ABSTRACT

INTRODUCTION: Dental caries are a limiting factor in maintaining dental and medical readiness in the military. Untreated dental caries can lead to dire health consequences. Consistent and comprehensive access to dental care is often limited due to the intensive operational demands on our nation's warfighters. The standard of care for dental caries is a surgical model where diseased tooth tissue is surgically removed and restored with appropriate restorative materials. While effective, it is not practical in the military operational environment, especially under time constraints. Dental restoratives offer military personnel a simple and preventive treatment of dental caries and are suitable as self-applied first aids. The purpose of this study was to measure the shear bond strengths of two dental restorative materials to human teeth paired with two different fluoride treatments and the hardness and biofilm formation on teeth after applying the fluoride varnishes. MATERIALS AND METHODS: Specimens were made of human molar teeth treated with each of the following four materials: glass ionomer cement GC Fuji II LC Capsules, Filtek Z250, Riva Star steps 1 and 2, or Mark3 NaF varnish. Step 1 of Riva Star consists of silver diamine fluoride and step 2 contains potassium iodide. On human molar slabs, 10 circular specimens of 5 cm in diameter were prepared with restoratives according to manufacturer procedures. Etch-Rite and a proprietary aluminum chloride-based cavity conditioner were used as etchants on tooth surfaces for the Filtek Z250 and glass ionomer cement, respectively. After at least 24 hours underwater, each assembly was removed, and the shear bond strength of the adhesive was measured according to International Organization for Standardization (ISO) 29022.The hardness was measured according to ISO 14233. Hardness measurements were performed before varnish application, then after storage in an incubator at 37 °C for 4 hours in a demineralization solution (pH = 4.5), and after 1 day in a mineralization solution (pH = 7). A crystal violet staining assay was used to measure biofilm formation of Streptococcus mutans bacteria on human molar teeth after the application of fluoride varnish. RESULTS: We report a 16% increase in shear bond strength of the Filtek Z250/Riva Star coupled treatment compared to the Filtek Z250/Mark3 NaF coupled treatment. We also demonstrate a significant 84% decrease in bond strength with a GC Fuji II LC/Mark3 NaF treatment compared to control (P = .0002), while Riva Star remains statistically unchanged. Enamel and dentinal hardness are significantly improved when Riva Star is applied compared to NaF varnish. A 25%-35% (P < .0001) decrease in oral biofilm formation was observed on samples where a Riva Star or NaF varnish was applied. CONCLUSIONS: Mechanical and antimicrobial testing indicated Riva Star, compared favorably with and in some cases, performed better in the laboratory than a Mark3 NaF varnish. Hardness measurements indicated Riva Star is more effective in dentin tubule occlusion compared to NaF varnish. Our findings help provide practical suggestions to dental treatment, particularly to the unique dental environments seen in the military. Riva Star may be used as an adjunctive treatment prior to placing a final restoration. This study supports the use of Riva Star in conjunction with GC Fuji II LC or Filtek Z250 restorative materials, making it a promising treatment in military dental applications.

OBJECTIVES: To investigate canine retraction (CR) and anchorage loss (AL) among average facial height (AFH) and high facial height subjects (HFH) with or without piezocision surgery (PS). MATERIALS AND METHODS: This was a split-mouth, randomized clinical trial. Twenty-three females (aged 19.05 ± 2.95 years) who presented with Class II division I malocclusion requiring bilateral maxillary extraction and who fulfilled eligibility criteria were included and categorized into two groups: AFH (12 participants) and HFH (11 participants). Atraumatic extractions were performed 10 weeks following bonding. Before space closure, impressions were taken to fabricate models, which were scanned to generate digital models. Each participant had PS on the randomly assigned side. Space closure was undertaken using 100-g nickel-titanium coil closing springs on 0.019 × 0.025-inch stainless steel archwire. Digital models were collected 6 and 12 weeks post-PS. They were superimposed using reliable reference points and a region of interest on the palate, and crown movements were analyzed in three dimensions. RESULTS: Three months post-PS, intergroup comparisons showed that rates of CR for control sides (mean = 1.88 ± 0.83 mm for AFH, mean = 1.76 ± 0.62 mm for HFH) and intervention sides (mean = 1.48 ± 0.74 mm for AFH, mean = 1.40 ± 0.85 mm for HFH) were not significantly different. AL was not significantly different (P > .05) between groups. CONCLUSION: Regardless of whether the patient underwent PS, CR and AL rates for AFH and HFH patients were not significantly different.

