ABSTRACT

SUMMARY: The trachea is a tubular organ lying between larynx and lungs containing smooth muscle, membranes, and cartilage. This paper evaluated the dimessions of the trachea and main bronchi morphometry in healthy adults using Computed Tomography. This retrospective observational study was performed with 170 healthy adult subjects (89 females, 52.35 %; 81 males,47.65 %). The length of the trachea, the anteroposterior and transverse diameter of the trachea and the right and left main bronchi were measured. From these measurements, the trachea shape were calculated and four types of trachea were identified as circular, oval, horseshoe-shaped, and rectangular. All measurements were significantly higher in males than females (excluding tracheal bifurcation angle). According to the value obtained by dividing the anteroposterior by the width of the trachea, tracheal shapes are considered; the circular shape was seen 104 subjects (61.2 %), followed by oval type (34 subjects), horseshoe type (24 subjects) and rectangular type. (8 subjects). Also, the most frequently seen was circular type in both females and males. Hovewer, there was no significant difference between sex in terms of trachea shape. Additionally, a striking finding was that trachea morphometry and morphology showed the significance according to age dependent changes. Trachea measurements were affected several reasons such as used methods, age, sex, or race. This study has many clinical importance as it may reduce the risk of accidental damage to these area by clinicians such as cardiothoracic surgeons, anesthetist, or radiologist.


La tráquea es un órgano tubular que se encuentra entre la laringe y los pulmones y que contiene músculo liso, membranas y cartílago. Este trabajo evaluó las dimensiones de la tráquea y la morfometría de los bronquios principales en adultos sanos mediante Tomografía Computarizada. Este estudio observacional retrospectivo se realizó con 170 sujetos adultos sanos (89 mujeres, 52,35 %; 81 hombres, 47,65 %). Se midió la longitud de la tráquea, el diámetro anteroposterior y transversal de la tráquea y los bronquios principales derecho e izquierdo. A partir de estas mediciones, se calculó la forma de la tráquea y se identificaron cuatro tipos de tráquea: circular, ovalada, en forma de herradura y rectangular. Todas las mediciones fueron significativamente mayores en hombres que en mujeres (excluyendo el ángulo de bifurcación traqueal). Según el valor que se obtiene al dividir el anteroposterior por el ancho de la tráquea, se consideran las formas traqueales; la forma circular fue observada en 104 sujetos (61,2 %), seguida del tipo ovalado (34 sujetos), tipo herradura (24 sujetos) y tipo rectangular (8 sujetos). Además, el tipo más frecuente fue el circular tanto en mujeres como en hombres. Sin embargo, no hubo diferencias significativas entre sexos en términos de forma de la tráquea. Además, un hallazgo sorprendente fue que la morfometría y la morfología de la tráquea mostraron importancia según los cambios dependientes de la edad. Las mediciones morfométricas de la tráquea se vieron afectadas por varios motivos, como los métodos utilizados, la edad, el sexo o la raza. Este estudio tiene importancia clínica ya que puede reducir el riesgo de daño accidental por parte de médicos como cirujanos cardiotorácicos, anestesistas o radiólogos.

ABSTRACT

SUMMARY: The temporal bone is a complicate structure which located on the lateral skull. The objective of the present study was to determine the temporal bone air spaces' morphometry, morphology, and pneumatization in Turkish healthy adult people. This retrospective observational study was carried out from 82 subjects (47 males, 35 females) aged 18-69 years. The external auditory canal and related structures' diameters and the volume of these areas were evaluated. The means and standard deviations of the Meatus acusticus externus length (MAEL), meatus acusticus externus pars cartilaginea length (MAEcL), meatus acusticus externus pars ossea length (MAEoL), meatus acusticus externus pars cartilaginea surface (MAEcS), meatus acusticus externus pars ossea surface (MAEoS), meatus acusticus externus volume (MAEV), meatus acusticus externus pars cartilaginea volume (MAEcV), meatus acusticus externus pars ossea volume (MAEoV), processus mastoideus air cells volume (PMACV), cavum tympani volume (CTV), and temporale pneumatic spaces volume (OTPSV) were found as 23.21±3.70 mm, 12.69±3.72 mm, 7.80±3.70 mm, 669.89±107.7 mm2, 267.50±30.51 mm2, 743.50±119.6 mm3, 971.97±156.69 mm3, 419.51±48.67 mm3, 5915.93±650.34 mm3, 673.48±91.93 mm3, 7813.34±717.49 mm3 have found in 82 subjects, respectively. In this paper, the morphometric and volume properties of the temporal bone cavities measurements were significantly higher in males than females. These results may both provide reference values of Turkish healthy population, and lead to decrease potential surgical complications about temporal and mastoid regions.


