ABSTRACT

BACKGROUND: Myiasis is a neglected disease, characterized by ill-defined diagnostics and management protocols. Published epidemiological and clinical studies of myiasis are still scarce, although several countries, such as Oman, have reported a few cases over the past 30 years. This study explores the epidemiological profile and clinical characteristics of myiasis in South Batinah Governorate (SBG), Oman. METHODOLOGY: A prospective surveillance study was conducted in SBG from 1st November 2018 to 31st October 2019. Clinical and epidemiological data were collected using a pre-designed reporting form for suspected and confirmed cases. RESULTS: A total of 188 cases were reported, of which 81.4% were male. One-third of cases were 11-20 years old, and a quarter reported exposure inside house. The rate of exposure was 16 per 10000 in Nakhal and Wadi Mawel. About 57% patients reported multiple re-exposure. Almost 60% of patients developed nasal or oral myiasis, 25% developed myiasis in the eye, and 4% in the ear. The most common respiratory symptoms were cough (83%), runny nose (48%), and foreign body sensation (35%). Over 50% of patients with eye exposures had redness, pain, and itchiness. Most patients (89.3%) kept animals at homes. Many cases occurred between December 2018 and April 2019, with almost one-third of exposures occurring at 5 p.m. Re-exposure likelihood for patients residing in Nakhal and Wadi Mawel was high; OR = 2.49 (95% CI 1.14-5.45), with OR = 3.59 (95% CI 1.37-9.39) for 11-20-year-olds and, OR = 3.01 (95% CI 1.07-8.42) for patients ≥ 31 years. CONCLUSION: The myiasis exposure rate is high in certain areas and age groups, which is most likely associated with animal-related activities. The disease has a significant impact upon people's health in SBG. Therefore, urgent active-surveillance and clinical studies are warranted to explore possible preventive measures and treatment options. Adopting one health approach could offer an effective strategy for preventing myiasis in human and animal populations.

ABSTRACT

Objectives: This study was aimed at exploring and analyzing the epidemiological profile, surveillance, and response to COVID-19, including transmission dynamics and cluster formation. Methodology: This was a retrospective analysis of surveillance data, including contact tracing, risk factors, and clinical information. Binary logistic regressions were used to assess the likelihood of admission, cluster formation, and of each individual being an index patient. Clusters were demonstrated through geographic data systems, network analysis, and visualization software. Results: A total of 1100 COVID-19 cases were diagnosed from 20 March to 7 June 2020, of which 144 (13.1%) were asymptomatic. The median time from symptom onset to admission was 7 days (IQR, 4.5-10), and the median symptom duration was 5 days (IQR, 3-9). Eighty-nine clusters containing 736 patients were identified. The surveillance and control actions were divided into three phases. Clusters began to form in phase 2 and became more pronounced in phase 3. Patients ≥50 years of age and patients presenting with fever had relatively higher odds of admission: OR = 12.85 (95% CI 5.13-32.19) and 2.53 (95% CI 1.24-5.17), respectively. Cluster formation was observed among females, asymptomatic patients, and people living in Awabi: OR = 2.3 (95% CI 1.7-3.1), 6.39 (95% CI 2.33-17.2), and 3.54 (95% CI 2.06-6.07), respectively. Patients working in the police and defense sectors had higher odds of being an index patient: OR = 7.88 (95% CI 3.35-18.52). Conclusion: Case-based interventions should be supported by population-wide measures, particularly movement restrictions. Establishing prevention teams or district units, or primary care will be crucial for the control of future pandemics. Prevention should always be prioritized for vulnerable populations.

ABSTRACT

Background: The changing epidemiological profile of the COVID-19 pandemic and the uncertain clinical picture of patients characterise this ongoing and most challenging health event. Objectives: To report clinical features, laboratory characteristics, and mortality risk factors among COVID-19 patients admitted to a secondary hospital in Oman. Methods: A retrospective study for the first 455 patients admitted with COVID-19 to Rustaq hospital from 12th April, 2020 to 27th September, 2020. A predesigned questionnaire collected data from the hospital medical electronic system. Results: The mean age was 42.84 (SD = 19.86) years, and the majority of patients were aged 30 to 59 and 60 or above; 207 (45.5%) and 189 (41.5%), respectively. Male patients constituted approximately two-thirds of the subjects. Fever, dyspnea and cough were the most common presenting symptoms (69%, 66%, and 62%, respectively), while comorbidities with diabetes mellitus and hypertension were 47% and 44%, respectively. Bacterial growth was identified at approximately 10%. Bivariate analysis turned out to be significant with a number of factors. However, multivariate analysis showed significance with patients aged over 60 (OR = 7.15, 95% CI 1.99-25.63), dyspnea (OR = 2.83, 95% CI 1.5-5.33), dyslipidemia (OR = 1.93, 95% CI 1.02-3.66) and being bed-ridden (OR = 5.01, 95% CI 1.73-14.44). Durations from onset of symptoms to admission and respiratory distress were lower among patients who died; p = 0.024 and p = 0.001, respectively. Urea, Troponin and LDH may act as potential diagnostic biomarkers for severity or mortality. Conclusions: This study identified groups of patients with a higher risk of mortality, with severe disturbance in the laboratory markers while some could act as potential diagnostic biomarkers.

