ABSTRACT

Introduction and importance: Hydatid cyst disease is a zoonotic disease caused by Echinococcus granulosus and is recognized as a significant health issue in many countries, particularly Mediterranean countries. Hydatid cysts in the musculoskeletal system are rare, with only a few reported cases. These usually occur as secondary cysts resulting from the hematogenous dissemination from primary sites. Case presentation: This paper reports a case of a 77-year-old man with a mass in his thigh that had been increasing dramatically in size for 4 months with no signs of local inflammation, fever, or any other symptoms. Findings from an MRI were consistent with a large abscess, so a true-cut biopsy was taken before referral. A pathological study after surgery revealed the lesion was a hydatid cyst, and a previously performed biopsy caused a rupture of the cyst. Clinical discussion: Misdiagnosing hydatid cysts, especially in non-usual areas, may lead to an unwelcome biopsy and consequences. Conclusion: This case report highlights the importance of considering hydatid cysts when encountering any enlarging mass, regardless of its location or rate of growth.

ABSTRACT

Efficient and reliable data routing is critical in Advanced Metering Infrastructure (AMI) within Smart Grids, dictating the overall network performance and resilience. This paper introduces Q-RPL, a novel Q-learning-based Routing Protocol designed to enhance routing decisions in AMI deployments based on wireless mesh technologies. Q-RPL leverages the principles of Reinforcement Learning (RL) to dynamically select optimal next-hop forwarding candidates, adapting to changing network conditions. The protocol operates on top of the standard IPv6 Routing Protocol for Low-Power and Lossy Networks (RPL), integrating it with intelligent decision-making capabilities. Through extensive simulations carried out in real map scenarios, Q-RPL demonstrates a significant improvement in key performance metrics such as packet delivery ratio, end-to-end delay, and compliant factor compared to the standard RPL implementation and other benchmark algorithms found in the literature. The adaptability and robustness of Q-RPL mark a significant advancement in the evolution of routing protocols for Smart Grid AMI, promising enhanced efficiency and reliability for future intelligent energy systems. The findings of this study also underscore the potential of Reinforcement Learning to improve networking protocols.

ABSTRACT

Rhinosporidiosis is one of the granulomatous diseases endemic in Asia, Africa, and Europe, with Southern India and Sri Lanka having the greatest prevalence rates. It is typically understood to affect the upper respiratory system. Involvement of the lungs beyond the trachea is infrequent as compared to the upper respiratory tract. We revealed an uncommon case of disseminated rhinosporidiosis in a diabetic patient, who initially presented with shortness of breath associated with cough and fever. Two months prior to these symptoms, he was having oral ulcer and dysphagia and, subsequently, loss of weight. Chest radiograph and CT thorax revealed military nodules with multiple suppurative neck and mediastinal lymphadenopathy and bilateral adrenal lesions. He was initially investigated for tuberculosis, metastatic malignancy, or lymphangitic carcinomatosis before a biopsy revealed Rhinosporidiosis. Hence, histopathological or laboratory evidence is frequently crucial to back up imaging concerns so the appropriate treatment can be given.

Introduction and importance: Tuberculosis (TB) has been one of the most devastating diseases to humanity in recent decades; although pulmonary infection is the most common, infection of any other organ is familiar as well. Colon cancer is another disease affecting the gastrointestinal (GI) system and mostly targets people over 50. Only a few studies mentioned the co-existence of cancer and TB occurring at the same place and time. Hence, the authors report a rare case of concurrent ascending colon adenocarcinoma and colonic TB. Case presentation: A 49 -year-old man presented to our clinic with constipation and abdominal pain. Two colonoscopies were performed, and two biopsies were taken; the first one showed granulomatous inflammation consistent with TB, and the second one showed low-grade adenocarcinoma. Computed tomography showed annular thickening of the ascending colon with infiltrates around the lesions. A right hemicolectomy was performed, and the final pathology confirmed adenocarcinoma grade II and extensive TB granulomas involving the colon into the serosa and the lymph nodes. Anti-TB medications were administered after surgery. Clinical discussion: Due to appropriate diagnostic methods, TB and cancer were detected at an early stage. In our treatment protocol, no adjuvant chemotherapy was applied after surgery due to the possibility of drug interaction with anti-TB medications. Conclusion: The two diseases may co-exist; thus, diagnosing them may not be the easiest, not to mention the lack of a clear treatment protocol in case of their accompany.