El hueso temporal es una compleja estructura ubicada en el parte lateral del cráneo. El objetivo del presente estudio fue determinar la morfometría, morfología y neumatización de los espacios aéreos del hueso temporal en individuos adultos turcos sanos. Este estudio observacional retrospectivo que se llevó a cabo en 82 sujetos (47 hombres, 35 mujeres) de entre 18 y 69 años. Se evaluaron los diámetros del meato acústico externo y las estructuras relacionadas y el volumen de estas áreas. Las medias y las desviaciones estándar de la longitud del meato acústico externo (MAEL), la longitud de la parte cartilaginosa del meato acústico externo (MAEcL), la longitud de la parte ósea del meato acústico externo (MAEoL), la superficie de la parte cartilaginosa del meato acústico externo (MAEcS), la superficie de la parte ósea del meato acústico externo (MAEoS), volumen del meato acústico externo (MAEV), volumen de la parte cartilaginosa del meato acústico externo (MAEcV), volumen de la parte ósea del meato acústico externo (MAEoV), volumen de las células aéreas del proceso mastoideo (PMACV), volumen del cavum tympani (CTV) y el volumen de los espacios neumáticos temporales (OTPSV) se encontró como 23,21 ± 3,70 mm, 12,69 ± 3,72 mm, 7,80 ± 3,70 mm, 669,89 ± 107,7 mm2, 267,50 ± 30,51 mm2, 743,50 ± 119,6 mm3, 971,97 ± 156,69 mm3, 419,5. 1±48,67 mm3, 5915,93 ± 650,34 mm3, 673,48 ± 91,93 mm3, 7813,34 ± 717,49 mm3, respectivamente. En este artículo, las propiedades morfométricas y de volumen de las mediciones de las cavidades del hueso temporal fueron significativamente mayores en hombres que en mujeres. Estos resultados pueden proporcionar valores de referencia de la población sana turca y conducir a una disminución de las posibles complicaciones quirúrgicas en las regiones temporal y mastoidea.

ABSTRACT

OBJECTIVE: To evaluate the correlation between ultrasonography (US) and computed tomography (CT) in prone position to detect retrorenal colon (RRC). MATERIALS AND METHODS: In this prospective study, we included 310 patients who presented to our clinic between April 2017 and June 2019. All patients were evaluated for RRC with US and CT in prone position. Kappa compliance coefficient (κ) was used to express the compliance between CT and US outcomes. RESULTS: This study included 119 (38%) female and 191 (62%) male patients with a mean age of 44.5 ± 16.1 years. With both CT and US, RRC was detected in a total of 42 (13.5%) patients. RRC was found in 9 patients (8%) on the right side, in 32 patients (18%) on the left side and in 1 patient on both sides with CT (P = .036). However, with US, it was detected in 10 patients (8%) on the right, in 31 patients on left side and in 1 patient on both sides (P = .083). The RRC detection compliance coefficients were excellent between US and CT (κ: 0.945). CONCLUSION: US used in prone position is a suitable and practical imaging alternative to CT for detection of RRC.

ABSTRACT

This study was undertaken to determine the morphometry of the piriform aperture width and height in Turkish population aged between 18-60 years. It was a retrospective study in which 200 subjects were included 106 males and 94 females, ranging from 18 up to 60 years. Subjects having brain CT in the Radiology Department. Statistical analysis were evaluated with SPSS 21.00 programme. ANOVA Test were used to determine the significance between measurements and age group. The p<0.05 value was considered as significant. The groups were divided into four groups according to age. The overall means and standard deviations of the measurements were: piriform aperture height, 45.19±2.91 mm; piriform aperture width, 24.98±2.85 mm; the golden ratio, 1.84±0.19 in males, respectively whereas, the same measurements were 42.84±2.88; 23.46±2.15 mm; 1.83±0.19 in females, respectively. Also there were an increase in piriform aperture width measurement as the age increased. These anatomical values provides more important knowledge to determine the dimensions of these structures in clinic, surgical processes.


