ABSTRACT

PURPOSE: Pregnancy is a period in a woman's life that has important consequences on oral health, particularly for gingival health. Present study aims to identify women at higher risk of developing periodontal disease (gingivitis and periodontitis) during late pregnancy and evaluate how this condition evolves during this period. METHODS: Prospective cohort study was designed with pregnant women who were assessed during the first and third trimesters of gestation in a southern Spanish public hospital. Data regarding gingival and periodontal health, oral hygiene, and overall health status (obesity and diabetes mellitus) were collected. Reporting followed STROBE checklist. RESULTS: Significantly higher number of women had the periodontal and gingival disease in the third trimester of gestation compared with in early pregnancy. In the third trimester of gestation, 42 (28.6%) and 63 (42.9%) of women presented symptoms of periodontal disease and gingival disease, respectively. Obesity (OR 2.834; 95%CI 0.919-8.741), worse oral hygiene during the first trimester of gestation (OR: 4.031; 95%CI 2.12-7.65), and periodontal disease during early pregnancy (OR: 15.104; 95%CI 3.60-63.36) most effectively predicted periodontal disease during late pregnancy. CONCLUSIONS: Pregnancy is associated with exacerbated periodontal and gingival disease symptoms throughout the different trimesters of gestation. Obesity and oral hygiene during early pregnancy were the risk factors that most contributed to the aforementioned changes in periodontal disease.

ABSTRACT

PURPOSE: To evaluate the evolution of perceived quality of life in relation to oral health during pregnancy and to determine the risk factors involved in this process. METHODS: A follow-up study was carried out with pregnant Spanish women. Two oral examinations and an oral health-related quality of life (OHRQoL) assessment, using the OHIP-14 questionnaire, were performed in the first and third trimester of pregnancy. Data on sociodemographic characteristics, medical history, O'Sullivan Test measures, oral hygiene habits, history of caries, and periodontal status of participants were collected through structured medical-dental questionnaires. RESULTS: A complete dataset comprising 246 pregnant women was available for analysis. Overall scores for negative impacts on the OHIP questionnaire were significantly higher during late pregnancy (74%). This indicated a deterioration in oral health-related quality of life amongst participants. Items describing "painful aching", "self-consciousness", "unsatisfactory diet", and "interrupted meals" showed the greatest increase between the first and third trimester of gestation. Multivariate analysis showed that pre-gestational/gestational diabetes mellitus and poor oral hygiene habits during the first trimester of gestation were directly associated with worse oral health-related quality of life during the third trimester of gestation (hyperglycemia: OR 2.86; 95% CI 1.019-8.050: p = 0.043 / oral hygiene: OR 1.33; 95% CI 0.970-1.836; p = 0.076). CONCLUSIONS: In the present research, hyperglycemia during pregnancy and poor oral hygiene habits during the first trimester of gestation led to a higher risk of poor oral quality of life during late pregnancy.

ABSTRACT

BACKGROUND: Vitamin D has important functions outside of bone metabolism. Deficiency has been associated with several adverse outcomes during pregnancy such as preeclampsia and prematurity. There is an increasing body of literature on this topic with studies performed to date having produced contradictory results. OBJECTIVE: To synthesize the literature about vitamin D deficiency and its association with preeclampsia and prematurity in order to determine if maternal vitamin D insufficiency and/or deficiency during pregnancy is associated with the prevalence of preeclampsia and prematurity. DESIGN: A systematic review and meta-analysis of observational and interventional studies. METHODS: Two independent researchers reviewed the included studies according to PRISMA reporting guidelines. A protocol for this review was registered in PROSPERO with the registration number: "CRD42019136318". Three electronic databases (PubMed, ScienceDirect and Web of Science); were searched in order to identify eligible studies. Observational and interventional studies were selected which had been published in the last 6 years, and analysed the association between maternal vitamin D concentrations during pregnancy and the development of preeclampsia and/or preterm birth. Data were extracted and presented in tables and figures. Fixed and random-effects meta-analyses were performed on the studies which provided enough sample data to calculate odds ratios. Results from both statistical methods were compared. Meta-analysis cut-off points for vitamin D insufficiency and deficiency were defined as <75nmol/L and <50nmol/L, respectively. RESULTS: Fifty-five studies met the inclusion criteria. Fixed-effects meta-analysis of the interventional studies indicated that vitamin D supplementation acts as a prevention factor for preeclampsia and prematurity. Fixed-effects meta-analysis of observational studies concluded that vitamin D insufficiency and deficiency are associated with a higher risk of developing preeclampsia. However, prematurity and vitamin D were only associated when maternal vitamin D concentrations was <75 nmol/L. Random-effects meta-analysis found no significant association between vitamin D, preeclampsia and prematurity in either observational or interventional studies. CONCLUSION: Higher vitamin D concentrations during pregnancy could be associated with a decreased risk of preeclampsia and prematurity but statistical significance of associations depends on the study design used. Well-designed clinical trials with vitamin D supplementation are needed in order to better define associations.