Over the last decade, the notion of community resilience, which encompasses planning for, opposing, absorbing, and quickly recovering from disruptive occurrences, has gained momentum across the world. Critical Infrastructures (CI) are seen as critical to attaining success in today's densely populated countries. Such infrastructures must be robust in the face of multi-hazard catastrophes by implementing appropriate disaster management and recovery plans. Given these facts, it is critical to establish a new methodological perspective with an integrated system for effective disaster management of CI, as well as an intelligent application that will aid in the construction of more resilient and sustainable cities and communities. This perspective proposes a holistic gaming scenario application for assessing the vulnerability and accessibility of critical infrastructures during multi-hazard events, with a primary focus on conducting an integrated assessment for critical infrastructures and their assets. ABSTRACT

Agrichemical adjuvants that combine a highly selective, efficient, and active mode of operation are critically needed to realize a more sustainable approach to their usage. Herein, we report the synthesis and full characterization of two new metal-organic frameworks (MOFs), termed UPMOF-1 and UPMOF-2, that were constructed from eco-friendly Ca2+ ions and naturally occurring, low-molecular weight plant acids, l-malic and d-tartaric acid, respectively. Upon structural elucidation of both MOFs, a widely used fungicide, hexaconazole (Hex), was loaded on the structures, reaching binding affinities of -5.0 and -3.5 kcal mol-1 and loading capacities of 63% and 62% for Hex@UPMOF-1 and Hex@UPMOF-2, respectively, as a result of the formation of stable host-guest interactions. Given the framework chemistry of the MOFs and their predisposition to disassembly under relevant agricultural conditions, the sustained release kinetics were determined to show nearly quantitative release (98% and 95% for Hex@UPMOF-1 and Hex@UPMOF-2, respectively) after >500 h, a release profile drastically different than the control (>80% release in 24 h), from which the high efficiency of these new systems was established. To confirm their high selectivity and activity, in vitro and in vivo studies were performed to illustrate the abilities of Hex@UPMOF-1 and Hex@UPMOF-2 to combat the known aggressive pathogen Ganoderma boninense that causes basal stem rot disease in oil palm. Accordingly, at an extremely low concentration of 0.05 µg mL-1, both Hex@UPMOF-1 and Hex@UPMOF-2 were demonstrated to completely inhibit (100%) G. boninense growth, and during a 26 week in vivo nursery trial, the progression of basal stem rot infection was completely halted upon treatment with Hex@UPMOF-1 and Hex@UPMOF-2 and seedling growth was accelerated given the additional nutrients supplied via the disassembly of the MOFs. This study represents a significant step forward in the design of adjuvants to support the environmentally responsible use of agrichemical crop protection.

ABSTRACT

The polyacrylamide/gelatin-iron lanthanum oxide (P-G-ILO nanohybrid) was fabricated by the free radical grafting co-polymerization technique in the presence of N,N-methylenebisacrylamide (MBA) as cross linker and ammonium persulfate (APS) as initiator. The P-G-ILO nanohybrid was characterized by the various spectroscopic and microscopic techniques that provided the information regarding the crystalline behavior, surface area, and pore size. The response surface methodology was utilized for the statistical observation of diclofenac (DF) adsorption from the wastewater. The adsorption capacity (qe, mg/g) of P-G-ILO nanohybrid was higher (254, 256, and 258 mg/g) than the ILO nanoparticle (239, 234, and 233 mg/g). The Freundlich isotherm model was the best fitted, as it gives the higher values of correlation coefficient (R2 = 0.982, 0.991 and 0.981) and lower value of standard error of estimate (SEE = 6.30, 4.42 and 6.52), which suggested the multilayered adsorption of DF over the designed P-G-ILO nanohybrid and followed the pseudo second order kinetic model (PSO kinetic model) adsorption. The thermodynamic study reveals that adsorption was spontaneous and endothermic in nature and randomness onto the P-G-ILO nanohybrids surface increases after the DF adsorption. The mechanism of adsorption of DF demonstrated that the adsorption was mainly due to the electrostatic interaction, hydrogen bonding, and dipole interaction. P-G-ILO nanohybrid was reusable for up to five adsorption/desorption cycles.

