ABSTRACT

Introducción. La posibilidad de emplear una piel fácilmenteplegable ha permitido reconstruir defectos de la cavidad oral consiguiendouna gran funcionalidad. Aunque el colgajo radial es el colgajo que se utilizacon más frecuencia para reconstruir defectos de superficie de la cavidad oral,el colgajo lateral de brazo puede ser de elección en algunas situaciones.Objetivos. El objetivo del presente trabajo es mostrar las ventajas e inconvenientesy nuestras indicaciones y resultados del colgajo lateral de brazo enreconstrucción de defectos de la cavidad oral.Material y método. Se trata de un estudio prospectivo sobre la utilización delcolgajo lateral de brazo en la reconstrucción de defectos de la cavidad oraltras cirugía ablativa. Se ha valorado: la viabilidad del colgajo, la morbilidaddel lecho donante, la longitud del pedículo, la selección de vasos receptores,las complicaciones y los resultados funcionales de la zona reconstruida.Resultados. Hemos utilizado el colgajo lateral de brazo en 10 pacientes enreconstrucciones primarias tras cirugía ablativa por carcinoma epidermoidede la cavidad oral. Hubo un caso de necrosis por trombosis venosa. El defectodonante se cerró en 8 casos de modo directo y en 2 con un injerto librede espesor parcial. La longitud media del pedículo ha sido de 8,75 cm. En 9casos el resultado funcional de los pacientes ha sido satisfactorio.Conclusiones. El colgajo fasciocutáneo lateral de brazo permite la reconstrucciónde la cavidad oral consiguiendo buenos resultados funcionales. Ademásla morbilidad de la zona donante es mínima y puede realizarse cierredirecto del defecto cutáneo del brazo en la mayoría de los casos(AU)


Introduction. The availability of easily pliable skin hasallowed the functional reconstruction of oral cavity defects. Althoughthe radial forearm free flap is the most frequently used flap for thereconstruction of surface defects of the oral cavity, the lateral armfree flap may be preferable in some situations.Objectives. The aim of the present paper is to show the advantagesand disadvantages and our indications and results for the lateralarm flap in intraoral reconstruction.Material and methods. This is a prospective work on the use of thelateral arm free flap for the reconstruction of oral cavity defects afterablative surgery. The parameters that have been evaluated are: flapviability, morbidity in the donor site, length of the pedicle, selectionof recipient vessels, complications and functional results in thereconstructed area.Results. The lateral arm flap has been used in primary reconstructionafter ablative surgery for squamous cell carcinoma of the oral cavityin ten patients. One flap was lost because of venous thrombosis.The donor site was repaired by direct closure in 8 cases and a splitthickness skin graft had to be used in 2 cases. Mean pedicle lengthwas 8.75 cm. In 9 cases a favorable functional result was achieved.Conclusions. Fasciocutaneous lateral arm flap allows thereconstruction of oral cavity defects achieving good functional results.Morbidity in the donor site is minimal and, in most cases, directclosure permits the repair of the donor site(AU)

ABSTRACT

El objetivo del presente trabajo es valorar los resultados funcionales y estéticos de reconstrucciones complejas orofaciales, realizadas mediante la asociación de dos colgajos libres simultáneos de peroné y radial. Se ha realizado este tipo de reconstrucción en tres pacientes con buenos resultados funcionales y estéticos. La reconstrucción con dos colgajos libres está indicada en defectos complejos en los que la reconstrucción con un solo colgajo no obtendría resultados estéticos y funcionales adecuados. Cada colgajo se selecciona por sus características que lo hacen ideal para cada tipo de defecto, óseo y de partes blandas con la finalidad de conseguir un resultado estético y funcional superior al que se obtendría con un solo colgajo (AU)

ABSTRACT

Llamamos pseudoanquilosis, falsa anquilosis o constricción mandibular permanente a una restriccion crónica de la movilidad mandibular derivada de cualquier obstrucción mecánica o interferencia a dicho movimiento madibular por una condición patológica extraarticular. Los objetivos primarios del tratamiento de la pseudoanquilosis son restaurar tanto la apertura oral como la función masticatoria y evitar la recurrencia. La fisioterapia precoz y prolongada juega un papel fundamental para evitar la recidiva. Presentamos tres casos tratados en nuestro Servicio entre 1992 y 1998 (AU)

ABSTRACT

Although the oral mucosa is often the site of entry of actinomyces into the deeper tissues, actinomycosis in the oral mucosa is extremely rare. Actinomycotic lesions are usually described as either single or multiple abscesses or indurated masses with hard fibrous walls and soft central loculations. Actinomyces israelii is the principal cause of human actinomycosis. We present a rare case of actinomycosis caused by Actinomyces odontolyticus; it presented primarily as a long-standing ulcer of the oral mucosa mimicking a squamous cell carcinoma.

ABSTRACT

Giant cell arteritis is a systemic disease with a broad range of clinical signs and symptoms. Although the most frequently involved vessel is the superficial temporal artery, other arteries can be affected. Vasculitic changes in the facial artery usually present as jaw claudication. A report of a case of giant cell arteritis is presented in which facial artery involvement first manifested itself clinically as a submandibular mass. This is a very rare and atypical form of clinical presentation.

ABSTRACT

Actinic cheilitis is a premalignant condition of the lower lip that requires treatment. Several methods have been advocated to manage this disease. In this study, 19 cases of actinic cheilitis were treated by carbon dioxide (CO2) laser vermilionectomy with good results. Because it has the advantages of both scalpel vermilionectomy and laser vaporization of the vermilion, CO2 laser vermilionectomy is proposed as a good method of treating actinic cheilitis.

ABSTRACT

Intravascular Papillary Endothelial Hyperplasia (IVPEH) is a benign process, even uncommon. It was first described by Masson in 1923 under the name "Hemangioendotheliom vegetant intravascular". We present a case of IVPEH located in the masseter muscle. The treatment for this lesion was surgical excision. The histological examination is very important in order to separate this lesion from angiosarcoma.

ABSTRACT

Dens invaginatus is an abnormal enamel growth first described by Salter in 1855. It has an incidence between 0.4 - 10%, and the tooth most frequently affected is the lateral upper incisor. Its etiology is not explained. There are many asymtomatic cases and others with symptoms derived from periapical lesions. Treatment must be as conservative as possible. A bibliographic review has been made and we demonstrate a radiologic sequence of the many cases observed in our Service in the last two years.
