ABSTRACT

The therapy of children with acute lymphoblastic leukemia (ALL) in limited resource geospaces is challenging and must balance safety, efficacy, availability, and affordability. We modified the control arm of the St. Jude Total XI protocol for outpatient delivery including once-weekly daunorubicin and vincristine in initial therapy, postponing intrathecal chemotherapy until day 22, prophylactic oral antibiotics/antimycotics, use of generic drugs, and no central nervous system (CNS) radiation. Data were interrogated from 104 consecutive children ≤12 years (median, 6 years [interquartile range (IQR), 3, 9 years]. All therapies were given in an outpatient setting in 72 children. Median follow-up is 56 months (IQR 20, 126 months). A total of 88 children achieved a hematological complete remission. Median event-free survival (EFS) is 87 months [95% confidence interval (CI), 39, 60], 7.6 years in low-risk children (3.4, 8 years) whereas 2.5 years (1, 10 years) in high-risk children. The 5-year cumulative incidence of relapse (CIR) is 28% (18, 35%), 26% (14, 37%) in low-risk children and 35% (14, 52%) in high-risk children. Median survival for all subjects is not reached but must exceed 5 years. A total of 36 children relapsed at a median of 12 months (5, 23 months). Outcomes were comparable to those reported in the control arm of the Total Therapy XI study, but inferior to current treatment protocols in high-income countries. The average cost of the first 2 years of therapy was $28,500 USD compared with an average cost of approximately $150,000 USD in the US, an 80% saving. In conclusion, using an outpatient-based modification of the St. Jude Total XI protocol, we obtained good results with relatively few hospitalizations or adverse events and at a substantial saving. This model can be applied in other resource-poor geospaces.

ABSTRACT

Introducción: la enfermedad pulmonar obstructiva crónica (EPOC) es un trastorno res-piratorio caracterizado por síntomas clínicos y compromiso funcional que afecta la ca-pacidad aeróbica limitando las actividades cotidianas y la calidad de vida. La prueba de caminata de 6 minutos (C6M) es una prueba sencilla y de bajo costo que evalúa la capa-cidad de los pacientes para realizar sus actividades cotidianas. Objetivo: evaluar la re-lación entre la capacidad aeróbica medida por la distancia recorrida en la C6M y el se-xo, edad, disnea y comorbilidades cardiometabólicas en pacientes con EPOC. Material y métodos: estudio de corte transversal, descriptivo, basado en pruebas de caminatas de 6 minutos (C6M) realizadas en pacientes con EPOC. Resultados: se evaluaron 101 pacientes, hombres (63,4%), con una edad promedio de 74,1±8,7 años. Al correlacionar C6M con otras variables se encontraron diferencias estadísticamente significativas. La distancia media recorrida fue mayor en hombres que en mujeres (DM: 58,3 metros, IC 95%; 16 - 100,6, p=0,007). Los pacientes < 75 años, sin comorbilidades y disnea < 2 tu-vieron mejor desempeño en la C6M que los > 75 años (DM; 62,012 metros IC 95% 21,5 - 102,4, p=0,003), con comorbilidades (DM: 42,2 metros, IC 95%, 0.003 - 84,4; p=0,050) y disnea ≥ 2 (DM: 65,8 IC 95% 23,9 - 107,6, p=0,002). Conclusiones: el sexo femenino, la presencia de comorbilidad cardiovascular y metabólica, y la edad se asocian con dis-minución en la capacidad física aeróbica y funcional en los pacientes con EPOC. Introduction: chronic obstructive pulmonary disease (COPD) is a respiratory disorder characterized by clinical symptoms and functional impairment that affects aerobic capacity, limiting daily activities and quality of life. The 6-minute walk test (C6M) is a simple, low-cost test that assesses a patient's ability to perform their daily activities. Objective: to evaluate the relationship between aerobic capacity measured by the distance covered in the C6M and gender, age, dyspnea and cardiometabolic comorbidities in patients with COPD. Materials and methods: cross-sectional, descriptive study, based on 6-minute walk tests (C6M) performed in patients with COPD. Results: 101 male patients (63.4%), with a mean age of 74.1 ± 8.7 years, were evaluated. When correlating C6M with other variables, statistically significant differences were found. The mean distance traveled was greater in men than in women (MD: 58.3 meters, 95% CI: 16 - 100.6, p=0.007). Patients <75 years old, without comorbidities and dyspnea <2 had better performance in the C6M than those >75 years old (MD; 62 meters CI 95% 21.5 - 102.4, p=0.003), with comorbidities (MD: 42.2 meters, 95% CI, 0.003 - 84.4; p=0.050) and dyspnea ≥ 2 (MD: 65.8, 95% CI 23.9 - 107.6, p=0.002). Conclusions: female sex, the presence of cardiovascular and metabolic comorbidity, and age are associated with decreased aerobic and functional physical capacity in patients with COPD. (AU)

