MEDICAL and SOCIO-PSYCHOLOGICAL EFFECTS of the PANDEMIC on PATIENTS with HEREDITER BLEEDING DISORDERS and THEIR TREATERS

ABSTRACT

Introduction:

The new type coronavirus (COVID19) pandemic affects doctors and other healthcare professionals as well as patients that also causes changes in attitude and behaviors. We planned to reveal the effects of the COVID-19 pandemic on patients with hereditary bleeding disorders (HBDs) and on adult and/or pediatric hematologist following them.

Methods:

The incidence of the disease, the effects of the disease on the clinical status and ongoing treatments of the patients with HBDs were examined between March 2020-June 2021. A questionnaire was conducted with the patients with HBDs who had or did not have the disease, and the hematologist who treated and followed them.

Results:

COVID19 was detected in 40 (6.17%) of 648 HBDs registered in our center. According to the data of the Ministry of Health, the incidence of COVID19 in the same period was 6.5%. Four of them (10%) were 12-18;29 (72.5%) were in the 19-49 age range, 6 (15%) were over 50 years old, and 1 was under 12 years old. Fifteen patients had comorbidities. While only 2 of them were hospitalized, there were no admissions to the intensive care unit. Survey was applied to 35 hematologist and 70 HBDs that 40 of whom had COVID19 infection and 30 of whom did not. Seven (20%) of hematologist are less than 5 years;17 (49%) between 5-15 years and 11 (31%) of them had been specializing in HBDs for more than 15 years. Among the most important medical changes identified in those with HBDs during the pandemic were bleeding due to immobility and changes in treatment doses and socio-psychologically changes were weight gain, increased level of anxiety, social isolation and emotional states. The most concern was the excessive bleeding, the supply of medication, and the life of himself and his relatives. However, there was no difficulty in obtaining medicine. On the other hand, the concerns of the hematologist were about the patients. While the level of anxiety was the same in all experts, it increased as the seniority increased. Discussion/

Conclusion:

The differences caused by the pandemic on the attitudes and behaviors of those with HBDs and the specialists dealing with their follow-up/treatment should be considered, examined and resolved although the rates of contracting COVID19 infection in HBDs are not different from the population.
