ABSTRACT

Introduction: Case definitions are used to guide clinical practice, surveillance and research protocols. However, how they identify COVID-19-hospitalised patients is not fully understood. We analysed the proportion of hospitalised patients with laboratory-confirmed COVID-19, in the ISARIC prospective cohort study database, meeting widely used case definitions. Methods: Patients were assessed using the Centers for Disease Control (CDC), European Centre for Disease Prevention and Control (ECDC), World Health Organization (WHO) and UK Health Security Agency (UKHSA) case definitions by age, region and time. Case fatality ratios (CFRs) and symptoms of those who did and who did not meet the case definitions were evaluated. Patients with incomplete data and non-laboratory-confirmed test result were excluded. Results: A total of 263,218 of the patients (42%) in the ISARIC database were included. Most patients (90.4%) were from Europe arid Central Asia. The proportions of patients meeting the case definitions were 56.8% (WHO), 74.4% (UKHSA), 81.6% (ECDC) and 82.3% (CDC). For each case definition, patients at the extremes of age distribution met the criteria less frequently than those aged 30 to 70 years;geographical and time variations were also observed. Estimated CFRs were similar for the patients who met the case definitions. However, when more patients did riot meet the case definition, the CFR increased. Conclusions: The performance of case definitions might be different in different regions and may change over time. Similarly concerning is the fact that older patients often did not meet case definitions, risking delayed medical care. While epidemiologists must balance their analytics with field applicability, ongoing revision of case definitions is necessary to improve patient care through early diagnosis and limit potential nosocomial spread.

ABSTRACT

Introdução No Brasil, a pandemia de COVID-19 apresentou três grandes ondas. A primeira onda ocorreu de março a maio de 2020 e foi causada, predominantemente, pelas variantes B1.1.28, B1.1.33 e B1.1. A segunda onda se iniciou em dezembro de 2020 e teve seu pico em março e abril de 2021. As principais variantes envolvidas na segunda onda foram as variantes P1 (Gamma) e P2. A terceira e maior das ondas ocorreu em janeiro e fevereiro de 2022, sendo majoritariamente causada pela variante BA.1 (Ômicron). A prevalência da COVID-19 entre adultos é substancialmente maior do que na população pediátrica. Além disso, quando adoecem, crianças e adolescentes usualmente apresentam quadro mais leves e melhor prognóstico. Em nosso serviço, observamos diferença significativa na taxa de positividade de COVID-19 de acordo com a variante predominante na comunidade, com considerável aumento do número de casos no mês de janeiro 2022. Objetivo Avaliar o número de casos de COVID-19 em pacientes pediátricos atendidos em hospital público terciário no município de São Paulo e correlacionar esta incidência com as variantes predominantes na comunidade. Método Estudo retrospectivo, que avaliou o número de casos de COVID-19 em pacientes pediátricos do Instituto da Criança e do Adolescente do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (ICr-HCFMUSP), no período de 01/04/2021 a 25/04/2022. A incidência encontrada foi comparada às variantes do SARS-CoV-2 em circulação na comunidade. Resultados Observamos aumento do número de casos investigados a partir de maio de 2021, chegando a 259 casos/mês. Apesar disso, o aumento do número de casos confirmados de COVID-19 só ocorreu em janeiro de 2022 (n = 86), juntamente com o pico de circulação da variante Ômicron. O mesmo ocorreu em pacientes assintomáticos: aumento das solicitações de RT-PCR SARS-CoV-2 a partir de maio de 2021 (n = 247), entretanto o aumento de casos de COVID-19 assintomático só se deu em janeiro de 2022 (n = 33). A taxa de positividade aumentou apenas em janeiro de 2022, chegando a 31% em pacientes sintomáticos e 21% entre assintomáticos. Conclusão A análise da incidência dos casos de COVID-19 na pediatria mostrou maior taxa de positividade de casos sintomáticos e assintomáticos durante a circulação da variante Ômicron na comunidade, sugerindo maior susceptibilidade dessa população a esta variante, quando comparada às variantes que circularam em outros períodos da pandemia.

Introdução Na pediatria, os vírus respiratórios têm grande impacto na prática clínica, configurando uma importante causa de internação hospitalar. conhecimento dos vírus mais prevalentes e suas características tornam-se fatores fundamentais para o melhor manejo dos pacientes pediátricos. Objetivo Avaliar a curva epidemiológica dos principais vírus respiratórios identificados em hospital terciário de referência à população pediátrica no município de São Paulo durante entre Abril de 2021 a Abril de 2022. Método Estudo retrospectivo, que avaliou a incidência de casos de vírus respiratórios entre Abril de 2021 a Abril de 2022, em sintomáticos ambulatoriais e internados no Instituto da Criança e do Adolescente, detectados através de Painel Respiratório qualitativo por RT-PCR para os seguintes patógenos: Coronavírus humano, SARS-CoV-2, Rinovírus humano/Enterovírus (REV), Vírus Sincicial Respiratório (VSR), Metapneumovírus humano, Influenza A e B, Parainfluenza humano e Adenovírus humano (HAdV). O grupo dos coronavírus foi separado entre o SARS-CoV-2 e o grupo “Coronavírus” com os demais sorotipos. Resultados Nos isolados nos painéis respiratórios (Gráfico 1) observa-se o predomínio do REV ao longo do período, com de detecção em Agosto/21 (110 isolados positivos) e vale em Janeiro/22 e Fevereiro/22 com 20 e 18 isolados respectivamente, período precedido por um aumento da circulação de Influenza com pico de 87 detecções em Dezembro e, também, da variante Ômicron do SARS-CoV-2. A segunda maior detecção ocorre com o Parainfluenza (239 isolados), com predomínio entre Agosto a Dezembro, concentrando 79,9% do total de casos. O VSR foi o terceiro mais prevalente (n=231), destacando-se uma mudança no seu padrão sazonal nos meses de inverno, com seu pico de detecção ocorrendo ao final do ano. No Gráfico 2, expusemos a distribuição dos 7 principais patógenos isolados nos painéis virais. No total, foram solicitados 2037 painéis, com uma taxa de detecção no período de 65,3%. As codetecções somaram 25,1% dos isolados, com 334 amostras (Gráfico 4). A Tabela 1 correlaciona os principais patógenos e fornece o número de isolados em que aparece em concomitância com os demais vírus. Conclusão A análise de nossas amostras evidencia a maior prevalência do REV na população pediátrica, além da modificação da sazonalidade do VSR no período, bem como a queda de circulação de todos os vírus estudados com a chegada da variante Ômicron.