El estudio se realizó para determinar la morfometría del ancho y la altura de la abertura piriforme en la población turca de entre 18 y 60 años. Se llevó a cabo un análisis retrospectivo en el que se incluyeron 200 sujetos 106 hombres y 94 mujeres, entre los 18 y 60 años. Sujetos con TC cerebral en el Departamento de Radiología. El análisis estadístico se evaluó con el programa SPSS 21.00. La prueba ANOVA se utilizó para determinar la importancia entre las mediciones y el grupo de edad. El valor p <0,05 se consideró significativo. La muestra se dividió en cuatro grupos según la edad. Las medias generales y las desviaciones estándar de las mediciones fueron: altura de apertura piriforme, 45,19 ± 2,91 mm; ancho de apertura piriforme, 24,98 ± 2,85 mm; la proporción áurea, 1,84 ± 0,19 en varones, mientras que las mismas medidas fueron 42,84 ± 2,88; 23,46 ± 2,15 mm; 1,83 ± 0,19 en mujeres. También hubo un aumento en la medición del ancho de apertura piriforme a medida que la edad aumentó. Estos valores anatómicos proporcionan un conocimiento más importante para determinar las dimensiones de estas estructuras en procesos clínicos y quirúrgicos.

ABSTRACT

The aim of this study was to determine the normal values of brain ventricles and indices in healthy subjects in our population using magnetic resonance imaging (MRI) and to reveal sex and age related differences. The MRI of two hundred-sixty-five healthy individuals aged between eighteen and eighty-seven years were examined and the midsagittal and axial images were used for measurements. The measurements were performed from MRI on a Workstation. The following mean values of brain ventricles and indices were observed; frontal horn width (FHW) (33.14 mm); third (3rd) ventricle width (TVW) (3.37 mm); fourth ventricle anteroposterior width (FVWAP) (9.93 mm); fourth ventricle transverse width (FVWT) (12.40 mm); and the maximum transverse inner diameter of the skull (TIDS) (128.75 mm) in females. The same dimensions were 34.85 mm, 3.91 mm, 10.26 mm, 12.81 mm, and 134.68 mm in males, respectively. There were statistically significantly differences in the frontal horn width, third (3rd) ventricle width, and the maximum transverse inner diameter of the skull values in between sexes. The mean values of Evans' index which obtanied with maximum width between the frontal horns of the lateral ventricles divided by the maximum transverse inner diameter of the skull were found as 0.280 ±0.172 in females; whereas the same dimensions were calculated 0.276±0.161 in males. These values were lower in healthy male subjects than females, however; there were no found significantly difference between groups. Present findings obtained from MRI are necessary anatomical baseline data for interpreting pathological changes, planning surgery, and determining presence and progress of some neurological diseases.


