ABSTRACT

Intussusception is rare in adults. We report the first known case of adult intussusception caused by splenosis. The patient had chronic gastrointestinal bleeding and intermittent abdominal pain. The diagnosis of entero-enteric intussusception was made by CT scan. A laparoscopic-assisted small bowel resection was performed, leading to resolution of the symptoms.

ABSTRACT

The importance of outcome studies in patients undergoing spinal surgery is discussed. There are numerous questionnaires used to assess quality of life results in orthopedic patients. This suggests disagreement and difficulty in the assessing and comparing outcomes. Several of these instruments for assessing outcome are reviewed. Postoperative spinal infection can prolong a patients recovery from spinal surgery and exacerbate symptoms of lower back pain. A review of the authors' patients at 3 years follow-up suggests that outcome with postoperative infections was similar to other patients with low back pain undergoing fusion without postoperative infectious complications.

ABSTRACT

The purpose of this article is to demonstrate an improved method of AO femoral nail insertion. The AO/ASIF Universal Femoral Nail (Synthes, Paoli, Pennsylvania) has proven effective in the treatment of femoral shaft fractures. The procedure is sometimes difficult when nailing in the supine position, especially with large or obese patients. The ram device impinges on the flank soft tissue of the patient. By using the curved driving piece (355.16) attached to the femoral set, the ram and the ram rod are brought away from the patient's side, thus allowing a less obstructed nail insertion.

ABSTRACT

We have studied the selectivity of the murine reticuloendothelial system in recognizing and removing from the blood model immune complexes and several aggregated proteins. Although both IgG anti-DNP model immune complexes and micro-aggregated albumin exhibited rapid hepatic uptake, which is saturable, they did not cross-inhibit each other. Aggregates of ovalbumin, a glycoprotein rich in mannose (a sugar recognized by Kupffer cells), had no effect on the uptake by the liver of either IgG immune complexes or micro-aggregated albumin. The finding that transferrin aggregates were not taken up by the liver at all suggests that aggregation alone is not sufficient to overcome the lack of a specific hepatic receptor for that protein. Thus, several mechanisms exist whereby aggregated proteins can be cleared from the blood by the reticuloendothelial system, an observation consistent with the selective failure of reticuloendothelial function found in several human diseases.
