported on HCS. 638 and 1935 clinical trials were carried out at home and abroad respectively, of which the number of clinical trials carried out in Hubei reached 323. China and the United States conducted more interventional trials, and the United States had the most clinical trials entered in Phase II, there were a total of 142. Most of the clinical trials were in the stage of recruiting research subjects. In addition, a total of 539 clinical trials in China had passed ethical approval. A total of 5 documents were included in the Human Challenge Study, four maintained a positive attitude towards HCS and put forward corresponding thinking about ethical review. Under the COVID-19, clinical trials for new drugs and new coronavirus vaccines have rapidly increased, which puts forward higher requirements for the progress of ethical review. In the face of such public health emergencies and special research, it is urgent to establish a sound ethical review system and an ethical review system, as well as to construct an ethical model for special research. © 2021, Editorial Board of Pharmaceutical Biotechnology. ABSTRACT

Due to line-of-sight communication links and distributed deployment, Unmanned Aerial Vehicles (UAVs) have attracted substantial interest in agile Mobile Edge Computing (MEC) service provision. In this paper, by clustering multiple users into independent communities based on their geographic locations, we design a 5G-enabled UAV-to-community offloading system. A system throughput maximization problem is formulated, subjected to the transmission rate, atomicity of tasks and speed of UAVs. By relaxing the transmission rate constraint, the mixed integer non-linear program is transformed into two subproblems. We first develop an average throughput maximization-based auction algorithm to determine the trajectory of UAVs, where a community-based latency approximation algorithm is developed to regulate the designed auction bidding. Then, a dynamic task admission algorithm is proposed to solve the task scheduling subproblem within one community. Performance analyses demonstrate that our designed auction bidding can guarantee user truthfulness, and can be fulfilled in polynomial time. Extensive simulations based on real-world data in health monitoring and online YouTube video services show that our proposed algorithm is able to maximize the system throughput while guaranteeing the fraction of served users.

ABSTRACT

Background: The feasibility and the safety of the thoraco-laparoscopic esophagectomy (TLE) was proved by several prominent academic institutions. This technique is technically challenging and requires advanced laparoscopic and thora-coscopic skills. With experience gained from open esophagectomy as well as laparoscopic surgery, thoraco-laparoscopic esophagectomy was introduced in our hospital in 2016. We report our experience in performing TLE. Materials and Methods: We conducted a prospective, nonrandomized, observational study in Hue central hospital, which is one of the biggest hospitals in Vietnam, from January 2016 to January 2021. This study included the esophageal cancers that were diagnosed by endoscopy and confirmed by pathology. Esophageal cancer with cT1b-3N0M0 using chest CT, ultrasound-endoscopy, abdominal CT was indicated for resection initially, while esophageal cancer with cT4N0M0 or T3N(+)M0 was indicated for resection after neoadjuvant therapy. The patients had the ASA I-III. All the data were analyzed statistically using SPSS software (SPSS, Inc, Chicago, IL). Results: We used the TLE technique to operate for 52 consecutive patients. All patients were in a semi-prone position. The male/female ratio was 47/5. The mean age was 57.3±6.3 years, and the mean BMI was 20.5±3.3 kg/m2. The preoperative location of the esophageal cancer was the upper one-third in two (3.8%), the middle one-third in 24 (46.1%), and the lower one-third in 26 (50.0%). The majority of our patients had cTNM stage II (30, 57.7%). Only seven patients (13.7%) had cTNM stage I, whereas 15 patients (31.4%) had cTNM stage III. Of the 45 patients (stage II and III) who needed the neoadjuvant therapy, 30 (57.7%) received short-courses, 15 (28.8%) received long-course of chemotherapy. For 24 patients (46.1%), the histopathology was squamous cell carcinoma. The remaining 28 patients (53.8%) had adenocarcinomas. The operative time was 311.2±45.9 minutes. We did not record either conversion or intraoperative events. The mortality and morbidity rates were 1.9% and 23%, respectively. The hospital stay was 15.6±7.2 days. The median follow-up time was 22±1.5 months, and the overall survival rate at one year was 84.7%. Conclusion: Thoraco-laparoscopic esophagectomy for esophageal cancer with the patient in a semi-prone position is safe and effective, including the lower morbidity rate and the shorter operative time while preserving the long-term outcomes. © 2021, PJD Publications Ltd.
