ABSTRACT
The data on clinical course and long-term medical treatment of 215 patients with non-specific aortoarteritis are presented. Out of 180 patients who underwent antinflammatory treatment improvement was observed in 72.7%, fatal outcome occurred in 7.2%, while in the group of 35 patients who had no treatment or had irregular treatment 28.5% died. The patients were treated by steroid hormones, heparin, trentale, indocid. The treatment was long-term, sometimes up to 2 years.

The work delineates the experience in diagnoses and treatment of 230 patients with the malignant arterial hypertension syndrome that made 7.37% of all hypertensive patients treated in the USSR Research Centre of Cardiology. The analysis is presented of the data on the frequency of arising of this syndrome in different forms of symptomatic arterial hypertensions as well as essential hypertension, on characteristics of its clinical course and complications.

Results of conservative and operative treatment were evaluated in 34 patients with renovascular hypertension conditioned by non-specific aorta-arteritis with lesion of the renal arteries. There were noted late results of surgical treatment in 67% of patients in which the disease was elucidated at early stages and preliminary anti-inflammatory and hypotensive therapy was applied. A prolonged dispensary supervision for prevention and treatment of the disease exacerbation and recurrences of arterial hypertension was noted to be indicated for patients with non-specific aorta-arteritis.

Thirty patients with panarteritis of the aorta and its branches were investigated. The disease was verified by aortography, coronarography and autopsy data. On third of the patients had no clinical signs of the lesions of the coronary arteries, and ECG did not show myocardial hypertrophy. Coronaroangiography demonstrated lesions of the openings of the major coronary arteries by the type of the "burnt tree". Morphological picture and localization of the lesions were typical for the disease.

In 49 patients with panarteritis of the aorta and its branches the reaction of blast-transformation as one of the indicators of the immune system condition, of T-lymphocytes in particular, was studied. A definite significance of the lymphocytes blast-transformation reaction in confirming the autoimmune element in the pathogenesis of panarteritis of the aorta and its branches was established.

138 patients with panarteritis of the aorta and its branches were examined, and the clinical and morphological findings were compared. The disease is more widespread than has been assumed so far, and is more frequent in young women. Localization of the process in the abdominal aorta with involvement (stenosis) of renal arteries causes renovascular hypertension, which often has a malignant course, especially with an ambilateral affection of renal arteries. In the treatment, repetitive therapeutic courses with corticosteroids and heparin are of fundamental importance. With an isolated affection, especially in renovascular hypertension and disturbances of cerebral circulation, surgical treatment is also indicated.