El objetivo de este estudio fue determinar los valores normales de los ventrículos e índices cerebrales en sujetos sanos en nuestra población mediante el uso de imágenes de resonancia magnética (RM) y revelar las diferencias relacionadas con el sexo y la edad. Se examinó la resonancia magnética de 265 individuos sanos de entre 18 y 87 años, y se utilizaron las imágenes en sentido medio y sagital para las mediciones. Las mediciones se realizaron a partir de IRM en una estación de trabajo. Se observaron los siguientes valores medios de ventrículos e índices cerebrales: longitud del asta frontal (FHW) (33,14 mm); longitud del tercer ventrículo (TVW) (3,37 mm); longitud anteroposterior del cuarto ventrículo (FVWAP) (9,93 mm); longitud transversal del cuarto ventrículo (FVWT) (12,40 mm); y el diámetro transversal máximo del cráneo (SID) (128,75 mm) en las hembras. Las mismas dimensiones fueron 34,85 mm, 3,91 mm, 10,26 mm, 12,81 mm y 134,68 mm en machos, respectivamente. Hubo diferencias estadísticamente significativas en el ancho del asta frontal, el ancho del tercer ventrículo y el diámetro interno transversal máximo de los valores del cráneo entre los sexos. Los valores medios del índice de Evans que obtuvieron el ancho máximo entre los cuernos frontales de los ventrículos laterales dividido por el diámetro interno transversal máximo del cráneo se encontraron en 0,280 ± 0,172 en las mujeres; mientras que las mismas dimensiones se calcularon en hombres (0,276 ± 0,161). Sin embargo, estos valores fueron más bajos en hombres sanos que en mujeres; no se encontraron diferencias significativas entre los grupos. Los hallazgos actuales obtenidos de IRM son datos anatómicos de referencia necesarios para interpretar los cambios patológicos, planificar la cirugía y determinar la presencia y el progreso de algunas enfermedades neurológicas.

OBJECTIVE: To evaluate the feasibility and renoprotective effect of off-clamp partial nephrectomy (PN) by renal scoring systems. METHODS: After approval of the local ethics committee, the radiological and clinical data of patients with renal masses who underwent PN between January 2012 and January 2017 were evaluated in two university hospitals. Total 132 patients who underwent open surgery and off-clamp technique were included. All patients underwent contrast-enhanced computed tomography (CT) or magnetic resonance imaging (MRI) preoperatively. Preoperative demographic data, estimated glomerular filtration rate (e-GFR) and hematocrit changes, operation time, tumor volume and hospitalization time from patients were evaluated separately and statistically for each of the three scoring systems. RESULTS: Our study consisted of 132 patients with a mean age of 53.9 ± 13.9 with 69 male and 63 female. Statistically significant difference between the risk groups in RENAL and PADUA scoring were found according to tumor T stage and tumor volüme (p <0.005). Statistically significant difference was only found between risk groups of RENAL scoring system in e-GFR reduction (p<0.05). There was no statistically significant difference between the groups in the complications of all three classification systems (p > 0.005). CONCLUSIONS: In our study, all three scoring sytem successfully predicted the surgical complexity ve surgical outcomes and our results indicate that off-clamp PN has similar success and complications rates when compared to the literature. The off-clamp PN must be kept in mind to maintain postoperative renal functions eligible patients.

ABSTRACT

The purpose of this study was to evaluate the role of color Doppler sonography (CDS) in the diagnosis of extracranial vertebral artery dissections (EVADs). One hundred and fifty consecutive patients (age range 21-51 years, mean 44 years) with a clinical suspicion of vertebral artery dissection (VAD) were included in this study. All patients underwent CDS of vertebral arteries as the first-line imaging modality. Cervical T1-weighted fat-saturated axial MR images served as the gold standard. Of the 150 patients with a clinical suspicion of VAD, 27 patients were ultimately diagnosed with EVADs based on fat-saturated T1-weighted MR imaging. MR imaging was considered positive when crescentic hyperintensity (methemoglobin signal) was demonstrated at the wall of the vertebral artery. CDS was positive in 21 of these 27 patients and revealed either intramural hematoma or a dissecting membrane with two lumina. The most frequent site of involvement was the V1 to proximal V2 segment. The sensitivity, specificity, and positive and negative predictive values of CDS in the diagnosis of EVADs were 77.8, 98.4, 91.3, and 95.3%, respectively. CDS is a reliable diagnostic tool in the diagnosis of EVADs.