BACKGROUND: Even with the mild form of COVID-19, people need to practice the proper preventive measures to achieve health, safety and control spread of infection. Few studies assessed sound disinfection and rehabilitative knowledge. This study aims to assess the self-reported, knowledge of specific disinfection measures among post-recovery COVID-19 patients and identifying the most requested knowledge items regarding the prevention and post COVID rehabilitation measures. METHODS: This is an exploratory cross-sectional study using an electronically open survey. A pre-tested e-questionnaire was employed for data assembling. The sample size was calculated and a total of 417 people completed the questionnaire. Knowledge score was calculated for preventive and disinfection measures during and after COVID 19 infection. It comprised three sections: socio-demographics, study participants' knowledge regarding precautions, disinfection, and rehabilitation measures, as well as sources of knowledge about COVID-19. RESULTS: All participants infected with COVID-19 (82%) reported self-isolation for ten days after confirming the infection, with only 18% required hospitalization. Regarding the information needed by the participants, the highest requests were for the rehabilitation information after COVID-19 and preventive measures. Females under 30 years old and those with a college education or higher were significantly more likely to request rehabilitation information after COVID-19 (P-value 0.05). Nevertheless, males were significantly more interested in information regarding preventive measures; They were over 30 years old and had education below university (P-value 0.05). Participants (above 30 years old) had significantly higher knowledge of preventive and disinfection measures during and after COVID-19 infection (P = 0.030). CONCLUSION: After the COVID-19 experience, most participants demonstrated a great desire for rehabilitation information and proper preventive measures. This paves the way for delivering self-management and rehabilitation knowledge and emphasizing the significance of various prevention modalities.

ABSTRACT

The fabrication of a zinc hydroxide nitrate-sodium dodecylsulfate bispyribac modified with multi-walled carbon nanotube (ZHN-SDS-BP/MWCNT) paste electrode for uric acid and bisphenol A detection was presented in this study. Electrochemical impedance spectroscopy, chronocoulometry, square-wave voltammetry, and cyclic voltammetry were all used to examine the electrocatalytic activities of modified paste electrodes. The modified electrode's sensitivity and selectivity have been considered in terms of the composition of the modifier in percentages, the types of supporting electrolytes used, the pH of the electrolyte, and square-wave voltammetry parameters like frequency, pulse size, and step increment. Square-wave voltammetry is performed by applying a small amplitude square-wave voltage to a scanning potential from -0.3 V to +1.0 V, demonstrating a quick response time and high sensitivity. The ZHN-SDS-BP/MWCNT sensor demonstrated a linear range for uric acid and bisphenol A from 5.0 µM to 0.7 mM, with a limit of detection of 0.4 µM and 0.8 µM, respectively, with good reproducibility, repeatability, and stability as well. The modified paste electrode was successfully used in the determination of uric acid and bisphenol A in samples of human urine and lake water.

ABSTRACT

After amputation, granular hemocytes infiltrate the blastema of regenerating cephalic tentacles of the freshwater snail Pomacea canaliculata. Here, the circulating phagocytic hemocytes were chemically depleted by injecting the snails with clodronate liposomes, and the effects on the cephalic tentacle regeneration onset and on Pc-Hemocyanin, Pc-transglutaminase (Pc-TG) and Pc-Allograft Inflammatory Factor-1 (Pc-AIF-1) gene expressions were investigated. Flow cytometry analysis demonstrated that clodronate liposomes targeted large circulating hemocytes, resulting in a transient decrease in their number. Corresponding with the phagocyte depletion, tentacle regeneration onset was halted, and it resumed at the expected pace when clodronate liposome effects were no longer visible. In addition to the regeneration progress, the expressions of Pc-Hemocyanin, Pc-TG, and Pc-AIF-1, which are markers of hemocyte-mediated functions like oxygen transport and immunity, clotting, and inflammation, were modified. After the injection of clodronate liposomes, a specific computer-assisted image analysis protocol still evidenced the presence of granular hemocytes in the tentacle blastema. This is consistent with reports indicating the large and agranular hemocyte population as the most represented among the professional phagocytes of P. canaliculata and with the hypothesis that different hemocyte morphologies could exert diverse biological functions, as it has been observed in other invertebrates.