INTRODUCTION: Sleep is a physiological state of self-regulation. The international classification of sleep disorders now includes as a new category those occurring during pregnancy. Regular physical activity is known to improve the quality of life, one aspect of which is sleep quality. During pregnancy, physical activity is decreased but should not be eliminated, as studies have reported a high correlation between sleep disorders and the absence of physical activity. Regular physical exercise during pregnancy, whether performed in water or out of it, provides greater control of gestational weight gain. Furthermore, the reduced weight gain during pregnancy, as a result of physical exercise, is associated with greater physical resistance to the demands of childbirth, combats the fatigue caused by pregnancy and reduces back pain. All of these outcomes tend to enhance sleep quality, among other beneficial effects. OBJECTIVE: To determine whether, in pregnant women, there is an association between moderate-intensity physical activity in an aquatic environment and sleep quality. MATERIAL AND METHODS: A randomised clinical trial was conducted with a sample of 140 pregnant women aged 21-43 years, divided into two groups; Intervention Group and Control Group. The women were recruited in the twelfth week of gestation and took part in the [Study of] Water Exercise in Pregnancy programme from week 20 to week 37. Sleep quality was evaluated in the first and third trimesters of pregnancy, using the Pittsburgh Sleep Quality Index questionnaire. RESULTS: The Mann-Whitney U test showed that the results obtained were statistically significant (p<0.05). In the Intervention Group, 44 of the women (65.67%) were classified as "poor sleepers" versus 62 women (92.54%) in the Control Group. CONCLUSIONS: The [Study of] Water Exercise in Pregnancy method improves the quality of sleep in pregnant women, both subjectively and in terms of latency, duration and efficiency.

INTRODUCTION: Obesity treatment has been the subject of much controversy; various authors have recommended the application of a comprehensive treatment programme, and in the light of this previous research, we consider the question of what is the most effective programme of physical activity to reduce overweight and obesity in children and adolescents. AIMS: To analyse major studies on the effectiveness of physical activity in reducing overweight and obesity in children and adolescents. METHOD: Systematic review of the results of physical activity programmes, published in scientific articles, to reduce overweight and obesity. Using an automated database search in PubMed and Google Scholar, conducted from October 2013 to March 2014, we identified 85 valid items. In selecting the items, the criteria applied included the usefulness and relevance of the subject matter and the credibility or experience of the research study authors. The internal and external validity of each of the articles reviewed was taken into account. CONCLUSIONS: This review confirmed the effectiveness of physical activity in reducing overweight and obesity in children and adolescents. The most effective programmes were those combining aerobic and anaerobic exercises. It is generally accepted that at least 180 minutes per week should be dedicated to exercise, in the form of three 60-minute sessions of moderate intensity. Such programmes could be sufficient for persons with overweight or obesity. Researchers in this field agree that when a diet based on an appropriate distribution of meals is combined with regular physical activity, they reinforce each other, and thus optimum results are obtained. Weight reduction programmes that take account of family involvement are more effective than nutrition education itself or other routine interventions that fail to consider family involvement. The role of pa rents and of the persons around the child or adolescent is essential to reinforce positive behaviour toward lifestyle change.


Introducción: El tratamiento contra la obesidad ha sido muy discutido y varios autores abogan por un tratamiento integral de la obesidad. Todos los referentes conducen a plantear ¿cuál es el programa de actividad física más efectivo para reducir el sobrepeso y la obesidad de niños y adolescentes? Objetivo: Analizar los principales estudios sobre la eficacia de la actividad física para reducir el sobrepeso y la obesidad de niños y adolescentes. Método: Revisión sistemática de los resultados de programas de actividad física para reducir el sobrepeso y la obesidad publicados en artículos científicos. Se identificaron 85 artículos a través de la búsqueda automatizada en las bases de datos PUBMED y Google Scholar; se llevó a cabo entre los meses de octubre de 2013 y marzo de 2014. Para seleccionar los artículos fue preciso considerar la utilidad y la relevancia del tema estudiado y la credibilidad o experiencia de los autores en la temática. Se tuvo en cuenta la validez interna y externa de cada uno de los artículos revisados. Conclusiones: Se identificó la efectividad de la actividad física para reducir el sobrepeso y la obesidad de niños y adolescentes. El programa de actividad física más efectivo es el que combina ejercicios aeróbicos y anaeróbicos. Existe consenso en acumular más de 180 minutos a la semana dedicados a estos fines, con 3 sesiones de 60 minutos cada una de ejercicio físico a la semana. De una intensidad moderada, podrían ser suficientes para ejecutar un programa de ejercicio físico para esas personas con sobrepeso y obesidad. Coinciden los autores en que, cuando se combina una dieta controlada por una adecuada distribución de las comidas y con la práctica de actividad física, se potencian mutuamente, con lo que se obtie nen los mejores resultados. Los programas de reducción de peso que tengan en cuenta la participación familiar son más eficaces que la propia educación alimentaria y otras intervenciones de rutina que no consideren esa participación familiar. El rol de los padres y de las personas que rodean al niño y al adolescente es fundamental para reforzar las conductas positivas hacia el cambio de estilo de vida.