ABSTRACT

El objetivo de este documento es mostrar una síntesis de los principales sesgos que pueden aparecer en el estudio de la salud del trabajo, sus orígenes y consecuencias. Para ello, se realizó una revisión documental de libros y artículos publicados en los últimos 10 años en inglés y español. Se dejó fuera aquellos documentos que no estuvieran publicados en revistas con comité editorial y en caso de los libros, que no tuvieran registro ISBN. Se incluyeron 7 libros y 14 artículos. Un sesgo es un error sistemático, que sucede en el proceso de investigación y que pueden darse durante la planeación, recolección y procesamiento de datos. Se revisarán dos tipos principales de sesgos: 1) Selección y 2) Información y/o medición, en cada caso se describe la definición, potencial impacto en los resultados y se plantean una serie de ejemplos que ayudan a su compresión. Uno de los efectos principales de la presencia de sesgos en investigación es que ocasiona un error de clasificación en los participantes, que puede ser diferencial o no, esto impacta los resultados al sobre o subestimarlos, en otras palabras, se encuentran efectos mayores a los reales o peor, no los encontramos cuando si existen. Para ilustrar el impacto de los sesgos, se revisan distintos escenarios que ejemplifican los casos de sobreestimación y subestimación que originan resultados poco confiables(AU)


This document aims to show a synthesis of the main biases that can be present in occupational health studies, their origins and their consequences. For this purpose, a documentary review was conducted of books and articles published in the last 10 years in English and Spanish. Excluding those documents that were not published in journals with an editorial board and, in the case of books, that did not have an ISBN registry. A total of 7 books and 14 articles were included. A bias is a systematic error that occurs in the research process and can occur during the data planning, collection, and processing. Two main types of biases will be reviewed: 1) Selection and 2) Information and/or measurement biases, in each case their definition and, their possible impact on the results are described, a series of examples that may help to understand them are provided. One of the main effects of the presence of biases in research is that they cause an error in the classification of participants, which can be differential or not, and this has an impact on the results by overestimating or underestimating them, i.e., finding stronger effects than the real ones, or worse, not finding them when they exist. To illustrate the impact of biases, different scenarios were reviewed that provide examples of cases of overestimation and underestimation that lead to unreliable results(AU)

ABSTRACT Introduction: We have previously shown that some patients present thrombocytopenia (less than 100 × 109/L platelets) in non-alcoholic fatty liver disease (NAFLD). To further explore the nature of this association, we have now analyzed the association of thrombocytopenia with neutropenia (less than 0.5 × 109/L granulocytes) in NAFLD. Material and methods: Persons with NAFLD were prospectively accrued in the study after February 2018. The presence of NAFLD was defined by both serologic determinations (Fibromax ®) and liver transient elastography (TE/Fibroscan ®). Results: In 123 consecutive patients with NAFLD without cirrhosis, thrombocytopenia was identified in 20 (16%), whereas neutropenia was identified in 9 (7%). In the subset of 20 patients with NAFLD and thrombocytopenia, granulocytopenia was identified in 5 (25%), whereas in the subset of 9 patients with granulocytopenia, thrombocytopenia was identified in 5 (55%). We found a significant association between thrombocytopenia and both leukopenia and granulocytopenia (OR 8.25, 95% CI 1.9-34.2, p = 0.004). Conclusions: Both thrombocytopenia and neutropenia were identified in persons with NAFLD and, as there is a significant relationship between these two variables, we speculate that this finding may support the possibility of hypersplenism being involved in the cytopenias found in NAFLD without cirrhosis.