ABSTRACT

Any defect in the early phases of the normal complex embryogenic pathway might lead to anatomical anomalies that lead to the formation of ectopic thyroid gland tissue. Overall, ectopic thyroid tissue incidence is one in 300 000, and the rate of malignant transformation among those is merely 1%. However, documented cases of malignant transformation of ectopic thyroid tissue in the tonsils have not yet been - to the best of the knowledge - documented in published literature. Case Presentation: We illustrate the case of a 58-year-old female who was referred to the clinic after she underwent a tonsillectomy due to chronic discomfort and gradual difficulty in swallowing. Documentation of an ectopic primary Papillary Thyroid Carcinoma in one of the patient's excised tonsils was achieved after thorough histopathological and immunohistochemical analyses took place. Radiological evaluation excluded any metastatic involvement and paved the path toward surgical intervention, which was comprised of a total thyroidectomy. Clinical Discussion: Surgical resection of the patient's thyroid gland was achieved, and the subsequent histopathological analysis of the specimens revealed nodular hyperplasia with degenerative changes but without evidence of malignant transformation in her thyroid gland. Conclusion: Ectopic primary Papillary Thyroid Carcinoma is an immensely rare occurrence, regardless of population parameters. Its incidence could originate in various anatomical locations, but to the best of the knowledge, its incidence in the tonsils has not been previously documented in published literature. Adequate levels of clinical awareness in this situation could timely alleviate patient complaints and aid in performing optimal life-saving interventions.

Numerous analytical approaches have been developed to determine histamine levels in food samples due to its health consequences. Consuming histamine over the Food and Drug Administration (FDA)-regulated 50 mg kg-1 limit would result in chronic toxicity. Consequently, the present study discusses a novel electrochemical approach to evaluate histamine levels in fish products via a molecularly imprinted polymer (MIP) on an electrode surface. The film was produced with electropolymerized polyurethane (PU), which maintained the histamine compound. Fourier-transform infrared (FTIR) spectroscopy was applied to verify the MIP manufactured in this study. The capability of the polymer was measured by assessing its electron shifts with cyclic voltammetry (CV) and electrochemical impedance spectroscopy (EIS). Differential pulse voltammetry (DPV) was also employed to validate the sensing method. The MIP/screen-printed electrode (SPE) and non-imprinted polymer (NIP)/SPE recorded a linear response ranging from 1 to 1000 nmol L-1 at the 1.765 and 709 nmol L-1 detection limits. The sensing technique was subsequently utilized to determine the histamine levels in selected samples at room temperature (25 °C). Generally, the sensor allowed the accurate and precise detection of histamine in the fish samples. Furthermore, the approach could be categorized as a simple technique that is low-cost and suitable for on-site detections.

ABSTRACT

The toxicity of histamine has attracted numerous researchers to develop a method for histamine determination purposes. The Food and Drug Administration (FDA) unequivocally prohibits the consumption of histamine above 50 mg·kg-1. Thus, an innovation in histamine detection in fish has been developed in this research. The investigation of the histamine level in fish has been conducted by using an electrochemical sensor approach and producing a polymer via molecularly imprinted polymer (MIP) on a screen-printed electrode. The technique was validated by assessing the shifts in electron shifting using the cyclic voltammetry (CV) approach and electrochemical impedance spectroscopy (EIS), whereas differential pulse voltammetry (DPV) was applied to validate the sensor method. The instruments showed a linear response ranging from 1-1000 nmol·L-1, with a detection limit of MIP/SPE at 1.765 nmol·L-1 and 709 nmol·L-1 for the NIP/SPE, respectively. The sensing technique was employed to determine the histamine level in selected samples at room temperature (25°C). The outcomes of this study indicated that the validated chemical sensor allowed accurate and precise detection of fish samples and can be categorized as a simple approach. The instrument is inexpensive and suitable for on-site detection.

