ABSTRACT
Upon comparative evaluation of functional activity of bronchoalveolar lavage (BAL) cells and those of peripheral blood in patients with acute lung abscesses it was found that BAL cells reflect more pronounced changes in functional activity of immunocompetent cells. Investigation of immune status in acute lung abscesses demonstrated the significance of a clinical pattern of the disease and severity of purulent destruction. The relationship may be also inverse, i.e. the specific response of the immune system to antigenic loading ++pre-determines relevant form of the disease. The addition of pleural ++pyo-inflammatory side effects is probably due to phagocytic insufficiency. Functional derangements of phagocytic system are similar to those in gangrene though less distinct.

Under study were results of clinical, immunological and bacteriological examinations of 130 patients with acute abscesses of the lungs. The complex treatment included antibacterial therapy taking into account the antibiotic sensitivity of the microflora, correction of disturbances of the protein and water-salt metabolism, desintoxication measures, immunotherapy and sanitation of purulent cavities and the tracheobronchial tree.

Based on the findings of direct upper lymphography performed before and during the operation, the degree of radicality of the Patey surgery and radical mastectomy after the Holstead technic was studied in breast cancer patients. It has been shown that in technical respect the Patey operation is inferior in radicality to radical mastectomy and should be performed only on strict indications. It is believed that, it is not yet the time to use the Patey surgery in wide practice instead of radical mastectomy, since even the latter sometimes fails to provide the removal of all regional lymph nodes.