ABSTRACT

Purpose: Overuse of short-acting ß2-agonists (SABAs) for asthma is associated with a significant increase in exacerbations and healthcare resource use. However, limited data exist on the extent of SABA overuse outside of Europe and North America. As part of the multi-country SABA use IN Asthma (SABINA) III study, we characterized SABA prescription patterns in Colombia. Patients and Methods: This observational, cross-sectional cohort study of SABINA III included patients (aged ≥12 years) with asthma recruited from seven sites in Colombia. Demographics, disease characteristics (including investigator-defined asthma severity guided by the 2017 Global Initiative for Asthma report), and asthma treatments prescribed (including SABAs and inhaled corticosteroids [ICS]) in the 12 months preceding the study were recorded using electronic case report forms during a single study visit. Results: Of 250 patients analyzed, 50.4%, 33.2%, and 16.4% were enrolled by pulmonologists, general medicine practitioners, and allergists, respectively. Most patients were female (74.0%) and had moderate-to-severe asthma (67.6%). Asthma was partly controlled or uncontrolled in 57.6% of patients, with 15.6% experiencing ≥1 severe exacerbation 12 months before the study visit. In total, 4.0% of patients were prescribed SABA monotherapy and 55.6%, SABA in addition to maintenance therapy. Overall, 39.2% of patients were prescribed ≥3 SABA canisters in the 12 months before the study visit; 25.2% were prescribed ≥10 canisters. Additionally, 17.6% of patients purchased SABAs over the counter, of whom 43.2% purchased ≥3 canisters. Maintenance medication in the form of ICS or ICS/long-acting ß2-agonist fixed-dose combination was prescribed to 36.0% and 66.8% of patients, respectively. Conclusion: Our findings suggest that prescription/purchase of ≥3 SABA canisters were common in Colombia, highlighting a public health concern. There is a need to improve asthma care by aligning clinical practices with the latest evidence-based treatment recommendations to improve asthma management across Colombia.

ABSTRACT

OBJECTIVE: Determine the proportion of vaccinated patients in a private hematology and internal medicine outpatient clinic and potential factors in adherence in at-risk patients (due to onco-hematological diseases). MATERIALS AND METHODS: This is a cross-sectional study of outpatients from a private clinic. We applied a non-validated instrument to all patients attending the outpatient clinic from May to October 2021. According to the primary diagnosis, we classified patients into onco-hematological and non-onco-hematological patients. Since national authorities exclusively executed and planned the rollout of vaccines, the order and eligibility defined by authorities of vaccination was considered when conducting the analysis and patients were classified according to the their corresponding group. RESULTS: 397 participants were accrued, 269 (68%) had an onco-hematological condition. In the whole group, 73 (18.3%) had a history of infection. Vaccination history was present in 286 persons (72%); 82% had two doses. In the subset of 269 persons with an onco-hematological condition, 191 (71%) were vaccinated, whereas 95 participants with non-hematological conditions (73%) had received the vaccine. Vaccination status was associated with age (OR 1.07, 95%CI: 1.03,1.10, p<0.0001) and body mass index (OR 1.11, 95%CI: 1.04,1.17, p<0.0001). CONCLUSIONS: According to our study, vaccination adherence at our center is significantly different from the nationwide proportion of vaccines.

