ABSTRACT

BACKGROUND: Analysis of the health profile of participants aged over 65 has been initiated in balneotherapy. AIMS: To determine the benefits of a 3-week outpatient balneotherapy program on patient-reported outcomes. METHODS: Our sample consisted of 1471 people. The average age of the study population was 72.5 years (± 5.1) and 67% of the participants were females. Symptoms intensity was measured by visual analogue scale, and health-related quality of life (HRQL) with the EQ5D questionnaire before and after the program. Changes of perceived functional status were rated using a 3-point Likert scale. RESULTS: The results indicated a significant increase in the fear of falling and a decrease in fatigue. EQ5D reveals a significant improvement after the program. The perceived change scores remained stable, except for anxiety/depression. CONCLUSIONS: The study indicated significant improvements of HRQL and a decrease of perception of fatigue and anxiety in elderly people participants in a 3-week balneological programme.

ABSTRACT

BACKGROUND: A falls prevention programme has been initiated in balneotherapy at Balaruc-les-Bains. AIMS: To determine the health profiles of subjects who are at risk of falls, over 65 years of age and attending balneotherapy. METHODS: Questionnaires were used to evaluate people on their fear of falling. Fatigue was assessed by visual analog scale as well as by functional status over the past 12 months. EQ-5D-3L, the IPAQ questionnaire and Fried's frailty scale were all used. Patients' functional capabilities were tested using the Unipedal stance test, the TUG test, the SPPB, the Tandem walking test and the isometric manual grip strength test. RESULTS: Out of the 1471 patients (72.45 years ± 5.10), the women (67%) were tested. In the last 12 months, 485 of these 1471 patients fell (33%) and 37% of them suffered a severe injury. 45-50% of these subjects are now in good health. Women had a significantly higher impaired perception of their health than men (0.02 < p < 0.0001). According to Fried's criteria, 10.2% of the population is considered frail, with a significantly greater number of women (p < 0.0001). Static equilibrium was subnormal (less than 12 s during the TUG). The number of steps in tandem position discriminates individuals and gender as does the speed of moving from A to B and muscular strength. CONCLUSIONS: More than one-third of the subjects (more women than men) aged 65 or older visiting the Balaruc-les-Bains health resort are pre-frail or frail. They all have a recent history of falls, suffer from impaired muscle strength, and have balance and gait disorders.

ABSTRACT

La diabetes mellitus (DM) va en aumento en nuestro país, con alta prevalencia de complicaciones especialmente por mal control metabólico. Objetivo: evaluar asociación de condición de salud, variables sociodemográficas y años de diagnóstico con satisfacción del paciente con características del tratamiento de diabetes. Material y método: estudio de prevalencia en 340 pacientes de 1.100 usuarios DM2 controlados en Programa Cardiovascular de un Centro de Salud Familiar (CESFAM) de Macul, Región Metropolitana, Chile, previo consentimiento informado. Muestreo aleatorio simple (confianza 95 por ciento, merma 10 por ciento). Recolección de información en CESFAM y/o visita domiciliaria por investigadoras e internas de enfermería entrenadas. Satisfacción del paciente, medida con subescala satisfacción del cuestionario Diabetes Quality of Life (EsDQOL). Análisis con estadística descriptiva, Chi2, Odd Ratios, IC (95 por ciento). Resultados: Población mayoritariamente adulta mayor, femenina, promedio años estudio 8,7. El 42,6 por ciento tiene diagnóstico de DM hace más de 10 años. Refieren mayor satisfacción con: tratamiento medicamentoso, controles recibidos, vida en general. Existe mayor insatisfacción con: tiempo ocupado en ejercicios, cómo duerme, vida sexual. Satisfacción con tratamiento medicamentoso se asocia a compensación de diabetes (p= 0,026). Pacientes insatisfechos con tratamiento tienen 2 veces más riesgo de descompensación que los satisfechos, IC [1,08-3,7]. Insatisfacción con tiempo dedicado a enfermedad, controles, tiempo ocupado en controles y ejercicio indicado se asocian a complicaciones (p < 0,05). Conclusión: Insatisfacción con características del tratamiento de DM2 se asocia mayormente a complicaciones que a compensación de DM, pacientes insatisfechos tienen más riesgo de complicaciones que los satisfechos.