ABSTRACT

3-Monochloropropane-1,2-diol esters (3-MCPDE) are food contaminants commonly found in refined vegetable oils and fats, which have possible carcinogenic implications in humans. To investigate this clinically, we conducted an occurrence level analysis on eight categories of retail and cooked food commonly consumed in Malaysia. This was used to estimate the daily exposure level, through a questionnaire-based case-control study involving 77 subjects with renal cancer, with 80 matching controls. Adjusted Odds Ratio (AOR) was calculated using the multiple logistic regression model adjusted for confounding factors. A pooled estimate of total 3-MCPDE intake per day was compared between both groups, to assess exposure and disease outcome. Among the food categories analysed, vegetable fats and oils recorded the highest occurrence levels (mean: 1.91 ± 1.90 mg/kg), significantly more than all other food categories (p < .05). Risk estimation found the Chinese ethnic group to be five times more likely to develop renal cancer compared to Malays (AOR = 5.15, p = .001). However, an inverse association was observed as the 3-MCPDE exposure among the Malays (median: 0.162 ± 0.229 mg/day/person) were found to be significantly higher than the Chinese (p = .001). There was no significant difference (p = .405) in 3-MCPDE intake between the cases (median: 0.115 ± 0.137 mg/day/person) and controls (median: 0.105 ± 0.151 mg/day/person), with no association between high intake of 3-MCPDE and the development of renal cancer (OR = 1.41, 95% CI: 0.5091-2.5553). Thus, there was insufficient clinical evidence to suggest that this contaminant contributes to the development of renal malignancies in humans through dietary consumption. Further research is necessary to support these findings, which could have significant public health ramifications for the improvement of dietary practices and food safety measures.

ABSTRACT

Piezoelectric microelectromechanical system (piezo-MEMS)-based mass sensors including the piezoelectric microcantilevers, surface acoustic waves (SAW), quartz crystal microbalance (QCM), piezoelectric micromachined ultrasonic transducer (PMUT), and film bulk acoustic wave resonators (FBAR) are highlighted as suitable candidates for highly sensitive gas detection application. This paper presents the piezo-MEMS gas sensors' characteristics such as their miniaturized structure, the capability of integration with readout circuit, and fabrication feasibility using multiuser technologies. The development of the piezoelectric MEMS gas sensors is investigated for the application of low-level concentration gas molecules detection. In this work, the various types of gas sensors based on piezoelectricity are investigated extensively including their operating principle, besides their material parameters as well as the critical design parameters, the device structures, and their sensing materials including the polymers, carbon, metal-organic framework, and graphene.

ABSTRACT Coronavirus disease (COVID-19) is an infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Its main form of transmission is through respiratory droplets. Case reports have described the presence of this virus in biological materials such as blood, feces, urine, and tears, which generate hypotheses about other means thereby the disease is transmitted. In this report, we describe a case of SARS-CoV-2 identified on the eye surface of an asymptomatic health-care professional. The nasopharyngeal reverse transcription polymerase chain reaction test, using a sample collected on the same day, and the serological test, performed 3 months later, did not reveal any evidence of SARS-CoV-2 infection. These results alert on the possibility of a false-positive reverse transcription polymerase chain reaction result for the ocular surface or the presence of the virus in the conjunctival mucosa in individuals without infection.


RESUMO A COVID-19 é uma doença infeciosa causada pelo SARS-CoV-2, sendo sua principal forma de transmissão através de gotículas respiratórias. Já existem relatos de caso descrevendo a presença desse vírus em materiais biológicos como sangue, fezes, urina e lágrima, o que gera hipóteses sobre outros meios de transmissão da doença. Neste estudo, descrevemos um caso de identificação do vírus SARS-CoV-2 na superfície ocular de um profissional de saúde assintomático. A transcrição inversa da reação em cadeia da polimerase da nasofaringe, coletada no mesmo dia, e o teste sorológico, realizado três meses após, não detectaram qualquer evidência de infecção pelo SARS-CoV-2. Esses dados alertam para a possibilidade de resultado falso positivo da transcrição inversa da reação em cadeia da polimerase da superfície ocular ou a presença do vírus na mucosa conjuntival sem infecção.