ABSTRACT

The results of treatment of adolescents and adults with acute lymphoblastic leukemia (ALL) remain unsatisfactory. Pediatric-inspired treatments seem to be related with better outcomes. 126 adolescent and adult patients with ALL were treated in a 37-year period with a pediatric inspired combined chemotherapy (PICC) schedule, delivered on an outpatient basis and based on the St. Jude´s TOTAL XI pediatric protocol employing vincristine, prednisone, asparaginase, daunorubicin, etoposide, cytarabine, methotrexate, mercaptopurine and triple intrathecal therapy. 80 % of patients were able to receive the initial seven-week period of induction / consolidation fully as outpatients and 77 % achieved a complete remission. In adolescents and young adults (AYAs) the median probability of overall survival (OS) was 44 months, whereas the 5-year OS was 48 %. In adults, the median probability of OS was 24 months, and the 5-year OS was 32 %. Patients with T-cell ALL did significantly worse than those with a B cell phenotype (OS at 5 years 17 versus 40 %, respectively). These figures are better than those informed in our country employing more aggressive, in-hospital schedules such as the hyper-CVAD. We found that, in AYAs and adult patients with ALL, the use of an asparaginase-containing PICC delivered on an outpatient basis renders acceptable results, better than those obtained in similar socioeconomic circumstances employing adult-oriented schedules. Additional studies are needed to assess the usefulness of these PICC treatments in adult individuals with ALL treated in underprivileged circumstances, such as those prevailing in LMIC.

INTRODUCTION: The decision to get involved in the study and practice of medicine is not easy. Within the scientific environment, achieving both professional and personal success requires a strict discipline, where effort becomes an essential part of daily life; in addition, having family support becomes crucial in order for not to lose hope when confronting the different adversities that arise during medical training. OBJECTIVE: To identify families where at least two members belong to the Academia Nacional de Medicina de México (ANMM). METHODS: A cross-sectional study was carried out to identify families of Mexican doctors where at least two members, consanguineous or in-laws, have been or are ANMM members through a review of 2017 ANMM Directory and personal contact with the different academics. RESULTS: Information on 45 families belonging to the ANMM was collected. CONCLUSIONS: From this study, it is possible to show the great influence that some doctors have in their family environment, which makes the study of medicine attractive as a life project.

ABSTRACT

INTRODUCTION: High-dose melphalan (HD-Mel) has been successfully employed in autografting patients with multiple myeloma. An advantage of this regimen is that the total dose of Mel can be delivered in a single day, being particularly useful when non-frozen hematopoietic stem cells are employed in the autograft. MATERIAL AND METHODS: All consecutive patients with R/R lymphomas, both HL and NHL studied and treated at two different centers were prospectively included in a study of ASCT employing a single dose of HD-Mel (200 mg/m2). A group of R/R HL or NHL autografted employing BEAM-like preparative regimens was constructed matched by diagnosis and age. The primary endpoint of the study was overall survival (OS), the secondary endpoint was event-free survival (EFS). RESULTS: Twenty-five R/R HL/NHL patients were prospectively accrued in the study. There were 8 (32%) females, 13 (52%) patients had at least 1 adverse effect: 7 (28%) developed mucositis, 5 (20%) neutropenic fever, and 6 (24%) grade IV nausea. In the HD-Mel group, median overall survival (OS) was not achieved and OS at 36 months was 71%, the transplant-related mortality being 0%. In the control group, median OS was not achieved and the 36-month OS was 76%, results not statistically significant (p 0.5). The EFS was also similar in both groups (p 0.5). CONCLUSION: HD-Mel alone is non-inferior to a BEAM-like regimen as a preparative regimen for autografting patients with R/R HL and NHL. The regimen is adequate to graft persons with non-frozen stem cells.

ABSTRACT

This is the first report about the presence of pesticide residues in water and sediments from Chacahua-Pastoria Lagoon System in the Oaxaca Coast, Mexico. Organochlorine and organophosphate pesticides were analyzed in water and sediment samples collected during rainy and dry seasons in 2018. The pesticides extraction was carried out by 525.2 EPA and QuEChERS methods for water and sediments samples, respectively. The quantification was done by gas chromatography coupled to electron microcapture (µECD) and flame photometric (FPD) detectors. Univariate and multivariate analysis were carried out to evaluate the distribution patterns, comparing between lagoons, seasons and matrices. The spatial distribution of malathion and organochlorines in water showed the highest values near to adjacent land with some patterns differences. DDT was the most frequent in water and sediments samples, showed the highest concentrations in the Pastoria Lagoon. These results indicate the need to establish a permanent monitoring programs to implement mitigation measures.

