ABSTRACT

INTRODUCTION: Nurses are considered as one of the members of family physician teams, therefore it is important to explain the role of nurses in it. The aim of study was to determination of nurses' roles in the family physician team. METHODS: This research was conducted as a cross sectional descriptive analytical study. The study population was consisted of all the nurses who were working in under the supervision of Semnan University of Medical Sciences. The participants included 150 nurses who were selected via random stratified sampling method. The research instrument was a researcher-made questionnaire. RESULTS: Of all the participants, 30 persons (20%) were male and the rest were female (80%). The findings of the study reported based on the mean score of five. The collected data indicated that the nurses participating in the study, as a member of family physician team, had a tendency to take part in the following activities: support (4.44), care (4.37), management (4.05), counseling (3.93), prevention (3.81), and education (3.78). CONCLUSION: The results of the study showed that the nurses were more interested in playing a role in supporting and caring for patients, rather than participating in education and preventive activities. Since family physician teams must mainly provide community-based preventive services, the authorities should try to enhance the interests and capabilities of nurses to empower them to play a role in preventive and education activities.

ABSTRACT

INTRODUCTION: New technologies, including health information technologies, play an important role in effectiveness of management and nursing care services. AIM: This study was aimed to determining the use of health information technology in patient care management in a case study in Iran. METHODS: This Mixed method study was conducted in 2018 in Kowsar Hospital of Semnan, Iran. Data gathered by an observational checklist and one questionnaire included two main parts, one demographic and another assessment of information technology use in care management of inpatients. The researcher prepared the questionnaire and its validity was verified. The data were organized and analyzed in the form of a descriptive analytic report. In the process of data collection, 10 participants including nurses, head nurses, physicians, radiology experts and Information Technology (IT) managers were interviewed and data analyzed using Directed Content Analysis. RESULTS: Nurses were satisfied with the computerized system and believed it can facilitate the affair. From the nurse's viewpoint, the most common use of the Health Information Technology (HIT) were access (observation) of patients admit and discharge information (100%), providing medicine and equipment, transfer of patients (92.3%). The least of them were retrieve of evidence in the care process (0 %) and judgment and analysis of radiological diagnostic procedures (0%). The potential of electronic record is not still applicable. CONCLUSION: Use of modern information and communication technology in hospitals facilitates access and transfer of information, and also accelerates patient's admission and discharge process, relation between hospital units, providing medical equipment supporting affairs' process and diagnostic procedures. However, modifying organizational policies, improve the infrastructure and enhancing nurses' motivation in documenting nursing reports can be effective in increasing the impact of information technology in care management processes especially in electronic record and nurse's clinical judgment and evidence-based care.

ABSTRACT

BACKGROUND: Community-oriented nursing care is an important model of nursing care. Counseling and Nursing Care Service Centers (CNCSCs) have been providing these private services to the Iranian community for nearly two decades. Resource management, cost-benefit analysis and affordability are important steps in providing these services. The present study was conducted to explore the challenges of financial transactions between CNCSCs and their clients. METHODS: This study has a qualitative design and was conducted on a total of 30 participants, consisting of CNCSC managers, staff, physicians and clients who were selected through purposive theoretical sampling. Data were collected through in-depth interviews and direct observations and were analyzed using conventional qualitative content analysis. RESULTS: The analysis of the data led to the extraction of three main categories, including the flaunted atmosphere due to direct financial transaction, instability in determining tariffs for nursing services and the use of strategies for cost-effective services and client satisfaction. CONCLUSION: To increase affordability and satisfaction and expand private community-based nursing. Services, appropriate financial policies should be designed and applied that can lead to transparent and simple financial transactions with the clients by way of indirect monetary exchanges. These policies should be designed in a systematic manner with integrity, facilitate inter-sectorial cooperation in the health sector and be cost-effective for the clients, insurance companies and the health system.