INTRODUCTION: Regular physical activity is known to be very beneficial to health. While it is important at all stages of life, during pregnancy doubts may arise about the suitability of physical exercise, as well as the type of activity, its frequency, intensity and duration. AIMS: To analyse major studies on the influence of physical activity on maternal and foetal parameters. METHOD: Systematic review of physical activity programmes for pregnant women and the results achieved, during pregnancy, childbirth and postpartum. 45 items were identified through an automated database search in PubMed, Scopus and Google Scholar, carried out from October 2013 to March 2014. In selecting the items, the criteria applied included the usefulness and relevance of the subject matter and the credibility or experience of the research study authors. The internal and external validity of each of the articles reviewed was taken into account. CONCLUSIONS: The results of the review highlight the importance of physical activity during pregnancy, and show that the information currently available can serve as an initial benchmark for further investigation into the impact of regular physical exercise, in an aquatic environment, on maternal-foetal health.


Introducción: Las múltiples ventajas que aporta a la salud la práctica de una actividad física de forma continuada son conocidas. Aunque es importante en todas las etapas de la vida, cuando llega la gestación aparecen algunas dudas sobre la conveniencia de realizar ejercicio físico, así como del tipo de actividad, de su frecuencia, intensidad y duración. Objetivo: Analizar los principales estudios sobre la influencia de la actividad física sobre los parámetros materno- fetales. Método: Revisión sistemática de programas de actividad física destinados a las mujeres embarazadas y sus resultados durante el embarazo, el parto y el postparto. Se identificaron 45 artículos a través de la búsqueda automatizada en las bases de datos PUBMED, Scopus y Google Scholar; se llevó a cabo entre los meses de octubre de 2013 y marzo de 2014. Para seleccionar los artículos fue preciso considerar la utilidad y la relevancia del tema estudiado y la credibilidad o experiencia de los autores en la temática. Se tuvo en cuenta la validez interna y externa de cada uno de los artículos revisados. Conclusiones: Los resultados de la revisión ponen de manifiesto la importancia de la actividad física durante el periodo gestacional y se constata que la información disponible en la actualidad puede servir de referente inicial para continuar profundizando en los resultados que sobre la salud materno fetal tiene la práctica de actividad física en el medio acuático.

INTRODUCTION: There are different parameters to express the loss of weight in the treatment of overweight and obesity: absolute loss, percentage of loss, decrease in BMI, etc. OBJECTIVE: To determine the magnitudes more used in the bibliography in order to establish criteria for uniformity in the expression of those results. METHODS: a systematic review of the last five years has made at Proquest, CINHAL, Scopus, with descriptors "body fat distribution" and "diet" and "diet, reducing" and "weight loss". Articles published in English, French and Spanish were selected. Inclusion criteria were used: articles only dietary treatment of overweight in humans, and exclusion criteria: not dietary treatments, metabolic diseases, less than 50 patients and less than 8 weeks of treatment. Title, summary, methodology, results and discussion have been analysed by two researchers independently. RESULTS AND DISCUSSION: 854 items found, only 61 met the criteria. These were grouped into 5 subgroups, as they expressed the weight loss (weight or fat loss in kg; weight and fat loss in kg; weight or fat loss in %; weight loss in % and fat loss in kg or vice versa; weight and far loss in%). The results show a lack of homogeneity in the loss, being the percentage the most used expression. CONCLUSIONS: There is great heterogeneity in the expression of results of the treatments for weight loss; the diet is one of the least used tools; the analysis of clinical trials of intervention reflects a high quality in subjects older than 18 years, highlighting the lack of this type of research lines in children under 18 years. Therefore, it should standardize the magnitudes of expression of the success of these treatments and increase the lines of research on this topic.


Introduccion: Existen diferentes tratamientos para reducir el sobrepeso y la obesidad; no obstante, los resultados de los tratamientos sobre la pérdida de peso tienen una expresión muy heterogénea. Objetivo: Determinar las unidades de medida más utilizadas en los tratamientos de pérdida de peso, mediante la revisión de artículos científicos. Método: Se ha realizado una revisión sistemática de los últimos 5 años en CINHAL, Proquest y Scopus. Se han seleccionado los artículos publicados en inglés, francés y español. Los criterios de inclusión han sido: artículos de tratamiento únicamente dietético del sobrepeso en humanos. Los criterios de exclusión: tratamientos no dietéticos, enfermedades metabólicas, menos de 50 pacientes y menos de 8 semanas de tratamiento. La revisión ha sido realizada por dos investigadores independientes. Resultados y discusión: De 854 artículos, sólo 61 cumplían con los criterios establecidos. Estos se agruparon en 5 subgrupos, según expresaban la pérdida, en kilos o en porcentajes. Los resultados muestran falta de homogeneidad en la expresión de dicha pérdida. Conclusiones: Existe una gran heterogeneidad en la expresión de los resultados de los tratamientos de pérdida de peso; la dieta es una de las herramientas menos usadas; el análisis de los ensayos clínicos de intervención refleja una alta calidad en los sujetos mayores de 18 años, destacando la carencia de este tipo de líneas de investigación en los menores de esa edad. Por ello, se deberían estandarizar las magnitudes de expresión del éxito de dichos tratamientos y aumentar las líneas de investigación sobre este tema.