ABSTRACT

There is data suggesting that the clinical behavior of multiple myeloma (MM) may be different in Latin Americans than in Caucasian or African-Americans, consistent with a less aggressive course of MM in Latinos. We analyzed the overall survival (OS) of 139 persons with MM in a single institution in México, as well the variables which were associated with long-term OS. Of all patients, the median OS was 11 years whereas the 5-year and 10-year OS were 75% and 55% respectively. The analysis of variables showed that the variable related with five-year survival was having hematopoietic stem cell transplantation (HSCT), whereas the variables related with 10-year survival were HSCT, age at diagnosis (patients younger than 50 survived longer), light chain type (kappa survived longer) and ISS stage (stage I patients survived longer). The only variable associated with both 5 and 10-year survival was HSCT. A plateau in the OS was reached after 10 years, both in grafted and non-grafted patients. We have confirmed the critical role of HSCT in the prognosis of persons with MM, independent of the induction treatment or the maintenance post-transplant, and we have identified a better prognosis in this cohort, as compared with African-Americans or Caucasians, since the proportion of long-term survivors in our group is seemingly better than those in other populations.

RESULTS: In 123 consecutive patients with NAFLD without cirrhosis, thrombocytopenia was identified in 20 (16%), whereas neutropenia was identified in 9 (7%). In the subset of 20 patients with NAFLD and thrombocytopenia, granulocytopenia was identified in 5 (25%), whereas in the subset of 9 patients with granulocytopenia, thrombocytopenia was identified in 5 (55%). We found a significant association between thrombocytopenia and both leukopenia and granulocytopenia (OR 8.25, 95% CI 1.9-34.2, p = 0.004). CONCLUSIONS: Both thrombocytopenia and neutropenia were identified in persons with NAFLD and, as there is a significant relationship between these two variables, we speculate that this finding may support the possibility of hypersplenism being involved in the cytopenias found in NAFLD without cirrhosis.

ABSTRACT

La aspergilosis pulmonar clásicamente se ha considerado como una enfermedad de paciente inmunocomprometido (neoplasias hematológicas, neutropenia severa, trasplante de órgano sólido, virus de la inmunodeficiencia humana/síndrome de inmunodeficiencia adquirida VIH/SIDA) o que afecta a pacientes inmunocompetentes, pero críticamente enfermos. Sin embargo, durante la pandemia por COVID-19 el comportamiento de las infecciones fúngicas ha cambiado, siendo observada con mayor frecuencia en la actualidad. Se presenta el caso de un paciente masculino de 20 años, procedente de la Isla de San Andrés, al norte de Colombia, sin patología previa conocida, ingresa al servicio de urgencias cursando con cuadro de tres días de fiebre y tos seca. A su ingreso el paciente está consciente, con fluctuación de la atención, taquicardia y febril. Examen pulmonar y abdominal sin alteraciones. Paraclínicos con leucocitosis, neutrofilia, prueba rápida para COVID-19 negativa y NS1 para dengue negativa. La radiografía de tórax en proyección lateral evidencia infiltrados intersticiales y en tomografía, lesiones nodulares de bordes mal definidos algunos espiculados, con infiltrado intersticial vecino (depósitos tumoral o infeccioso). Resultado de RT- PCR SARSCoV-2 positivo: por hallazgos radiológicos se realiza biopsia pulmonar por toracoscopia, cuyo estudio histológico revela nódulos múltiples y estructuras fúngicas en su interior compatible con aspergillus. Se inicia terapia de primera línea con voriconazol y egreso asintomático al completar tratamiento. Se concluye que las infecciones fúngicas representan un reto diagnóstico, su retraso, por falta de sospecha en el paciente inmunocompetente impacta en la morbimortalidad. El aumento de las infecciones fúngicas durante la pandemia, obliga a considerar la aspergilosis como diagnóstico diferencial en paciente con COVID-19, con fiebre persistente y hallazgos radiológicos atípicos.


