ABSTRACT

Introducción. La biopsia de una lesión mamaria no palpable es una intervención cada vez mas frecuente como consecuencia de las campañas para la detección precoz del cáncer de mama. Pacientes y método. Se presenta una serie de 107 biopsias de lesiones mamarias no palpables realizadas mediante la colocación preoperatoria de un arpón. Resultados. Las lesiones más frecuentes fueron las nodulares y las microcalcificaciones. En el 65,6 por ciento de los casos se empleó anestesia local y sedación intravenosa; el 53,8 por ciento de las intervenciones se realizó en régimen de cirugía mayor ambulatoria. Se detectaron 34 (31,7 por ciento) lesiones malignas, 11 (32,3 por ciento) carcinomas in situ. Un 85,2 por ciento de los tumores se diagnosticaron en estadios precoces y el 64,7 por ciento fueron subsidiarios de cirugía conservadora. El valor predictivo positivo (VPP) de la serie fue de 0,31; las lesiones mixtas y estrelladas tuvieron los VPP más altos, 0,83 y 0,50, respectivamente. Por grupos de edad, las pacientes mayores de 59 años presentaron el VPP más alto (0,40), aunque ni la edad ni el tipo de lesión presentaron diferencias estadísticamente significativas (p > 0,05). Conclusiones. En nuestra experiencia, este tipo de intervenciones permite diagnosticar carcinomas en estadios precoces subsidiarios de tratamiento conservador (AU)

ABSTRACT

A patient with a history of epigastric abdominal pain and occasional vomiting is presented. During the study of an upper gastrointestinal hemorrhage, gastroduodenal invagination secondary to a gastric lipoma of 5.5 cm in diameter was diagnosed. Upper digestive endoscopy and gastroduodenal study were not diagnostic. Echography detected a duodenal mass suspect of invagination. CAT diagnosed the lipomatous nature of the tumor. Surgery confirmed gastroduodenal invagination with a gastric lipoma with ulceration in the mucosa which covered the same. Enucleation of the tumor was performed. Histologic study established the diagnosis of gastric lipoma. The post operative period was uneventful. A review of the clinical, diagnostic and therapeutic aspects of this rare disease is reported.

ABSTRACT

Ultrasonographic findings in two patients with acute diverticulitis of the left colon are described. Echographic identification of diverticula and thickened (> 4 mm) hypoechoic bowel wall were common findings. The final diagnosis was confirmed by contrast enema and colonoscopy. Patients had clinical signs and symptoms of acute diverticulitis. Diverticula were visualized as echogenic shadowing foci in thickened bowel wall (1 case) or as outpouches of the intestinal wall in the pericolonic soft tissues (1 case). One patient had edema of the pericolic fat. Additional studies are needed to confirm the usefulness of ultrasounds in the diagnosis of colonic diverticulitis.