Diabetes Mellitus is increasing in our country, with a high prevalence of complications mainly because of poor metabolic control. Objectives: to evaluate association of health condition, demographic variables, and years since diagnosis with patient satisfaction regarding disease treatment. Methodology: prevalence study, 340 patient taken out of a population of 1.100 patients attended in the Cardiovascular Program of an outpatient clinic in Macul district. Informed consent was requested. Simple randomized sample (CI 95 percent, loss 10 percent). Data collected in the outpatient clinic or through home visits done by researchers or trained senior nursing students. Patient satisfaction was measured by the satisfaction subscale of the Diabetes Quality of Life Questionnaire (EsDQOL). Analysis: descriptive statistics, Chi2, Odds Ratios, CI (95 percent). Results: Population of mainly older adults, female, average years of school attendance 8.7. 42.6 percent has been diagnosed since more than 10 years. They refer greater satisfaction with: pharmacologic treatment, care received in outpatient visits, life in general. There exists more dissatisfaction with: time spent in exercise, sleep, sexual life. Satisfaction with pharmacologic treatment has association with diabetes compensation (p= 0.026). Patients dissatisfed with treatment have 2 times more risk of poor metabolic control than satisfed patients, IC[1.08-3.7]. Dissatisfaction with time spent in the disease, outpatient visits, and time spent in exercises, are associated with complications (p < 0.05). Conclusion: Dissatisfaction is more associated to complications than to poor metabolic control. Patient dissatisfed have more risk of complications than those satisfed.

ABSTRACT

Objetivo: Identificar condición de salud (CS) de los pacientes diabéticos tipo 2. Medir el cumplimiento de las atenciones de salud según guía clínica. Identificar variables asociadas a CS. Material y método: estudio de pre-valencia, abril 2010. Población: 1.100 pacientes DM2 controlados en Programa Cardiovascular, considerando criterios de inclusión, exclusión, consentimiento informado. Muestra: 340 por aleatorización simple (confianza 95 por ciento merma 10 por ciento). Recolección de datos realizada en CESFAM o visita domiciliaria por investigadoras e internas de enfermería entrenadas, con encuesta validada, exámenes, plantilla para información y ficha clínica. Para el análisis se utilizó estadística descriptiva, Chi2, Odds Ratios con IC (95 por ciento). Resultados: CS: descompensados (HbA1c>7 por ciento) 56.5 por ciento, patologías agregadas 97.9 por ciento, complicaciones 25 por ciento, mayor frecuencia retinopatía diabética. hombres mayor riesgo de amputación (p=0.003). Presión arterial >130/85 mm.hg. 58.2 por ciento, sobrepeso-obesidad 71.2 por ciento, autovalencia (adultos mayores) 23.1 por ciento. Cumplimiento recomendaciones ministeriales: controles/profesionales/año a lo menos cinco 41.5 por ciento, CESFAM adapta esta recomendación. Evaluación anual de: pie diabético 78 por ciento, fondo ojo 41,6 por ciento. Indicación régimen (nutricionista) último control 80 por ciento. Cinco controles profesionales/ año o más es protector de compensación de diabetes, IC (0.62-0.95), resto de recomendaciones y adaptación de éstas no se asocian a condición de salud (p > 0.05). Conclusiones: Los componentes de condición de salud están en general por sobre lo esperado. En ningún paciente se cumple en un 100 por ciento las recomendaciones de la guía clínica. Cinco o más controles profesionales anuales se asocian a compensación de la diabetes. Importante cumplir recomendaciones para mejorar control metabólico y disminuir/retrasar complicaciones para una mejor calidad de vida de pacientes.