INTRODUCTION: The accelerometer is shown as one of the most accurate techniques in recording and saving the amount and level of physical activity, by each person in a given period of time. AIMS: This review aims to describe and analyze the main items that use this method to assess physical activity. METHODS: The review articles were identified through the following specialized Internet browser: SCOPUS, PUBMED, GOOGLE SCHOLAR, those were selected for inclusion with a total of 56 items. The validity of the articles was given by the degree of evidence demonstrated by describing the recommendations and the applicability to our context. This review has considered studies evaluating physical activity through accelerometers. RESULTS: The results show that this method can be used in ages 3 to 90 years. It can also be used in subjects with overweight/obesity, articulation injuries, Down syndrome (just children), autism and people with psychological problems. Studies in pregnant women show satisfactory results. CONCLUSION: The literature reviewed provides the accelerometer as a reliable and effective method to assess physical activity.


Introducción: La acelerometría, se muestra como una de las técnicas más fiables, en el registro y almacenamiento de la cantidad y el nivel de actividad física, realizada por cada persona y en un periodo de tiempo determinado. Objetivo: Esta revisión tiene como objetivo describir y analizar los principales artículos que utilizan este método para valorar la actividad física. Método: Los artículos seleccionados para ser incluidos en esta revisión se identificaron a través de los siguientes motores de búsqueda especializados: SCOPUS, PUBMED, GOOGLE SCHOLAR; con un total de 56 artículos. La validez de los artículos estuvo dada por el grado de evidencias demostrado, por las recomendaciones que describen y por la aplicabilidad a nuestro contexto. Esta revisión ha considerado los estudios que evalúan la actividad física a través de acelerómetros. Resultados: Los resultados demuestran que este método se puede utilizar en edades comprendidas entre 3 y 90 años. También se puede utilizar en personas con sobrepeso/ obesidad, con lesiones articulares, niños con síndrome de Down, autistas, así como personas con problemas psicológicos. Los estudios en mujeres embarazadas muestran datos fiables. Conclusión: La literatura analizada establece la acelerometría como un método eficaz para valorar la actividad física.

INTRODUCTION: Intense physical exercise provoke muscle damage, that in sedentary people can increase cardiovascular risk. Phlebodium decumanum (PD) has shown to have immunomodulator effects in models of moderate intense physical activities in well conditioned groups. To evaluate the PD effects during eccentric exercise, as a model of muscle inflammation protocol, on a sedentary population with cardiovascular risk. METHODS: This is an experimental, double-blind, multigroup randomized study. Experimental Group 1 (n = 17)received PD, 9 doses of 400 mg (total amount 3.6 g) every 8 hours during 3 days, and Control Group 2 (n = 16)received a placebo. All the subjects performed two treadmill ergoespirometry tests: first, a modified Bruce protocol to discard ischemic responses during exercise and to evaluate VO2max before the experimental phase;and second, with an eccentric protocol (14% descending ramp test) during 10 minutes in stable state at 70-80%VO2max, as experimental inflammatory protocol.We compared intra and inter groups to evaluate differences in the pre and post-test differences results on blood muscle damage variables. RESULTS: The study shown statistically significant differences in all pre-post intra-groups results in muscle damage variables (CK, LDH and Myoglobin, but not in Cardiac Troponin), and in functional lower-limb test (SJand CMJ). The comparison of inter-group results shown less muscle damage and less functional lower-limb deterioration in Group 1 compared with Control group, with statistical significance in both cases. Differences in handgrip dynamometry were no statistically significant. CONCLUSIONS: The eccentric exercise protocol in that study has proven to be a good model to induce muscle and functional damage in sedentary people. Short PD treatment has shown to reduce muscle and functional acute damages compared with placebo control group in this specific population.


Introducción: El ejercicio intenso provoca un daño muscular inflamatorio que, en sujetos sedentarios provoca un aumento del riesgo cardiovascular. El Phlebodium decumanum (PD) ha evidenciado efectos inmunomoduladores protectores frente a ese daño en los deportistas. Para conocer los efectos del PD en una población sedentaria frente al ejercicio excéntrico, y como modelo del daño muscular inflamatorio. Metodología: Se llevó a cabo un estudio experimental, doble ciego, multigrupo, randomizado, con un grupo experimental (n = 17) al que se le administró una formulación de PD (3,6 g/sujeto distribuidos en 9 dosis de 400 mg desde el 3.er día pretest), y un grupo control (n = 16) que tomó sustancia placebo. Se realizaron dos ergoespirometrías en tapiz rodante a cada participante: una previa al estudio (protocolo de Bruce modificado) para descartar signos de isquemia durante el esfuerzo y valorar el VO2max; la segunda, aplicando un protocolo excéntrico (14% de desnivel descendente), durante 10 minutos en estado estable a una intensidad entre 70-80% del VO2max individual, como protocolo experimental. Se efectuaron comparaciones intragrupo e intergrupo del porcentaje de cambio pre-postesfuerzo en variables sanguíneas y de funcionalidad muscular. Resultados: El estudio evidencia aumentos significativos de enzimas musculares MG, CPK y LDH en los dos grupos de estudio, sin cambios para la TncI, siendo significativamente menores en el grupo al que se le administró PD. Se observaron reducciones significativas de los test funcionales SJ, CMJ en ambos grupos, lo que mostró un apreciable menor descenso en el grupo PD. Se apreció una reducción del índice elástico y de la dinamomentría manual solo en el grupo control, aunque las diferencias con el grupo PD no alcanzaron una significación estadística. Conclusiones: El protocolo del ejercicio excéntrico en el presente estudio ha inducido daños musculoesqueléticos y en la funcionalidad muscular, que han resultado significativamente menores en el grupo PD, al mostrar los efectos protectores del Phlebodium Decumanum en tratamientos cortos, frente al daño muscular también en el esfuerzo agudo.