Mycobacterium abscessus is a ubiquitous rapid-growing mycobacterium that belongs to a diverse group of non-tuberculous mycobacteria. It can cause frequent and serious infections in patients with or without risk factors. This article reports a case of disseminated M. abscessus in an immunocompetent Omani male with native joint septic arthritis.

ABSTRACT

Real protein interaction network (PIN) is dynamic. Researchers created dynamic PIN by combining static PIN with gene expression data to explain the dynamicity evolution of protein interactions. However, all available approaches failed to recognize low- or high-expression proteins as active proteins. Therefore, determining an adequate threshold is one of the system's biological challenges. In this study, a quartile one (q-one) method is proposed to determine the active time points for each protein according to its expression value's features and construct dynamic protein interaction networks (DPINs) in which a protein is added to the network if it is active in two successive time points. This leads to reduce the number of DPINs to half. The efficiency of the q-one approach and three-sigma (3-sigma) method in detecting protein complexes is evaluated using Markov cluster method, clique percolation method, and ClusterONE algorithms. In most cases, q-one outperforms the 3-sigma method in recall, precision and F-measure. This is in addition to its ability to reveal the dynamicity within the protein-protein interaction network and identify essential proteins.

ABSTRACT

Context: Hereditary hypophosphatemic rickets with hypercalciuria (HHRH) is a rare autosomal recessive disorder, which is characterized by renal phosphate wasting, hypercalciuria, increased 1,25-dihydroxyvitamin D, and decreased parathormone (PTH) levels. Objective: Here we report different clinical features of two siblings with HHRH, confirmed with molecular diagnosis. Subjects and methods: 16.4 years old boy (P1), and 8.7 years old girl (P2) were referred to our outpatient clinic due to clinical suspicion of metabolic bone diseases. Results: P1 had severe hypophosphatemia. Additionally, PTH concentration was near to the lower limit, 1,25-dihydroxyvitamin-D concentration was near to the upper limit. P2 had relatively milder clinical and laboratory findings. Bilateral renal calculi were detected on ultrasound in both of them. HHRH was suspected due to their described biochemistry and the presence of bilateral renal calculi. Molecular analysis of SLC34A3 gene revealed a homozygous variant c.756G>A (p.Gln252=) and a splice donor variant c.1335+2T>A. After oral phosphate treatment, clinical and biochemical improvements were observed. However treatment nonadherence of patients was a barrier to reach treatment goal. Conclusion: The clinical phenotype due to the same mutation in the SLC34A3 gene may vary even among the members of the same family. An accurate diagnosis is important for the appropriate treatment.

OBJECTIVES: Turner Syndrome (TS) is associated with a high risk of cardiac anomalies and cardiovascular disease. We aimed to evaluate patients with TS (n=33) for cardiac and aortic pathology using thorax magnetic resonance angiography (MRA). SUBJECTS AND METHODS: Clinical findings, karyotypes, echocardiogram (ECHO) findings and thorax MRA results were evaluated. Aortic dimensions were measured and standard Z scores of aortic diameters along with aortic size index (ASI) were calculated. RESULTS: Mean age of the patients was 13.7±3.4 years. MRA revealed cardiovascular pathology in 10 patients (30%). CoA (n=4), aberrant right subclavian artery (n=3), dilatation of the ascending aorta (n=1), tortuosity of the descending aorta (n=1) and fusiform dilatation of the left subclavian artery (n=1) were found. Two of the four patients with CoA found on MRA were detected with ECHO. Mean diameter of the sinotubular junction was found to be elevated [mean±SD: 2.4±1.5]. Z scores for the diameters of the isthmus, ascending aorta and descending aorta were in normal ranges. 45,X patients were found to have significantly higher ASI values than non 45,X patients (p=0.036). CONCLUSION: Our findings indicate that patients with TS should be evaluated with MR imaging studies in addition to ECHO to reveal additional subtle cardiac and vascular anomalies. CoA which is very distally located or which has mild nature may not be seen by ECHO. The increase in ASI observed in 45,X patients may herald the development of life-threatening complications. Therefore, frequent follow-up is warranted in these patients.