ABSTRACT

Background: Covid-19 pandemic has left deep psychological impacts, especially among infected patients. It is extremely important to understand the extent of those effects, while improving the compliance with isolation measures at the same time. Objectives: To detect prevalence of stress using two psychological scales and examine the stress associated factors, also to identify self-isolation compliance rates among COVID-19 patients. Methods: Cross-sectional research was conducted from 15 November to 22 December 2020, involving 379 patient participants selected via systematic random sampling. Kessler 10 Psychological Distress (K10) and the impact of event scale-revised (IES-R) tests were used to ascertain the levels of distress. Results: K10 measure revealed elevated stress amongst 121 (31.9%) of participants, whereas IES_R indicated the level was 37.7%. Using the K10 indicated the multivariate analysis was significant for females (OR = 2.482, 95% CI: 1.532-4.021), patients with financial problems (OR = 2.332, 95% CI: 1.270-4.282) and patients experiencing shortages of essentials (OR = 4.920, 95% CI: 2.524-9.590). The IES-R scale indicated that only female and patients experiencing shortages scored significantly in multivariate analysis, (OR = 1.895, 95% CI: 1.1223-2.935) and (OR = 2.928, 95% CI: 1.1580-5.424), respectively. Those undergoing shorter isolation periods reported lower levels of stress on both K10, p=0.016 and IES-R, p=0.002. Approximately 90% of patients used their own towels during isolation. Moreover, 80.2% slept in separate rooms and 74% used masks in the presence of other family members. Essential supply shortages were reported by 14.2% of respondents. Conclusions: Self-compliance rates were not optimal, while psychological distress was more prevalent among some groups. Intervention is imperative to minimize stress and improve self-isolation compliance.

ABSTRACT

Melioidosis is a severe disease that can affect humans and animals. It is caused by Burkholderia pseudomallei, which is an environmental aerobic gram negative bacteria. Despite being endemic in Southeast Asia and Northern Australia, many cases have been reported from different regions, including the Middle East. This is the third imported case of B. pseudomallei from Oman in a 46-year-old Indian male with left knee septic arthritis in less than three weeks after arrival in Oman. He underwent open arthrotomy, and his synovial fluid culture grew a bacteria with dry, pink colonies that were oxidase positive, susceptible to amoxicillin-clavulanic, identified by API NE as B. pseudomallei, and confirmed by molecular tests. This single case report highlights the urgent need to increase molecular diagnostic capacity and improve public health surveillance while maintaining a one health tripartite approach.

BACKGROUND: The established aim of the Saudi Health Electronic Surveillance Network (HESN) is to support the prevention and control of different health events, and to facilitate the delivery of other public health programs. This study aims to evaluate the perceptions of active HESN users regarding its general performance through five major components: practicability, design, data and communication, technical support, and general impression. METHODS: A cross-sectional study was conducted in 2016 using a sample of active HESN users. Out of 1535 active users, 700 were randomly selected. A predesigned electronic questionnaire was sent to each participant via email which was completed by 485 participants. Different composite scores were calculated and compared to the sociodemographic and other technical variables. RESULTS: The mean age of the participants was 36.92 ± 9.12 (24-65 years), and 57.8% of the sample were male. Riyadh and the KSA's eastern province represented the highest two regions of participation, at (18.4%) and (14.2%) participants, respectively. About 70.8% were generally satisfied with HESN, while 86.6%% believed that it is better than the traditional paper-work system. Participants who used to work more frequently expressed more level of satisfaction compared to those with minimal use per week or month (P ≤ 0.001). Internet speed displayed a significant association with the general level of satisfaction with HESN (P < 0.001). Additionally, users who accessed HESN with the Google Chrome browser displayed higher levels of satisfaction when compared to users who relied on other browsers (P = 0.003). CONCLUSION: Presently, the level of user satisfaction with HESN is reasonable. However, to achieve optimal outcomes for HESN usage, improvements should be considered.

ABSTRACT

Following the detection of the first human case of avian influenza A subtype H9N2 in 1998, more than 40 cases were diagnosed worldwide. However, the spread of the virus has been more remarkable and significant in global poultry populations, causing notable economic losses despite its low pathogenicity. Many surveillance studies and activities conducted in several countries have shown the predominance of this virus subtype. We present the case of a 14-month-old female in Oman with an A(H9N2) virus infection. This is the first human case of A(H9N2) reported from Oman and the Gulf Cooperation Countries, and Oman is the second country outside of southern and eastern Asia to report a case (cases have also been detected in Egypt). The patient had bronchial asthma and presented with a high-grade temperature and symptoms of lower respiratory tract infection that necessitated admission to a high dependency unit in a tertiary care hospital. It is of urgency that a multisector One Health approach be established to combat the threat of avian influenza at the animal-human interface. In addition to enhancements of surveillance and control in poultry, there is a need to develop screening and preventive programs for high-risk occupations.