ABSTRACT

Salivary cortisol is a steroid hormone that is produced in the hypothalamic-pituitary-adrenal axis and secreted into saliva when persons are under stress. High levels of cortisol in saliva can be produced by many different factors, including obesity and certain psychological disorders. The articles selected for inclusion in this review were identified using Google Scholar and Medline, and this search obtained a total of 57 items. The validity of these studies was established according to the degree of evidence presented, by citations and by their applicability to the healthcare context in Spain. Specifically, this review takes into consideration studies of salivary cortisol and stress in children and adults, and those examining the relation between high levels of salivary cortisol and other disorders such as anxiety, attention-deficit/hyperactivity disorder, social phobia or emotional deprivation. These studies show that salivary cortisol is a clear indicator of stress in both children and adults. High levels of this hormone in saliva are associated with the following main consequences: reduced immune function, affecting healing and thus prolonging recovery time; delayed growth in children; increased blood pressure and heart rate in both children and adults.


El cortisol salival es una hormona esteoridea producida en el eje hipotálamo-pitutitario-adrenal que se segrega en la saliva, sobre todo cuando las personas se estresan. Un nivel elevado de cortisol en saliva puede ser producido por muchos factores diferentes, entre los que se encuentran algunas alteraciones, como la obesidad y otras afecciones psicológicas. Los artículos seleccionados para ser incluidos en esta revisión se tomaron de Google Scholar y Medline, y con un total de 57 artículos. La validez de los mismos viene dada por el grado de evidencias demostrado, por las recomendaciones allí recogidas y por la aplicabilidad a nuestro contexto. En esta revisión se han considerado los estudios que investigan el cortisol salival y el estrés en niños y adultos. También se han tenido en cuenta los que relacionan unos niveles altos de cortisol salival con otras alteraciones, como ansiedad, TDAH, fobia social o la carencia afectiva. Los estudios demuestran que el cortisol salival es un claro indicador de estrés, tanto en niños como en adultos. Las principales consecuencias de un alto nivel de esta hormona en la saliva son las siguientes: reducción de la función inmunológica, que afecta a la cicatrización y, como consecuencia, a un mayor tiempo de recuperación. También se relaciona con un déficit en el crecimiento físico de los niños, así como con un aumento de la presión arterial y de la frecuencia cardíaca, tanto en niños como en adultos.

UNLABELLED: In Western cultures, female breasts are strongly related to the world of sexuality and physical attractiveness, although this can vary according to the social context. OBJECTIVE: To determine the influence of social context on the body image perception of women undergoing breast cancer surgery. MATERIAL AND METHOD: An observational, descriptive and cross-sectional study was performed. Study settings were the State Oncology Center of the ISSEMyM (Social Security Institute of the State of Mexico and its Municipalities) in Toluca (Mexico) and San Cecilio University Hospital in Granada (Spain). The study sample comprised 72 mastectomized females, 30 from Mexico and 42 from Spain. Data were gathered on their socio-demographic variables, self-reported personal and family clinical histories, and scores on the validated Hopwood Body Image Scale (BIS). RESULTS: In the Spanish group of mastectomized women, 67.7% were in active employment compared with 43.3% of the Mexican group, a significant difference (p < 0.05). Body image perception was superior in women connected to the world of work and with a higher educational level. The women in a more developed social context had a significantly (p < 0.05) better body image perception. CONCLUSIONS: The social context of masectomized women affects their body image perception, which is influenced by their occupation and educational level.


El pecho de la mujer está muy relacionado en la cultura occidental con el mundo de la sexualidad y el atractivo físico, aunque puede variar en función del contexto. Objetivo: Determinar la influencia del contexto social en la percepción de la imagen corporal de las mujeres intervenidas de cáncer de mama. Material y método: Se llevó a cabo un estudio observacional, descriptivo y transversal. Los escenarios del estudio estuvieron constituidos por el Centro Oncológico Estatal del ISSEMyM en la ciudad de Toluca (México), y el Hospital Clínico San Cecilio de la ciudad de Granada (España). La totalidad del universo estuvo formado por 72 mujeres mastectomizadas. De ellas, 30 correspondieron a México y 42 a España. Se recogieron datos de variables sociodemográficas y las mujeres respondieron a preguntas sobre su historia clínica personal y familiar. Se aplicó la Escala validada BIS (Body Image Scale) de Hopwood. Resultados: El 67,7% mujeres mastectomizadas españolas se encuentran activas laboralmente en comparación al 43,3% de las mujeres mexicanas. Diferencia estadísticamente significativas en los dos grupos (p < 0,05). En la medida en que las mujeres se vinculan al mundo laboral e incrementan su nivel de escolaridad, la aceptación de la imagen corporal muestra mejores resultados. Las mujeres que viven en contextos sociales desarrollados tienen una mejor percepción de su imagen corporal. Con una diferencia significativa de (p < 0,05). Conclusiones: El contexto social influye en la percepción de la imagen corporal de las mujeres intervenidas de cáncer de mama. La ocupación laboral y el grado de escolaridad fueron determinantes de la percepción de la misma.

