ABSTRACT

Some indices of the system of homeostasis, lipid metabolism, lipid peroxidation (LPO), the levels of IRI, C-peptide, glycosylated Hb and glycemia (on an empty stomach) were investigated in 131 patients with noninsulin dependent diabetes mellitus (the patients ranged in age from 40 to 60). Correlation analysis has shown that derangements in the system of homeostasis, lipid metabolism and LPO are correlated and interrelated, resulting in disorder of the arterial blood flow in the lower limbs and hence to the development and progression of diabetic angiopathies. The authors have proposed a scheme of LPO involvement in the development of lower limb diabetic angiopathies.

ABSTRACT

To elucidate the nature of lipid defects in patients with diabetes mellitus (DM) concurrent with acute myocardial infarction (MI), the study was undertaken to examine the serum concentrations of total cholesterol, triglycerides, alpha- and beta-lipoproteins with DM in the presence of acute MI. 40 non-diabetic patients with acute MI, 23 diabetics with postinfarct cardiosclerosis, and 17 non-insulin-dependent diabetics without signs of coronary atherosclerosis. Urinary epinephrine and norepinephrine excretion was additionally determined in the acute period and 3-4 weeks after therapy. Homogeneous lipid metabolic parameters were found in CHD patients with and without DM and when transient hyperglycemia developed. The patients with acute MI exhibited some increase in lipid consumption to satisfy the energy need for the cardiovascular system, this being true for triglycerides in DM patients. The DM patients who showed low triglyceride levels had more frequently transmural MI and MI complicated with heart failure. Obesity and familial histories of DM and CHD in DM patients with acute MI were ascertained to be accompanied by reduced serum alpha-lipoprotein concentrations.

ABSTRACT

The impairment of sympathetic innervation of the urinary bladder neck was supposed to be a cause of the syndrome of diabetic retrograde ejaculation (DRE). The performance of the sphincter system had not been studied in this category of patients. The liquid profilometric technique was used to examine 3 groups of males: 8 patients with the DRE syndrome; 5 patients with diabetes mellitus (DM) without ejaculation disorders; and 7 apparently healthy subjects. All the examinees showed no organic changes in the prostate and urethra. The groups were matched by age, type, duration and severity of DM. The DRE syndrome patients had no elevation of intraureteral pressure in the area the inner sphincter of the urinary bladder, which evidenced its atony. In health the elevation of vesical pressure is usually accompanied by an increase in ureteral resistance (r = 0.96; p less than 0.001), thus maintaining the stability of the positive pressure gradient and preventing the escape of urine. Correlation analysis revealed alterations of interrelations between intravesical and sphincter pressures in diabetes mellitus patients. This phenomenon evidenced the disorders of somatic innervation of the outer ureteral sphincter more pronounced in patients with the DRE syndrome. No significant differences were revealed in the length of the anatomical and functional ureter as well as in the length of the posterior ureter of all the patients involved in the study.(ABSTRACT TRUNCATED AT 250 WORDS)

ABSTRACT

A study was made of the time course of the indices of the cellular and humoral immunity in 202 patients with diffuse toxic goiter (grave and moderately grave forms of the disease) aged 17 to 50 with a period of disease from 6 mos to 25 yrs and in 30 healthy persons of the same age. A decrease in the content of T-lymphocytes in the blood in percentage and absolute quantities, an increase in the content of O-lymphocytes and serum IgM and IgG indicated profound disorders in the immunological status of patients with diffuse toxic goiter. It was mostly observed in the patients with a period of disease up to 10 yrs.

ABSTRACT

Comparative studies on changes of hemodynamics in manual and leg isometric loads were conducted in 30 healthy persons and 38 patients with coronary heart disease. A possibility of a leg isometric load with a maximum effort for 5 min. was shown. Intracardiac hemodynamics was estimated by echocardiography results. A possibility to reveal early stages of myocardial contractile and pumping function in patients with coronary heart disease was established. Manual dynamometry does not permit revealing early stages of heart insufficiency.

ABSTRACT

The authors provide the data obtained on retrospective analysis of 30 ECG recorded in 30 men with coronary heart disease (CHD), confirmed by the clinical and coronarography data, during bicycle ergometry. The amplitude of the R wave in the V5 lead and the total amplitude of the R wave in the V3-V6 leads were analyzed. The area of the QRS complex was calculated as a sum of three triangles. While drawing up a triangle according to the S wave one of the sides went over from the S wave top across the J point till crossing the isoline. The amplitude and duration of the T wave were also taken into consideration. It was established that evaluation of the area of the QRS complex, whose magnitude is an integrated index of changes in all the elements of the ventricular complex, is of the greatest value in the diagnosis of CHD during bicycle ergometry.

ABSTRACT

The results are presented of comparison of electrocardiographic data obtained in 36 precordial and 6 standard precordial leads of ECG, and their correlation degree in 20 patients with acute transmural myocardial infarction (MI). Total R-wave and QS complex amplitude, the number of leads recording the mentioned changes (nR and nQS), total ST-segment elevation (epsilon ST) and the number of leads with ST elevation greater than or equal to 1 mm were assessed. The study demonstrated that R-wave and QS complex amplitude, and also the number of informative leads recording these changes were the most reliable markers of ischemia and necrosis. High authenticity of data in 6 leads-standard ECG and accurate correlation of examined indices with similar ones in 36 precordial leads throughout the observation at various points of the testing suggest that the treatment of QRS complex in 6 lead-standard ECG gives a reliable assessment of the prevalence and time course of ischemic myocardial damage in patients with anterior transmural myocardial infarction.