Purpose: To report a case of multiple bilateral retinal pigment epithelial detachments (PEDs) in a woman with systemic lupus erythematosus (SLE). Methods: Case Report. Results: A 28-year-old female with mild blurred bilateral vision in both eyes (OU) without pain or any other symptom was admitted to the hospital due to worsening renal function and uncontrolled high blood pressure (HBP). Best-corrected visual acuity (BCVA) was 20/30 and 20/40, right and left eyes, respectively. She had SLE, glucose-6-phosphate dehydrogenase deficiency, and immune thrombocytopenic purpura. BP was over 150/90 mmHg for more than 1.5 years, and she used corticosteroids at varying doses for more than 4 years. During hospitalization, she was taking prednisone 60 mg daily as Class IV lupus nephritis was diagnosed. On fundoscopy, she had a lacy retinal pattern, remarkably on the macula in OU. Spectral-domain optical coherence tomography revealed multiple bilateral serous PEDs and pachychoroid. Angiofluoresceinography displayed multiple pooling hyperfluorescence areas. Six months afterward, while she was on prednisolone 10 mg daily, and antihypertensive medications, BCVA was improved to 20/25 OU. Nevertheless, she had no retinal or choroidal changes. Her findings could be related to SLE choroidopathy, central serous chorioretinopathy-like disease, and/or hypertensive choroidopathy. Conclusions: Ocular involvement affects nearly one-third of SLE patients. The findings are variable and can include nearly any part of the eyeball. Multiple bilateral PEDs have been described in the literature; however, in this case, it is probably multifactorial and not only related to SLE.

ABSTRACT

Tuberculosis is a top 10 leading cause of death worldwide. Lungs are primarily involved organs in tuberculosis. The rest of cases are extrapulmonary tuberculosis (14% reported in 2017). Extrapulmonary tuberculosis always presents with non-specific symptoms, thus at risk of delay diagnosis and management. In genitourinary tuberculosis, kidney alone and kidney with urinary bladder or ureter is affected in more than 70% of cases. The ureter and urinary bladder infections are almost always secondary to tuberculous involvement of the kidney. Bacilli haematogenic spreading is a known transmission pathway to the kidney. In this case, we diagnosed isolated urinary bladder tuberculosis caused by direct gastrointestinal tuberculosis infiltration, a rare occurrence of extrapulmonary tuberculosis. We illustrate the multiorgan involvement in tuberculosis infection including pulmonary, gastrointestinal, peritoneal and urinary bladder.

ABSTRACT

Seismic resilience index (SRI) approach describes the post-seismic recovery phase of a single building by incorporating the time dimension. In recent decades, numerous quantitative frameworks have been developed to quantify earthquake resilience. The seismic resilience of existing reinforced concrete (RC) structures in Malaysia under far-field (FF) and near-field (NF) earthquake scenarios is examined in this research methodology using a seismic resilience index (SRI) and functionality curve. SRI is implemented to a four-story RC school building and a five-story RC hospital structure damaged by the 2015 Ranau earthquake. In addition, a total of 25 existing RC structures have been selected as target case studies for evaluating the SRI and presenting the results graphically using the Geographical Information System (GIS) platform. These buildings are primarily divided into three clusters: low, mid, and high rise. Non-linear dynamic analysis (NL-DA) is utilized in this approach to analyze the case studies and the selected buildings using the Finite Element platform (FE). Moreover, a group of seismic records encompassing of far-field and near-field are selected for simulation process using non-linear time history analysis (NL-THA). Eventually, Incremental Dynamic Analysis, fragility and vulnerability assessment, functionality assessment, time of recovery (TREC), loss of resilience (LOR), robustness (R) and SRI indicator are all part of this approach.•SRI helps decision-makers create or suggest repair or retrofitting solutions with acceptable recovery time and cost.•The consequences of failures for people's lives, properties, as well as the economy, will be mitigated.•Improve the sustainability and serviceability of structures to ensure seismic safety and meet Sustainable Development Goal 11.

ABSTRACT

In this work, a composite strain sensor is fabricated by synthesizing MXene and deposition of polypyrrole on top of the flexible electrospun PVDF nanofibers. The fabricated sensor exhibits a conductive network constructed with MXene and polypyrrole of microcracks network structure, demonstrating its strain sensing properties. The presence of these microcracks serves as mechanical weak points, which leads to sensitivity enhancement, while the electrospun fiber substrate act as a cushion for strain loading under large deformations. The as-prepared MXene@Polypyrrole PVDF sensor has a gauge factor range of 78-355 with a sensing range between 0-100%. Besides strain deformations, the sensor can operate in torsional deformation and human motion, indicating the sensor's potential as a wearable health monitoring device.