BACKGROUND: Maternal age is a preponderant variable in the epidemiological analysis of the premature birth. Studies show that in the extreme ages of the maternal life there is a risk of premature birth that generates a high rate of neonatal morbidity. OBJECTIVES: [corrected] Determine the effect on the extreme ages of women residents in the province of Alicante on the total of the premature births. METHOD: An explanatory, retrospective case-control study was conducted during the period from January 1st, 2008 to December 31st, 2011. The study was based on the revision of the newborn registers from the Neonatal Screening Center of the province of Alicante. All the preterm were included, this means between 22 & 36 complete weeks of pregnancy (5,295 out of 78,391 newborn which represents 6.75% of prematurity), and a random sample of the deliveries with 37 weeks or more of pregnancy (control group). The age of the mother was studied as independent variable and the prematurity as dependent variable. RESULTS: Clearly shows an increased risk of prematurity among teenage mothers compared to the age group nearest to them, which is confirmed by a squared Chi test which gives a significantly different distribution (p < 0,0001) and an OD for very preterm of 2,41 (1,51-3,24) and of preterm of 1,71 (1,32-2,19). This probability is also higher among mothers over 40 years old with an OD of 1,86 (1,39-2,48) and 1,66 (1,44-1,91) for very preterm newborns and preterm newborns respectively. DISCUSSION: The results clearly manifest that teenagers and older pregnant mothers are at higher prematurity and low birth weight risk, therefore imposes the need to trace educational interventions to minimize this problem from the results in this research.


Antecedentes: La edad materna es una variable preponderante en el análisis epidemiológico del nacimiento prematuro. Los estudios muestran que en las edades extremas de la vida materna hay un riesgo de nacimiento prematuro que genera una tasa elevada de morbilidad neonatal. Objetivos: Determinar el efecto de las edades extremas de mujeres residentes en la provincia de Albacete sobre de nacimientos prematuros. Método: Se realizó un estudio explicativo, retrospectivo de casos-control durante el periodo de 1 de enero de 2008 a 31 de diciembre de 2011. El estudio se basó en la revisión de los registros de recién nacidos del Centro de Cribado Neonatal de la provincia de Alicante. Se incluyeron todos los recién nacidos pretérmino, es decir, entre las semanas 22 y 36 de gestación (5.295 de 78.391 recién nacidos, lo que representa un 6,75% de prematuridad), y una muestra aleatoria de los nacimientos con más de 37 semanas de gestación (grupo control). Se estudió la edad materna como variable independiente y la prematuridad como variable dependiente. Resultados: El estudio muestra claramente un mayor riesgo de prematuridad entre las madres adolescentes en comparación con el siguiente grupo más cercano en edad, lo que se confirmó por una distribución significativamente distinta con el test Chi cuadrado (p < 0,0001) con una OD para los grandes prematuros de 2,41 (1,51-3,24) y de 1,71 (1,32-2,19) para los prematuros. Esta probabilidad también fue mayor en madres con edades de más de 40 años con una OD de 1,86 (1,39-2,48) y de 1,66 (1,44- 1,91) para recién nacidos grandes prematuros y prematuros, respectivamente. Discusión: Los resultados muestran claramente que las madres adolescentes y las más mayores tienen mayor riesgo de hijos prematuros o con bajo peso al nacimiento, lo que motiva la necesidad de establecer intervenciones educacionales para disminuir el problema, a partir de los resultados de esta investigación.

Recent studies show an alarming increase in the rate of overweight / obesity among the infant - juvenile population. Obesity in childhood is associated with a significant number of complications, such as sleep apnea syndrome, insulin resistance and type 2 diabetes, hypertension, cardiovascular disease and some cancers. It is estimated that the prevalence of sleep apnea in children is 2-3% in the general population, while in obese adolescents, varies between 13% and 66%, according to various studies. It is associated with impairment of neurocognitive function, behavior, cardiovascular system, metabolic disorders and growth. Sleep apnea is a serious public health problem that increases when children and adolescents are overweight or obese. We hypothesize that aerobic endurance exercise can be an effective treatment for obesity and apnea at the same time. The aim of this study was to determine the influence of physical activity in children and adolescents with overweight / obesity in sleep apnea. An observational, descriptive, prospective, longitudinal study will be carried out in children with sleep apnea and obesity. The universe will be made up of 60 children and adolescents aged between 10 and 18 years, attending the endocrinology service for suffering of obesity in the Hospital Clinico San Cecilio of Granada during the period September 2012-September 2013. The smple will consist of children and adolescents that meet these characteristics and to whom their parents/tutors have authorized through the informed consent. Sleep apnea in children wil be measured by polysomnography and sleep quality questionnaire. There will also be a nutritional assessment by a food frequency questionnaire and an anthropometric assessment. Among the expected results are the lower overweight and obesity in children through the physical activity program. To reduce apnea and to improve sleep quality.


