ABSTRACT

PRCIS: Peripapillary retinoschisis may bias optical coherence tomography's monitoring of glaucoma progression. Its impact on glaucoma still remains uncertain. Only two out of the ten included studies illustrated a correlation between peripapillary retinoschisis and glaucoma progression. PURPOSE: The frequent use of optical coherence tomography increased the detection of peripapillary retinoschisis, which poses challenges in the follow-up of glaucoma patients. This systematic review aims to summarize the literature regarding peripapillary retinoschisis in glaucoma, exploring its prevalence, impact on disease, and clinical management implications. METHODS: We searched PubMed, Embase, Web of Science and Scopus with tailored search queries for each platform. All studies had to report peripapillary retinoschisis in glaucoma patients. Exclusion criteria included studies with less than 10 eyes, studies focusing on schisis outside the disc area, with concomitant retinal or optic nerve lesions, with animals, reviews, studies written in non-English language and congress abstracts. RESULTS: Ten studies were included, of which 7 were case-control, one was a cohort study and two were case series.Six studies showed that peripapillary retinoschisis often overlapped pre-existing retinal nerve fiber layer defects. One study reported that the de novo development of peripapillary retinoschisis was more frequent in eyes with glaucoma progression than in eyes without progression.Visual field findings were inconsistent, with just one study (out of six) showing that glaucoma patients with peripapillary retinoschisis experienced faster visual field deterioration than those without it. Overall, solely two studies (out of seven) associated peripapillary retinoschisis with faster glaucoma progression. CONCLUSIONS: Peripapillary retinoschisis biases optical coherence tomography analysis in glaucoma. Caution is needed against overestimation of retinal nerve fiber layer thickness when peripapillary retinoschisis develops, and misinterpretation of its resolution as rapid progression. Peripapillary retinoschisis' exact impact on glaucoma progression remains unclear.

ABSTRACT

Durante a pandemia de Covid-19, ações grupais de cuidado realizadas nos cursos de formação superior brasileiros tiveram de ser adaptadas para o ambiente virtual. Apresenta-se a experiência formativa realizada em 2020 por alunos da Universidade Federal de São Paulo juntamente com um grupo de mulheres em situação de vulnerabilidade, participantes do "Projeto Delicadas Coreografias", um projeto de extensão de cuidado em saúde por meio de práticas corporais. Considerando o sofrimento causado pelo momento pandêmico e as barreiras comunicacionais de acesso vivido pelos estudantes e pelas mulheres, foram realizados encontros remotos na busca de promover cuidado a partir da produção de um espaço de confiança, que fosse também lúdico, vincular e inventivo. Os encontros no ambiente virtual permitiram dar visibilidade e dizibilidade a processos singulares, configurando, então, em outros modos de produzir cuidado e tecer uma formação em saúde em tempos de pandemia.(AU)

During the Covid-19 pandemic, group care actions carried out in Brazilian higher education courses had to be adapted to the virtual environment. We present the training experience carried out in 2020 by students from Federal University of São Paulo, campusBaixada Santista together with a group of women in situations of vulnerability, participants of the "Project Delicates Choreographies", a project to extend health care through corporeal practices. Considering the suffering caused by the pandemic moment and the communicational barriers of access experienced by students and women, remote meetings were held in the search to promote care from the production of a space of trust, which was also playful, bonding and inventive. The meetings in the virtual environment allowed to give visibility and sayability to singular processes, configuring, then, in other ways of producing care and weaving a health education in times of pandemic. ABSTRACT

