ABSTRACT

Angiosarcoma of the breast in an uncommon entity. The authors report a well documented case of angiosarcoma. The patient, a 23 years old woman, had a voluminous mass of the left breast without inflammation. The evolution was rapidly fatal. Physical examination revealed a large painful breast mass with purplish discoloration cutaneous area in front of the lesion. The palpation revealed a thrill. The mammography had shown an area of increased density in the left breast. A complementary ultrasound examination detected a well circumscribed voluminous mass with hypoechogenic heterogenous echostructure. The Doppler examination detected a venous blood flow in the tumor. This finding was compatible with vascular tumor as angioma or angiosarcoma. The angioscanner showed a peripheral vascular enhancement and a centripetal diffusion of the contrast product. This vascular kinetic is seen commontly in the angioma. Magnetic resonance imaging in the T1 and T2 relaxation times detected an intermediate signal mass with bleeding areas. The patient had had a mastectomy and the histopathologic examination confirmed the diagnostic of angiosarcoma. The radiological aspacts of angiosarcoma of the breast are discussed depending on the literature data.

ABSTRACT

Ocular amyloidosis a an uncommon condition and conjunctival involvement rarely occurs, representing 15% of all ocular localizations. Clinical manifestations show a polymorphism requiring a histological diagnosis. Based on two cases of primary isolated conjunctivo-palpebral amyloidosis, we discuss the different ocular sites. The first case involved false ptosis, a conjunctival infiltration and a lower eyelid tumefaction; all located in the left eye. The second case had ptosis, a right lower eyelid tumefaction with ectropion and esthetic damage. Amyloidosis was confirmed at histologic examination of the conjunctival mucous after a special congo red coloration. In order to affirm the isolated conjunctival localization, it is necessary to eliminate another amyloidis site. The treatment was surgical with excision and cure of ptosis. Ocular amyloidosis is a rare condition with polymorphism manifestations. Confirmation is histological. The greatest difficulty is management due to disease recurrence.

ABSTRACT

A retrospective epidemiological study is reported concerning burn injuries in 775 children hospitalized at the unit of burn care of Casablanca between 1985 and 1993. The mean age was 3 years and 8 months; 69.9 per cent of the patients were below 5 years old. The male to female ratio was 1.43 to 1.0. The most common agents were scalding liquids (69.1 per cent) followed by flames (15.2 per cent). The authors underline through this report that preventive measures depend closely on the regional, environmental and sociocultural specificity.

ABSTRACT

The author report twenty one cases of tissue reconstruction defects secondary to necrotizing cellulitis of perineum and external genital organs, treated at the burns and plastic surgery unit of CHU Ibn Rochd of Casablanca, between 1992 and 1994. Depending on the extent of the defect, the techniques used were gracilis musculocutaneous flap in 8 cases, thigh fasciocutaneous flap in 7 cases, groin flap in 4 cases, simple skin sutures and skin grafts in one case, respectively. The morphological results were satisfactory. The mean hospital stay after reconstruction was 15 days.