Estudios recientes muestran un incremento alarmante en la tasa de sobrepeso/obesidad entre la población infanto - juvenil. La obesidad en la infancia se asocia con un importante número de complicaciones, como síndrome de apnea del sueño insulinorresistencia y diabetes tipo 2, hipertensión, enfermedad cardiovascular, algunos tipos de cáncer. Se estima que la prevalencia de apnea en niños es de un 2-3% en la población general, mientras que, en adolescentes obesos, varía entre el 13% y el 66%, según distintos estudios. Se asocia con la afectación de la función neurocognitiva, comportamiento, sistema cardiovascular, alteraciones metabólicas y del crecimiento. La apnea del sueño es un grave problema de salud pública que aumenta cuando los niños y adolescentes padecen sobrepeso y obesidad. Se postula el ejercicio de resistencia aeróbica como un tratamiento efectivo para la obesidad y la apnea de forma conjunta. El objetivo de este estudio es conocer si la actividad física en niños con sobrepeso/obesidad disminuye la apnea del sueño. Se realizará un estudio observacional, descriptivo, prospectivo, longitudinal con niños que padecen apnea del sueño y obesidad. El universo estará constituido por 60 niños y adolescentes con edades comprendidas entre 10 y 18 años que acudan a la consulta de endocrinología por tener obesidad en el Hospital Clínico San Cecilio de Granada durante el período de septiembre 2012-septiembre 2013. La muestra estará formada por niños y adolescentes que cumplan estas características y que sus padres/tutores hayan autorizado a través del consentimiento informado. La apnea del sueño se medirá en los niños mediante una polisomnografía y un cuestionario de calidad del sueño. También se hará una valoración nutricional a través de un cuestionario de frecuencia de consumo alimentario y una valoración antropométrica. De entre los resultados esperados están el bajar el sobrepeso y obesidad en los niños mediante el programa de actividad física. Disminuir la apnea y mejorar la calidad del sueño.

ABSTRACT

Different studies4, 5 relate that stress increases in children and adolescents with overweight and obesity, and consequently their salivary physiological responses (AEA salivary alpha-amylase, cortisol, cytokines, leptin), so in this study we relate these two parameters to see their progress through a program of physical activity. If we manage to reduce overweight or obesity, these physiological responses and stress should also be reduced, thus improving the overall health status of these children and adolescents. The overall objective of the study was to determine the influence of physical activity in obese children and adolescents in perceived stress. An observational, descriptive, prospective and longitudinal study will be carried out. The universe is made up of 60 overweight / obese children and adolescents aged between 10 and 18 years. The assessment will take place from September 2012 to September 2013. To collect saliva samples, the ELISA8 method will be used. Variables such as BMI, lifestyle and diet will also be collected Among the expected results are to lower overweight and obesity in children through physical activity program. To reduce stress and to normalize physiological salivary parameters.


Diferentes estudios1,2 relacionan que el estrés aumenta en los niños y adolescentes con sobrepeso y obesidad, y en consecuencia sus respuestas fisiológicas salivales (Alfaamilasa salivar AEA, Cortisol, Citoquinas, Leptina), por eso en este estudio queremos relacionar estos dos parámetros para ver su evolución a través de un programa de Actividad Física. Si logramos reducir el sobrepeso u obesidad se debería reducir también estas respuestas fisiológicas y el estrés, por lo que mejoraría el estado de salud general de estos niños y adolescentes. El objetivo general del estudio es conocer la influencia de la actividad física, en niños y adolescentes obesos, en el estrés percibido. Se realizará un estudio observacional, descriptivo, prospectivo y longitudinal. El universo estará constituido por 60 niños y adolescentes con edades comprendidas entre 10 y 18 con sobrepeso/obesidad. La valoración tendrá lugar desde septiembre de 2012 hasta septiembre de 2013. Para la recogida de las muestras de saliva se utilizará el método ELISA3. También se recogerán variables como el IMC, hábitos de vida y alimentación. De entre los resultados esperados están el bajar el sobrepeso y obesidad en los niños mediante el programa de actividad física. Disminuir el estrés fisiológico y normalizar los parámetros salivares.

INTRODUCTION: The satisfaction's analysis is being used as an instrument to create different sanitary reforms to improve the quality and numerous studies aim to the increase the mother's satisfaction directly related to the maternity care. OBJECTIVES: [corrected] Identify the woman satisfaction's degree about birth attention, accompaniment during nativity and the breastfeeding's term. MATERIAL AND METHOD: [corrected] Descriptive transversal study in the university hospital San Cecilio in Granada (España), during the time of August 2011 to 2012, it performed with a second prospective tracing phase to a N = 60 mothers. It used a protocol (Annex 1) after 24 hours in hospital and at 14 days by telephone. After 3 months, it performed a tracing pertaining to the baby food. RESULTS: The global satisfaction's level about birth is high in study population. It has been shown that breastfeeding (P = 0,514) and vaginal birth without epidural (P = 0,320) creates higher satisfaction for mother. On the other hand, birth satisfaction related with duration of breastfeeding. CONCLUSION: Satisfactory mothers' opinion related with birth care and accompaniment during nativity increases in women whose birth happened in a uncomplicated way without epidural and they started early breastfeeding.