BACKGROUND: The use of natural plant antioxidants has been an alternative approach to synthetic antioxidants for food applications. This study investigated the antioxidant properties of the leaves of Curcuma longa L. against the oxidation of soybean oil using leaf extracts obtained from seven different types of organic solvents and compared to the action of efficient synthetic antioxidants which are widely used and low cost but with toxicity risk. The application of natural alternatives from sources still little studied is a proposal and the extraction of compounds with high antioxidant capacity may vary according to the type of solvent, its mixture, its polarity and other factors that burden or harm the health of the consumer. METHODS: The leaf extracts were sanitized, dried, and obtained for solvent selection. The quantification of total phenolic compounds (TPC) and antioxidants was performed by using different combinations and proportions of solvents: water (100%), ethanol (100%), methanol (100%), ethanol/water (70%:30%), methanol/water (70%:30%), methanol/acetic acid (99.5%:0.5%), and acetone/water/acetic acid (70%:28:2%). The solvent selected was the one that extracted the highest content of phenolic and antioxidant compounds (using the free radical DPPH scavenging method and FRAP assay). It used an accelerated storage test and experimental design to assess oxidative stability (using peroxide index (PI), thiobarbituric acid reactive substances (TBARS), dienes (DC) and triple conjugates (TC) and statistical analysis). RESULTS: The ethanol/water solvent was efficient in the extraction of TPC (2422 mg GAE 100 g-1). The mixture of solvents extracted an average of 23.65% more TPC when compared to the single solvents. Regarding the antioxidant capacity, the ethanol/water solvent showed 1847 mg TE 100 g-1 using the DPPH and 217 µmol of FeSO4 100 g-1 using the FRAP assays. In addition, the oxidation of soybean oil was delayed by the accelerated storage test showing equivalent results to BHT at 0.02% for the values of PI, TBARS and DC/TC. CONCLUSIONS: The E/W extract showed the highest yield in the extraction of phenolic compounds and is a safe solvent for food. The addition of Curcuma longa L. leaf extract to soybean oil caused reductions in PI, TBARS, CD and CT values. The leaf extract of C. longa L. is considered to be a good alternative to synthetic antioxidants against the oxidative degradation of soybean oil.

BACKGROUND:: Heparanase is an enzyme that cleaves heparan sulfate chains. Oligosaccharides generated by heparanase induce tumor progression. Basal cell carcinoma and squamous cell carcinoma comprise types of nonmelanoma skin cancer. OBJECTIVES:: Evaluate the glycosaminoglycans profile and expression of heparanase in two human cell lines established in culture, immortalized skin keratinocyte (HaCaT) and squamous cell carcinoma (A431) and also investigate the expression of heparanase in basal cell carcinoma, squamous cell carcinoma and eyelid skin of individuals not affected by the disease (control). METHODS:: Glycosaminoglycans were quantified by electrophoresis and indirect ELISA method. The heparanase expression was analyzed by quantitative RT-PCR (qRTPCR). RESULTS:: The A431 strain showed significant increase in the sulfated glycosaminoglycans, increased heparanase expression and decreased hyaluronic acid, comparing to the HaCaT lineage. The mRNA expression of heparanase was significantly higher in Basal cell carcinoma and squamous cell carcinoma compared with control skin samples. It was also observed increased heparanase expression in squamous cell carcinoma compared to the Basal cell carcinoma. CONCLUSION:: The glycosaminoglycans profile, as well as heparanase expression are different between HaCaT and A431 cell lines. The increased expression of heparanase in Basal cell carcinoma and squamous cell carcinoma suggests that this enzyme could be a marker for the diagnosis of such types of non-melanoma cancers, and may be useful as a target molecule for future alternative treatment.

ABSTRACT

A displasia arritmogênica de ventrículo direito (DAVD) é uma das principais causas de morte súbita em indivíduos jovens. Suas manifestações e consequente diagnóstico costumam ser precoces ainda na adolescência ou no adulto jovem. Nesse contexto, o diagnóstico em doente sexagenário sem história familiar sugestiva ou sintomas prévios e com possibilidade de tratamento torna esse caso único na literatura.


Arrhythmogenic right ventricular dysplasia is one of the major causes of death in younger patients and its manifestations and diagnosis used to be early. In this context, diagnosis in a sexagenary patient without famylial history or previous symptoms and with possibility of correct treatment make this case unique.

BACKGROUND: To correlate underlying diseases, in autopsies of patients with pulmonary thromboembolism (PTE) to histological findings and manifestations reviewed in the medical records. METHODS: The autopsy records between 2001 and 2008 of 291 patients whose cause of death was PTE were reviewed. The following data were obtained: age, sex, clinical "in vivo" manifestations, postmortem pathological patterns, and main associated underlying diseases, cancers, and surgeries performed in the last hospitalization. The pulmonary histopathological changes were categorized as diffuse alveolar damage, pulmonary edema, alveolar hemorrhage, and lymphoid interstitial pneumonia. Odds ratios of positive relations were obtained by logistic regression and were considered significant when P<0.05. RESULTS: The median age was 64 years old. About 64% of patients presented cardiovascular illness associated with PTE. The most prevalent pulmonary finding was pulmonary edema. Only 13% of cases had clinical suspicion of PTE. Acute respiratory failure was positively related to pulmonary edema, alveolar hemorrhage, and diffuse alveolar damage as well as hemodynamic instability to alveolar hemorrhage and diffuse alveolar damage. CONCLUSION: We found important relations between clinical data and histological findings of patients with fatal PTE. A greater understanding of the pulmonary physiopathological mechanisms involved with each disease associated to PTE could improve its diagnosis and treatment.