SUMMARY: The aim of this study was to identify sphenoid sinus dimensions; and distance between columella nasal and sphenoid sinus; and columella nasal and hypophysis in healthy adult subjects using magnetic resonance imaging (MRI) and to evaluate differences between genders and age groups. The MRI results of 300 healthy subjects (192 females; 108 males) aged 18-68 years were studied. The midsagittal and axial images were used for shape of the sphenoid sinus, and the distance measurements of its related adjacent structures on MRI. The mean values of the distance between columella nasal; and columella nasal and hypophysis; and sinus sphenoidalis width were 65.73±5.22 mm, 87.05±4.79 mm and 37.67±8.40 mm in females respectively, whereas the same values were 71.79±5.06 mm, 94.52±6.07 mm and 41.95±9.32 mm in males, respectively. The means of all measurements were lower in healthy female subjects than in healthy males.Additionally, the classification of sphenoid sinus types were determined to be postsellar type (131), sellar type (46), presellar type (14) and conchal type (1) in females, respectively. The same measurement were determined as postsellar type (82), sellar type (23) and presellar type (3) in males, respectively. Differences between sexes and age related changes were observed in the variations of the size, location and shape of sphenoid sinus were observed The observations presented in this report have defined anatomic parameters that need to be taken into consideration for reference data to determine gender discrepancies, age related changes and helpful for radiologists and clinicians to plan safe surgical approach and avoid surgical risks.


RESUMEN: El objetivo de este estudio fue identificar las dimensiones del seno esfenoidal y la distancia entre la columela nasal y el seno esfenoidal y, la glándula nasal y la hipófisis en sujetos adultos sanos con imágenes de resonancia magnética (RM), para evaluar las diferencias entre los sexos y los grupos de edad. Se estudiaron los resultados de RM de 300 sujetos sanos (192 mujeres, 108 hombres) de 18 a 68 años. Se usaron imágenes sagitales y axiales para la forma del seno esfenoidal y las mediciones de distancia de sus estructuras adyacentes relacionadas en la RM. Los valores medios de la distancia entre columela nasal; y columela nasal e hipófisis; y la anchura del seno esfenoidal fue de 65,73 ± 5,22 mm, 87,05 ± 4,79 mm y 37,67 ± 8,40 mm en mujeres, respectivamente, mientras que los mismos valores fueron de 71,79 ± 5,06 mm, 94, 52 ± 6,07 mm y 41,95 ± 9,32 mm en varones, respectivamente. Las medias de todas las mediciones fueron menores en mujeres sanas que en hombres sanos. Adicionalmente, se determinó que la clasificación del seno esfenoidal era de tipo postsellar (131), tipo selar (46), tipo presellar (14) y tipo conchal (1), en las mujeres, respectivamente. Las mismas medidas se determinaron como tipo postsellar (82), tipo sellar (23) y tipo presellar (3) en varones, respectivamente. Se observaron diferencias entre sexos y los cambios relacionados con la edad en las variaciones de tamaño, ubicación y forma del seno esfenoidal. Las observaciones presentadas en este informe establecen parámetros anatómicos que se deben considerar como referencia para determinar las diferencias de sexo, y edad. La información de estas diferencias será útil para los radiólogos y los médicos en la planificación de un abordaje quirúrgico seguro y para evitar riesgos quirúrgicos.

ABSTRACT

Paranasal sinus (PNS) foreign bodies are not common. They are usually due to penetrating trauma and iatrogenic events. On imaging, radiopaque foreign bodies can easily be detected by X-ray views of PNS. CT scan may be necessary to evaluate the exact location of foreign body in some cases. Foreign body in the PNS should be removed as early as possible. Approach and technique of its removal depend upon its size, shape, and location. Nasal endoscopic examination can be helpful for these cases. We present a pediatric girl case of penetrating FB injury inserted into the maxillary sinus towards nasopharynx in a suddenly braking car.

Objective This study was performed to assess the complication and diagnostic rates of computed tomography (CT)-guided transthoracic needle biopsy of pulmonary parenchymal and mediastinal lesions. Methods Patients who were suspected to have a malignancy based on chest imaging and CT and could not be otherwise diagnosed were evaluated. Results Sixty-five patients were included; 48 (73.8%) were male and 17 (26.2%) were female. Their average age was 58 years. The lesion size ranged from 11 to 105 mm. The most common specific histologic subtype was adenocarcinoma, and the least common was lymphoma. The diagnostic rate was 90.8%. The mean complication rate was 15.4%. Statistically significant associations were found between the complication rate and needle size and between the needle path length and lesion size. Conclusion CT-guided needle biopsy is an effective diagnostic method for patients with mediastinal and parenchymal lesions before thoracotomy. This method can also reliably differentiate benign and malignant tumors.