In this work we have tried to prepare Ni and Ag doped ZnO nanopowders using the sol gel technique. The influence of Ni and Ag (1, 3 and 5 mol.%) on the crystalline structure and optical properties of ZnO was investigated. The samples were characterized by XRD, FTIR and UV-visible spectrophotometer. XRD patterns confirmed the wurtzite formation of doped and undoped ZnO nanopowders. The average crystallite sizes of the prepared samples found from XRD were 19 nm for undoped ZnO, from 17 to 22 nm for Ni-ZnO and from 19 to 26 nm for Ag-ZnO. The average crystallite size of Ag-ZnO increased with increasing Ag contents. Different optical properties of Ni-ZnO and Ag-ZnO nanopowders were observed for different Ni and Ag content. The band gaps of Ni-ZnO and Ag-ZnO nanopowders were lower than that of the undoped ZnO (3.1 eV). The band gaps of Ag-ZnO were lower than that of Ni-ZnO. The optical properties of ZnO were enhanced by Ni (mol.%) in the UV region and by Ag (3 and 5 mol.%) in the visible region.

ABSTRACT

Defining protein complexes in the cell is important for learning about cellular processes mechanisms as they perform many of the molecular functions in these processes. Most of the proposed algorithms predict a complex as a dense area in a Protein-Protein Interaction (PPI) network. Others, on the other hand, weight the network using gene expression or geneontology (GO). These approaches, however, eliminate the proteins and their edges that offer no gene expression data. This can lead to the loss of important topological relations. Therefore, in this study, a method based on the Gene Expression and Core-Attachment (GECA) approach was proposed for addressing these limitations. GECA is a new technique to identify core proteins using common neighbor techniques and biological information. Moreover, GECA improves the attachment technique by adding the proteins that have low closeness but high similarity to the gene expression of the core proteins. GECA has been compared with several existing methods and proved in most datasets to be able to achieve the highest F-measure. The evaluation of complexes predicted by GECA shows high biological significance.

ABSTRACT

Soft porous nanocrystals with a pronounced shape-memory effect exhibit two- to three-fold increase in elastic modulus compared to the microcrystalline counterpart as determined by atomic force microscopy nanoindentation. The increase in rigidity is consistent with the known shape-memory effect displayed by the framework solid at the nanoscale. Crystal downsizing can offer new avenues for tailoring the mechanical properties of metal-organic frameworks.

ABSTRACT

Ageing is considered to be the major and non-modifiable risk factor for the development of hypertension and cardiovascular diseases. During ageing, the vascular system undergoes structural and functional alterations, including endothelial dysfunction, thickening of the vascular wall, reduced distensibility and increased arterial stiffness. Vascular rigidity results from fibrosis and remodelling of the extracellular matrix, processes that are associated with ageing and are amplified in hypertension. These events may be induced by vasoactive agents, such as angiotensin II, endothelin-1, and aldosterone, which are increased in the vasculature during aging and hypertension. Complex interaction between the process of ageing and prohypertensive factors results in accelerated vascular remodelling and fibrosis, as well as increased arterial stiffness. Hypertension accelerates and augments age-related vascular remodelling and dysfunction, and ageing may impact on the severity of vascular damage in hypertension, thus strongly suggesting close interactions between biological ageing and blood pressure elevation. Molecular and cellular mechanisms underlying vascular alterations in ageing and hypertension are common and include aberrant signal transduction, oxidative stress and activation of pro-inflammatory and pro-fibrotic transcription factors. Strategies to suppress age-associated vascular changes can ameliorate vascular damage associated with hypertension. This article looks into vascular alterations occurring during ageing and hypertension, focussing particularly on arterial stiffness and vascular remodelling, also emphasizing the importance of diagnostic methods.

