ABSTRACT

INTRODUCTION: Purtscher-like retinopathy as a complication of acute pancreatitis is a disease which causes generalized retinal and macular oedema associated with retinal surface haemorrhage, which results in a permanent, sometimes partially reversible visual loss. CASE REPORT: The case report presented in this article refers to the case of a 36-year-old woman with severe acute pancreatitis. The pancreatitis led to the development of acute respiratory insufficiency with signs of septic shock and consequently, the patient was transferred to the critical care unit. The patient underwent a total of 8 surgical interventions with the revision of the abdominal cavity, necrectomy and application of the vacuum system into the abdominal cavity. Eye exams were conducted for impaired vision with the conclusion of Purtscher-like retinopathy associated with acute pancreatitis, occlusion of the central retinal artery of both eyes, confirmed by fluorescence angiography. After one year, alterations of the retina show no signs of improvement in this case; the loss of vision is permanent.Key words: acute pancreatitis Purtscher-like retinopathy.

ABSTRACT

Acute zonal occult outer retinopathy is noninfectious zonal dysfunction of outer retina often adjacent to optic nerve. It is accompanied by temporary disappearance of retinal fotoreceptor layer, visual acuity drop and late pigmentary fundus changes. Authors present a case report of a patient with unilateral involvement and spontaneous visual acuity resolution followed by quick reappearance of photoreceptor layer on OCT.

ABSTRACT

Lightning injuries are dangerous and often deadly trauma. We present a case of 41 year old woman who suffered an indirect lightning injury, dropping into unconsciousness followed by amnesia. Initial irritation of her left eyeball was followed by subtle changes in retinal pigment epithelium without any drop in visual acuity for a period of three months. Subsequently there began a slow deterioration of visual acuity and progression of cataract causing BCVA to drop to 5/50. Patient underwent uncomplicated cataract phacoemulsification with IOL implantation which resulted in restoring BCVA to 5/5 postoperatively. Such cases are scarce in literature.

ABSTRACT

Citamopram is an antidepressant drug with fatigue, sleep disorders, appetite changes and visual impairment being the most common side effects. We are not aware of any published cases of cystoid macular edema following medication of this drug. We present a case of 55 year old woman with medical history of depression placed on Citalopram medication who noticed deterioration of right eye vision. Best corrected visual acuity was 20/120, ophthalmic coherence tomography and fluorescein angiography revealed cystoid macular edema. Two months following drug withdrawal edema disappeared and best corrected visual acuity improved to 20/32. Due to lack of other probable causes of edema and quick disappearance following drug withdrawal it is plausible to think of Citalopram as causative factor of cystoid macular edema in this patient.

ABSTRACT

Bacillus cereus is a gram-positive, rod-shaped, beta hemolytic bacterium which rarely causes eye infections. We present a case of 15 year old boy with unilateral keratitis. He was treated for two months by his ophthalmologist who suspected herpetic keratitis. Patient most probably suffered secondary infection by Bacillus cereus. After bacillus was identified and targeted treatment given clinical picture improved rapidly. Until now there are only few cases of bacillus cereus keratitis published.

ABSTRACT

Thrombosis of upper ophthalmic vein is both rare and serious pathologic event. Authors present a case of isolated unilateral upper ophthalmic vein thrombosis in 76-year-old woman, who despite treatment ended with amaurosis and secondary neovascular glaucoma. In differential diagnosis authors focused on searching for inflammatory process of orbit with adjacent structures as well as local and general causes leading to venous thrombosis. None of these were proven; it was a rare case of a patient with isolated upper ophthalmic vein thrombosis.

ABSTRACT

Combined occlusion of central retinal vein and cilioretinal artery represents 40% of all cilioretinal artery occlusions. Pathogenesis of this uncommon occlusion remains controversial. We present three cases id combined vascular occlusion that were diagnosed at our department from 2003 until 2008. Our patients, all productive age, had undergone complete clinical examinations and investigations to determine the cause of this condition. One patient had been diagnosed to have heterozygous form of protrombine mutation. Curious is also possible connection of vascular occlusion and application of anti-Hepatitis A and B vaccine.