Objectives: To determine compliance with Chilean clinical guideline and to measure their impact through evaluation of patient’s health condition (HC), to determine associations. Methods: prevalence study, April 2010. Population: 1,100 diabetic patients controlled in the Cardiovascular Program, considering criteria of inclusion, exclusion, informed consent. Sample: 340 randomized patients (Confidence level: 95 percent; Loss: 10 percent). Information was collected in an outpatient clinic and through home visits performed by researchers and trained senior nursing students, with a validated survey, exams, staff for file information. For the purposes of the analysis, descriptive statistics was used, Chi2, Odds Ratios with CI (95 percent). Results: HC: decompensation (HbA1c>7 percent 56.5 percent derived illnesses 97.9 percent, complications 25 percent, greater frequency of diabetic retinopathy. Males with greater risk of amputation (p=0.003). Blood pressure >130/85 mm. hg. 58.2 percent, overweight-obesity 71.2 percent, self-care (older adults) 23.1 percent. Compliance with clinical guideline: controls/professionals/year at least five 41.5 percent, CESFAM adapts this recommendation. Annual evaluation of: diabetic foot 78 percent, fundoscopy 41.6 percent. Regime indication (dietician) last control 80 percent. Five professional controls/year or more protect compensation for diabetes, IC (0.62-0.95), other recommendations and adaptation of recommendations are not associated with health status (p > 0.05). Conclusions: components of hC are better than expected. In none of the patients are the recommendations of the clinical guideline fulfilled in a 100 percent. 5 or more annual outpatient visits are associated to good metabolic control. Compliance with clinical guideline recommendations is important to improve metabolic control, to lessen complications and to improve patient’s quality of life.

ABSTRACT

Pegylated interferon and ribavirin is the treatment of choice in patients with chronic hepatitis C infection. The most common side effects of interferon therapy are flu-like symptoms and psychiatric disorders. Pneumonitis is a less frequent complication associated with non-negligible mortality. We herein report a case of interferon associated pneumonitis in a patient with non-severe clinical symptoms and a normal chest radiography. Physicians should be aware of this entity during the differential diagnosis of respiratory symptoms in patients receiving treatment with interferon due to its high morbimortality and good resolution and outcome after drug withdrawal.

ABSTRACT

El interferón pegilado asociado a ribavirina es el tratamiento de elección en los pacientes con infección por hepatitis C crónica. Los efectos secundarios más comunes son los síntomas gripales y las alteraciones psiquiátricas. La neumonitis es una complicación poco frecuente, pero se asocia con una mortalidad no despreciable. Presentamos el caso clínico de un paciente con neumonitis intersticial asociada a interferón que presentaba signos y síntomas clínicos sutiles y una radiografía de tórax sin alteraciones. Se debe de tener presente esta entidad en el diagnóstico diferencial de los síntomas respiratorios en pacientes que reciben tratamiento con interferón debido a su elevada morbi-mortalidad y a su evolución favorable tras la retirada del fármaco(AU)

En estos lugares, habian comentarios de que un mismo condon femenino "Reality" habia sido utilizado por las trabajadoras sexuales por mas de una relacion sexual, ademas indicando que el condon masculino "Pantera" se rompe algunas veces. Para indicar dicha información emitida se decidio realizar un estudio cualitativo del uso de condones masculino y femenino por las trabajadoras sexuales comerciales en la ciudad de La Paz y El Alto para saber sus conocimientos, actitudes y practicas sobre condones masculinos y femeninos

ABSTRACT

Sarcomas of the kidney are exceedingly uncommon. They represent between 1 and 3% of all malignant renal tumors. Several histological types have been reported. We have reviewed the renal tumors diagnosed in our department in the last 25 years and found 4 cases of sarcoma out of 390 malignant renal tumors (1.02%). They were a liposarcoma, a storiform-pleomorphic fibrous malignant histiocytoma, a rhabdomyosarcoma and a pleomorphic leiomyosarcoma. We report the clinical data and the outcome of the patients, together with the immunohistochemical and ultrastructural findings in these cases.