This single center observational study aimed to evaluate the impact of staff preparedness by comparing the outcomes between two intensive care units (ICUs) from a hospital that had to expand ICU beds to deal with an incremented volume of critical patients. Patients consecutively admitted to these ICUs with suspected COVID-19, from March 1st until April 30th, 2020, were included. Both ICUs attended a similar population and had the same facilities, what differed was the staff: one previously well-established (ICU-1) and another recently assembled (ICU-2). 114 patients with severe respiratory syndrome were included. In-hospital mortality was 40%. Compared with patients in the well-established ICU-1, patients in the recently assembled ICU-2 were older (54 versus 61.5, p=0.045), received more antibiotics (93% versus 98%, p=0.001) and chloroquine/hydroxychloroquine 6% versus 30%, p=0.001), had a higher proportion of invasive mechanical ventilation (44% versus 52%, p=0.008) and had greater in-hospital mortality (30% versus 50%, p=0.017). The proportion of patients considered at high risk for death according to PSI was similar between the two ICU populations. Age ≥ 60 years (adjusted OR 2.33; 95% CI 1.02-5.31), need of invasive mechanical ventilation (adjusted OR 2.79; 95% CI 1.22-6.37), and ICU type (recently assembled) (adjusted OR 2.38; 95% CI 1.04-5.44) were independently associated with in-hospital mortality . This finding highlights the importance of developing support strategies to improve preparedness of staff recently assembled to deal with emergencies.

ABSTRACT

Coronavirus disease 2019 (COVID-19) has been associated with an increased risk of venous and arterial thrombotic disease. Although pulmonary embolism has been the most common thrombotic complication, there have been recent reports of COVID-19-associated large-vessel ischemic stroke, acute upper- and lower-limb ischemia, as well as infarctions of the abdominal viscera, including renal, splenic, and small bowel infarctions. Here, we describe a case of splenic infarction (SI) associated with aortic thrombosis, which evolved despite the prophylactic use of low-molecular-weight heparin (LMWH), in a 60-year-old female patient with COVID-19. The patient was treated clinically with a therapeutic dose of LMWH, followed by warfarin, and eventually presented a favorable outcome. We also present a review of the literature regarding SI in patients with COVID-19.

ABSTRACT

Non-intubated patients with acute respiratory failure due to COVID-19 could benefit from awake proning. Awake proning is an attractive intervention in settings with limited resources, as it comes with no additional costs. However, awake proning remains poorly used probably because of unfamiliarity and uncertainties regarding potential benefits and practical application. To summarize evidence for benefit and to develop a set of pragmatic recommendations for awake proning in patients with COVID-19 pneumonia, focusing on settings where resources are limited, international healthcare professionals from high and low- and middle-income countries (LMICs) with known expertise in awake proning were invited to contribute expert advice. A growing number of observational studies describe the effects of awake proning in patients with COVID-19 pneumonia in whom hypoxemia is refractory to simple measures of supplementary oxygen. Awake proning improves oxygenation in most patients, usually within minutes, and reduces dyspnea and work of breathing. The effects are maintained for up to 1 hour after turning back to supine, and mostly disappear after 6-12 hours. In available studies, awake proning was not associated with a reduction in the rate of intubation for invasive ventilation. Awake proning comes with little complications if properly implemented and monitored. Pragmatic recommendations including indications and contraindications were formulated and adjusted for resource-limited settings. Awake proning, an adjunctive treatment for hypoxemia refractory to supplemental oxygen, seems safe in non-intubated patients with COVID-19 acute respiratory failure. We provide pragmatic recommendations including indications and contraindications for the use of awake proning in LMICs.

ABSTRACT

Emergency departments are facing an unprecedented challenge in dealing with patients who have coronavirus disease 2019 (COVID-19). The massive number of cases evolving to respiratory failure are leading to a rapid depletion of medical resources such as respiratory support equipment, which is more critical in low- and middle-income countries. In this context, any therapeutic and oxygenation support strategy that conserves medical resources should be welcomed. Prone positioning is a well-known ventilatory support strategy to improve oxygenation levels. Self-proning can be used in the management of selected patients with COVID-19 pneumonia. Here, we describe our experience with two COVID-19-positive patients who were admitted with respiratory failure. The patients were successfully managed with self-proning and noninvasive oxygenation without the need for intubation.