ABSTRACT

The phenomenon of political liberalism and its genesis and evolution are explored. This phenomenon is studied using interrelated methodological principles at the crossroad of political philosophy, political theory, and international relations theory. The author analyzes the discourse of the "decline of Europe" and, more broadly, of the Western-centric world through the prism of changes in the forms of classical liberalism that have become part of political ideology. Special attention is paid to political neoliberalism, which from a once dynamic system of principles and values has turned into a dogma and is associated with the least popular aspects of the hyperphase of globalization. The elements of the weakening of Westcentrism are described, including the strategic decoupling of the United States and its European allies. The topic of the militarization of thinking and foreign policy of some countries is discussed. The author holds that the degradation of political neoliberalism clears the way to the search for a new balance between the ideological traditions of liberalism, conservatism, and collectivism.

ABSTRACT

BACKGROUND: Peripheral nerve interfaces have emerged as alternative solutions for a variety of therapeutic and performance improvement applications. The Defense Advanced Research Projects Agency (DARPA) has widely invested in these interfaces to provide motor control and sensory feedback to prosthetic limbs, identify non-pharmacological interventions to treat disease, and facilitate neuromodulation to accelerate learning or improve performance on cognitive, sensory, or motor tasks. In this commentary, we highlight some of the design considerations for optimizing peripheral nerve interfaces depending on the application space. We also discuss the ethical considerations that accompany these advances.

ABSTRACT

Size engineering is an emerging strategy to modulate the mechanical properties of crystalline materials. Herein, micro- and nanodimensional single crystals of the prototypical metal-organic framework (MOF) ZIF-8 are generated using solvothermal and solution methods, respectively. Atomic force microscopy-based nanoindentation technique was used to measure the Young's modulus values of micro- and nanodimensional individual ZIF-8 crystals. We demonstrate that crystal downsizing to nanoscale dimensions results in a 40% reduction in crystal stiffness. The change is attributed to a greater contribution of surface effects to the physical properties of nanocrystalline ZIF-8. The observed change in the mechanical properties may be used to explain reported size-dependent changes in gas adsorption of ZIF-8, thought to be a result of differences in framework flexibility at the nanoscale. Our work provides an important example on how downsizing of crystalline metal-organic materials can give rise to specific and tunable physical properties.

ABSTRACT

OBJECTIVE: Thyroid fine-needle aspiration (FNA) and cytology is a reliable diagnostic method used in the assessment of malignancy when evaluating thyroid nodules, in conjunction with clinical and ultrasonographic findings. The aim of this study is to compare clinical, ultrasonographic, cytological and histopathological findings in children who underwent thyroid FNA. METHODS: Subjects comprised 80 patients (52 female) aged 13.7±2.8 years at the time of FNA who where evaluated for thyroid nodules. Clinical, ultrasonographic and cytological findings of patients were evaluated retrospectively. RESULTS: Autoimmune thyroiditis was present in 30% and history of radiotherapy to the head or neck in 10%. The cytological diagnosis of patients included: inadequate or hemorrhagic sample in 10%; benign in 42.5%; atypia or follicular lesion of undetermined significance (AUS/FLUS) in 15%; suspicion of follicular neoplasia (SFN) in 7.5%; suspicion of malignancy (SM) in 8.8%; and malignant in 16.3%. Thirty-seven patients underwent thyroidectomy. Malignancy rates for histopathologic follow-up were 75%, 85.7% and 100% for SFN, SM and malignant categories, respectively. Only one benign and two AUS/FLUS FNAs were found to be malignant on histopathological examination. Among patients who had received radioiodinetherapy, 87.5% had malignancy. In this study, the sensitivity of FNA was 96%, specificity 50%, positive predictive value 90.9%, negative predictive value 75%, and diagnostic value of FNA was 89.2%. CONCLUSION: Thyroid FNA results were highly compatible with histopathological examination. Sensitivity, positive predictive value and diagnostic value of FNA were high.