Introducción: El análisis de la satisfacción se está utilizando como instrumento para crear diferentes reformas sanitarias para la mejora de la calidad y numerosos estudios apuntan al incremento de la satisfacción de la madre en relación directa con el cuidado en la maternidad. Objetivos: Identificar el grado de satisfacción de la mujer sobre la atención al parto, el acompañamiento durante el nacimiento y la duración de la lactancia materna. Material y método: Estudio descriptivo transversal en el Hospital Universitario San Cecilio de Granada (España), durante el periodo de tiempo Agosto del 2011 al 2012, se realizo con una segunda fase de seguimiento prospectivo a una N = 60 madres. Se utilizó un protocolo (Anexo 1) a las 24 horas en el hospital y a los 14 días por teléfono. A los 3 meses, se realizó un seguimiento relacionado con la alimentación del bebe. Resultados: El nivel de satisfacción global sobre el parto es alto en la población estudiada. Se ha demostrado que la lactancia materna (P = 0,514) y el parto vía vaginal sin epidural (P = 0,320) crea una mayor satisfacción en la madre. Por otro lado, la satisfacción sobre el parto guarda relación con la duración de la lactancia materna. Conclusión: La opinión satisfactoria de las madres relacionadas con la atención al parto y el acompañamiento durante el nacimiento se incrementa en aquellas mujeres cuyo parto ocurrió de forma eutócica sin epidural e iniciaron la lactancia materna precoz.

ABSTRACT

The sleep apnea-hypopnea syndrome (SAHS) is a disorder that currently affects a large number of children and adolescents. The aim of this review is to assess the factors causing this condition and the risk of suffering another disease associated with SAHS. The 50 articles selected for inclusion in this review were identified through GOOGLE SCHOLAR. The validity of the items was established by the degree of evidence obtained, by recommendations made in this respect and by the applicability to the situation observed. The review considers studies of SAHS in children and adolescents, taking into account those relating this disorder with obesity, hypertension, physical activity and other variables. The studies reviewed show that SAHS is associated with childhood obesity and that it increases the risk of cerebro-cardiovascular disease. It is also shown that SAHS decreases children's and adolescents' physical capacity and quality of life. Psychological aspects are also affected, impacting on academic performance, which is poorer than in healthy children. Therefore, early paediatric diagnosis of SAHS is important in order to prevent associated disorders.


El síndrome de apneas-hipoapneas del sueño, en lo adelante (SAHS) es una alteración que actualmente afecta a un número elevado de niños y adolescentes. Esta revisión tiene como objetivo evaluar los factores que generan esta alteración y el riesgo de padecer otro tipo de enfermedad asociada al SAHS. Los artículos seleccionados para ser incluidos en esta revisión se identificaron a través de GOOGLE SCHOLAR con un total de 50 artículos. La validez de los artículos estuvo dada por el grado de evidencias demostrado, por las recomendaciones que dejan y por la aplicabilidad a nuestro contexto. Esta revisión ha considerado los estudios que investigan el SAHS en niños y adolescentes. También se han tenido en cuenta los que relacionan esta alteración con la obesidad, la hipertensión arterial, la actividad física y otras variables. Los estudios demuestran que el SAHS está relacionado con la obesidad infantil y aumenta el riesgo de enfermedades cerebrocardiovascular. También se evidencia que disminuye la capacidad física y la calidad de vida de niños y adolescentes. Los aspectos psicológicos también se ven afectados, lo que influye en el rendimiento académico, que es inferior al de los niños sanos. Así pues, es importante un diagnóstico precoz del SAHS pediátrico, de modo que se eviten las alteraciones asociadas.

ABSTRACT

Recent studies show an alarming increase in levels of overweight and obesity among children and adolescents. The main objectives of this research were the following: (i) to carry out an anthropometric evaluation of the nutritional status and body composition of school children in the city and province of Granada; (ii) to compare the nutritional status of this population sample with national and international reference standards. The results obtained in this study showed that the general prevalence of overweight in both sexes was 22.03% and that 9.12% of the children were obese. Statistically significant differences were found between the variable, weight for age and sex (p < 0.05) and the variable, height for age and sex (p < 0.05). Regarding the body mass index, no statistically significant differences were found for the variable, sex (p = 0.182). This contrasted with the variable, age, which did show statistically significant differences (p < 0.05). As a conclusion, the results of our study highlighted the fact that these anthropometric values were much higher than national and international reference standards.

ABSTRACT

Human obesity is a disorder of multifactorial origin in which genetic and environmental factors are involved. To understand the mechanisms regulating energy intake and fat accumulation in the body, it is important to study the genetic alterations causing monogenic obesity. Most of the genes involved in monogenic obesity are associated with the leptin-melanocortin system; hence the importance of studying this system by analysing natural mutations in mice. Previous studies have described mutations in leptin and its receptor, proopiomelanocortin and prohormone convertase 1 associated with human obesity of monogenic origin. The aim of this study is to provide an updated review of the main characteristics and functioning of the leptin-melanocortin system, and its implications and potentialities in regulating food intake and body weight.

ABSTRACT

There is a general consensus that obesity is an eminently inflammatory process. This is justified by alterations observed in obese patients, which affect the secretion of certain cytokines such as ceruloplasmin, leptin, adiponectin, and interleukin-6 (IL-6), among others. In a parallel way, other research has also pointed out alterations in the composition of fatty acids in the erythrocyte membrane of overweight adults. The results obtained in our study confirm the existence of a significant correlation between the serum levels of some of the cytokines studied and the nutritional state of the sample studied. This means that for the population of children evaluated in our study, the serum concentrations of these biomolecules can be an important tool for the prediction of cardiovascular risk when they become adults. Furthermore significant differences were found regarding the composition of saturated fatty acids in the erythrocyte membrane.