ABSTRACT

En un estudio prospectivo se comparó eficacia, toxicidad renal, coclear y vestibular de gentamicina administrada en dosis fraccionada de 1,7 mg/kg peso cada 8 horas (DF) y dosis única de 3 mg/kg peso/día (DU). El sitio de la infección fue apendicular en 13 casos, urinario en 11, perianal en 9 y otros en 6 casos. En el 59 por ciento de los pacientes se aisló Escherichia coli y en el 23 por ciento otros gérmenes Gram negativos. Fueron tratados 21 pacientes con DF y 18 con DU. La evolución fue similar en relación al número de días con fiebre y compromiso del estado general de los pacientes. El recuento de glóbulos blancos, VHS y proteína C reactiva aumentaron al inicio del tratamiento, pero se normalizaron en ambos grupos al término del mismo. Se registraron 3 fracasos de tratamiento con DF y 1 con DU. La creatinina plasmática no se modificó al término del tratamiento (p = 0,79) y en un control a los 30 días post tratamiento (p = 0,79). El clearence de creatinina se redujo en ambos grupos, siendo esta reducción al término del tratamiento más severa en el grupo DU, pero sin alcanzar significación estadística (p = 0,62), con normalización a los 30 días. La evaluación del 8º par a través de pérdida de audición de frecuencia específica,pérdida media para frecuencias altas y pruebas calóricas no identificó daño post tratamiento en ambos grupos hasta 30 días de seguimiento. El costo diario de la DF fue de $ 1.552 y de la DU de $ 141. El consumo de tiempo de la administración de la DF fue nueve veces mayor. En conclusión, la modalidad de tratamiento con gentamicina en dosis única diaria resultó efectiva,segura, con menos fracasos de tratamiento, considerablemente más barata y de más rápida administración que la alternativa de dosis fraccionada, por lo que recomendamos el uso de gentamicina en dosis única diaria de 3 mg/kg/día

ABSTRACT

La litotripsia extracorpórea constituye el tratamiento de elección de la mayoría de los cálculos de la vía urinaria. En nuestro país existe un importante número de máquinas especialmente concentradas en el centro del país que han demostrado la eficacia de esta excelente alternativa terapéutica. En Enero de 1994 se constituyó la Unidad de Litotripsia de la Clínica de la Mutual de Seguridad de Calama (ULMC), y se comenzó un trabajo cooperativo con otros centros del Norte Grande de Chile, con lo cual nuestros pacientes no tienen que desplazarse grandes distancias para acceder a esta tecnología. Se presentan los resultados de esta experiencia cooperativa

ABSTRACT

The expression of three different actin genes in the sea urchin, Strongylocentrotus purpuratus, was monitored in embryos and adult tissues by using untranslated mRNA sequences as specific hybridization probes. Three distinct patterns of expression were found: muscle specific, embryo specific, and constitutive (i.e., present in all tissues examined). The actin genes encoding the muscle-specific and constitutively expressed genes were each found to be present once in the haploid genome. The embryo-specific probe could derive from either a single gene or a small subset of actin genes. These data demonstrate that at least three members of the sea urchin actin gene family are expressed in distinct ways and thus are probably associated with different regulatory programs of gene expression necessary for development of this metazoan.

ABSTRACT

Polyribosomes were obtained in high yield from Rhynchosciara salivary glands by a method which uses the non-ionic detergent Nonidet P 40, in moderate salt buffer and DTT as RNase inhibitor. The polysomal profiles from glands of animals which have just started to spin the communal cocoon are slightly but consistently different from those obtained from about 5 day older animals, in which DNA puffs were open in the salivary gland chromosomes. The heaviest polyribosomes which could be detected by either radioactive uridine or amino acid incorporation have 8 to 9 ribosomes. The data indicate the existence of stable messengers in the salivary glands.