Pulmonary Aspergillosis has classically been considered a disease of immunocompromised patients (hematological neoplasms, severe neutropenia, solid organ transplantation, human immunodeficiency virus / acquired immunodeficiency syndrome HIV / AIDS) or immunocompetent but critically ill patients. However, during the COVID-19 pandemic the behavior of fungal infections has changed, being observed more frequently today. A case of 20-year-old male patient, from San Andrés islands, north of Colombia; With no known prior pathology, he was admitted to the emergency department with a 3-day fever and dry cough. On admission, a conscious patient with fluctuating attention, tachycardia and fever. Pulmonary and abdominal examination without alterations. Paraclinical patients with leukocytosis, neutrophilia, rapid test for covid19 negative and NS1 for dengue negative. The chest X-ray in lateral projection shows interstitial infiltrates and in tomography as nodular lesions with poorly defined edges, some spiculated, with neighboring interstitial infiltrate (tumor or infectious deposits). A positive SARS COV 2 RT-PCR result, due to radiological findings, a lung biopsy was performed by thoracoscopy, the histological study of which revealed multiple nodules fungal structures inside compatible with aspergillus. First-line therapy with voriconazole was started, asymptomatic discharge upon completion of treatment. As conclusion fungal infections represent a diagnostic challenge, their delay, due to lack of suspicion in the immunocompetent patient, impacts morbidity and mortality. The increase in fungal infections during the COVID-19 pandemic makes it necessary to consider aspergillosis as a differential diagnosis in a patient with COVID-19, with persistent fever and atypical radiological findings.

BACKGROUND: The outcomes of Hodgkin´s lymphoma (HL) in México have not been widely reported. Simplified and affordable treatments have been adopted in middle-income countries. AIM: The aim was to evaluate long-used therapies for HL in México in a long-term basis. METHODS: In a 34-year time period, 88 patients with HL were treated at a single institution in México. Patients were treated with adriamycin bleomycin vinblastine and dacarbazine (ABVD) or mechlorethamine, vincristine, procarbazine, and prednisone (MOPP). Relapsed or refractory patients were given ifosfamide, carboplatin, and etoposide (ICE) followed by autologous or allogeneic stem cell transplants. RESULTS: Thirty-seven women and 51 men were included; the median age was 29 years. Patients were followed for a mean of 128 mo. The 310-mo overall survival (OS) was 83% for patients treated with MOPP and 88% for those treated with ABVD. The OS of patients who received autologous stem cell transplantation was 76% (330 mo) vs 93% (402 mo) in those who did not. CONCLUSION: HL may be less aggressive in Mexican population than in Caucasians. Combined chemotherapy renders acceptable results, regardless of clinical stage.

PURPOSE: The COVID-19 pandemic is a colossal challenge for global health; nonetheless, specific subgroups face considerably higher risks for infection and mortality. Among patients with malignant diseases, those with hematologic neoplasms are at a higher risk for poor outcomes. The objective of this study was to register treatment modifications associated with the COVID-19 pandemic and their short-term consequences in Latin America. METHODS: Multicenter, prospective, observational, cohort study including patients older than 14 years from 14 centers in four countries (Mexico, Peru, Guatemala, and Panama) who had a confirmed diagnosis of acute leukemia, and who were undergoing active treatment since the first COVID-19 case in each country until the cutoff on July 15, 2020. RESULTS: We recruited 635 patients. Treatment modifications because of the COVID-19 pandemic were reported in 40.8% of cases. The main reason for such modifications was logistic issues (55.0%) and the most frequent modification was chemotherapy delay (42.0%). A total of 13.1% patients developed COVID-19 disease, with a mortality of 37.7%. Several factors were identified as independently associated with mortality, including a diagnosis of acute myeloid leukemia (odds ratio 2.38 [95% CI, 1.47 to 3.84]; P < .001), while the use of telemedicine was identified as a protective factor (odds ratio 0.36 [95% CI, 0.18 to 0.82]; P = .014). CONCLUSION: These results highlight the collateral damage of COVID-19 in oncology patients.
