Incisional herniasurgery has witnessed animportant progress over the last years, not only as far as thepathophysiological and etiopathogenetic aspects are concerned, butalso from a technical point of view. Thanks to the recent findingson genetical disorders of the collagen, incisional hernia is todayconsidered a true "disease of the abdominal wall". Surgeons need tobe aware of all the different repairingtechniques, in order tomake an elective choice and to keep side effects and recidivesunder control. Today, on the basis of the most recent research onsynthesis of collagen and collagenolysis, the treatment of theincisional hernia should not disregard the use of syntheticprostheses and of biomaterials, both in the routinal surgicalpractice and in the most complex clinical cases.