BACKGROUND: Extranodal NK/T-cell lymphoma, nasal type (ENKTCL) has a high prevalence in Asia and Latin American countries, such as Mexico, where it encompasses 40% of all T-cell non-Hodgkin lymphomas. Historically, responses to anthracycline-based therapies have been disappointing. Since data about the effectiveness of L-asparaginase-based regimens in Mexico are limited, we compared both therapies in our center. METHODS: We performed a retrospective cohort of patients with newly diagnosed ENKTCL, who were divided into two groups for treatment and analysis (group 1: L-asparaginase-based regimen and group 2: anthracycline-based regimen) between 2001 and 2016. RESULTS: Of 36 patients with newly-diagnosed ENKTCL, 33 received at least one cycle of chemotherapy (22 in group 1 and 11 in group 2). Over a median follow-up interval of 17 months (range, 0-167), a complete response (CR) was observed in 45.5% of patients in group 1, compared to 27% of group 2 (P=0.45). Progression was more frequently observed in group 2 than in group 1 (54.5% vs. 18.4%, P=0.04). The median overall survival (OS) was 44 months in group 1, compared to 5 months in group 2 (P=0.012). The multivariate analysis showed that failure to achieve a CR after first-line therapy was the only significant factor for OS (HR, 3.04; 95% CI, 1.4-6.5; P=0.005). CONCLUSION: L-asparaginase-based regimens for patients with newly-diagnosed ENKTCL confer a survival advantage over anthracycline-based regimens.

ABSTRACT

Studies of the primary cultures of granulocytes, mononuclear, and monocyte-macrophage cells derived from human blood were performed using variola virus (VARV) in the doses of 0.001-0.021 PFU/cell (plaques-forming units per cell). Positive dynamics of the virus accumulation was observed only in the monocyte-macrophages with maximum values of virus concentration (5.0-5.5 Ig PFU/ml) mainly within six days after the infection. The fact of VARV replication in the monocyte-macrophages was confirmed by the data of electron microscopy. At the same time, virus vaccines when tested in doses 3.3 and 4.2 Ig PFU/ml did not show the ability to reproduce in these human cells. The people sensitivity to VARV as assessed from the data obtained on human monocyte-macrophages corresponded to -1 PFU (taking into account the smooth interaction of the virus in the body to the cells of this type), which is consistent to previously found theoretical data on the virus sensitivity. The human susceptibility to VARV assessed experimentally can be used to predict the adequacy of developed smallpox models (in vivo) based on susceptible animals. This is necessary for reliable assessment of the efficiency of development of drugs for treatment and prophylaxis of the smallpox.

ABSTRACT

Mice of the ICR outbred population were infected intranasally (i/n) with the variola virus (VARV, strain Ind-3a). Clinical signs of the disease did not appear even at the maximum possible dose of the virus 5.2 lg PFU/head (plaque-forming units per head). In this case, 50% infective dose (ID50) of VARV estimated by the presence or absence of the virus in the lungs three days after infection (p.i.) was equal to 2.7 ± 0.4 lg PFU/head. Taking into account the 10% application of the virus in the lungs during the intranasal infection of the mice, it was adequate to 1.7 lg PFU/lungs. This indicates a high infectivity of the VARV for mice comparable to its infectivity for humans. After the i/n infection of mice with the VARV at a dose 30 ID50/ head the highest concentration of the virus detected in the lungs (4.9 ± 0.0 lg PFU/ml of homogenate) and in nasal cavity tissues (4.8 ± 0.0 lg PFU/ml) were observed. The pathomorphological changes in the respiratory organs of the mice infected with the VARV appeared at 3-5 days p.i., and the VARV reproduction noted in the epithelial cells and macrophages were noticed. When the preparations ST-246 and NIOCH-14 were administered orally at a dose of 60 µg/g of mouse weight up to one day before infection, after 2 hours, 1 and 2 days p.i., the VARV reproduction in the lungs after 3 days p.i. decreased by an order of magnitude. Thus, outbred ICR mice infected with the VARV can be used as a laboratory model of the smallpox when evaluating the therapeutic and prophylactic efficacy of the antismallpox drugs.