INTRODUCTION: Monitoring of disease activity and the best therapeutic approach are a challenge in Takayasu arteritis (TA). When associated with acute coronary syndromes (ACS), the best interventional treatment has not been established. The objective of this study was to describe the baseline characteristics, clinical manifestations, treatment and long-term outcome of patients with TA and ACS. METHODS: We retrospectively analyzed eight patients between 2004 and 2010. The following data were obtained: age, gender, clinical and electrocardiographic manifestations, Killip class, risk factors for ACS, markers of myocardial necrosis (CK-MB and troponin), creatinine clearance, left ventricular ejection fraction, inflammatory markers (C-reactive protein and erythrocyte sedimentation rate [ESR]), medication during hospital stay, angiographic findings, treatment (medical, percutaneous or surgical) and long-term outcome. Statistical data were expressed as percentages and absolute values. RESULTS: All eight patients were women, median age 49 years. Typical chest pain was present in 37.5%. Elevated ESR was observed in 85.7%. Three patients underwent coronary artery bypass grafting, three underwent percutaneous coronary angioplasty (two with bare-metal stents and one with a drug-eluting stent) and two were treated medically. In-hospital mortality was 25%. There were no deaths during a mean follow-up of 30 months. CONCLUSIONS: In our study, patients who were discharged home had good outcomes in long-term follow-up with medical, percutaneous or surgical treatment. ESR appears to be associated with ACS in TA.

O lupus eritematoso sistêmico (LES) é a mais comum das doenças auto-imunes sistêmicas, ocorrendo com maior freqüência no sexo feminino, usualmente na faixa etária entre 16 e 55 anos1,2. Embora os rins classicamente sejam os órgãos mais acometidos no LES, o coração e a circulação cardiopulmonar também podem ser afetados de forma significativa3. Nesse contexto, a ocorrência de edema agudo de pulmão associado à miocardite lúpica é rara e de tratamento imunossupressor específico ainda incerto.

ABSTRACT

We report on a 30-year-old female patient, with biological mitral valve prosthesis due to symptomatic mitral stenosis and a history of acute myocardial infarction and generalized tonic-clonic seizure episodes, visual hallucinations, cerebral thromboembolic events and, at present, chorea and acute carditis. The patient was diagnosed with active rheumatic fever (RF), systemic lupus erythematosus (SLE) and Antiphospholipid syndrome (APS). The combination of three unusual diagnoses in the same patient makes this a unique case, modifying patient treatment and prognosis.

ABSTRACT

Trata-se de um caso de uma paciente de 30 anos do sexo feminino, com prótese biológica valvar mitral em razão de estenose mitral sintomática e antecedentes de infarto agudo do miocárdio, episódios de convulsões tônico-clônicas generalizadas, alucinações visuais, eventos tromboembólicos cerebrais, apresentando no momento coreia e cardite aguda. Foram diagnosticados na paciente febre reumática em atividade, lúpus eritematoso sistêmico e síndrome do anticorpo antifosfolipídeo. A combinação de três diagnósticos incomuns em um mesmo paciente torna esse caso único, modificando o tratamento e seu prognóstico.

Se trata de un caso de una paciente de 30 años del sexo femenino, con prótesis biológica valvular mitral en razón de estenosis mitral sintomática y antecedentes de infarto agudo de miocardio, episodios de convulsiones tónico-clónicas generalizadas, alucinaciones visuales, eventos tromboembólicos cerebrales, presentando en el momento corea y carditis aguda. Fueron diagnosticados en la paciente fiebre reumática en actividad, lupus eritematoso sistémico y síndrome del anticuerpo antifosfolípido. La combinación de tres diagnósticos infrecuentes en un mismo paciente vuelve este caso único, modificando el tratamiento y su pronóstico.

ABSTRACT

There is increasing interest in autoimmune diseases, especially their relationship with cardiovascular disease. Rheumatoid arthritis in particular has been considered an independent risk factor for coronary artery disease in recent years. Various studies have aimed to clarify important aspects of risk stratification and treatment options in patients with rheumatoid arthritis, and specific therapies are being studied that promise to reduce their long-term cardiovascular risk. We performed a wide-ranging review of the literature to highlight the importance of atherosclerotic and inflammatory mechanisms in coronary artery disease. We also suggest strategies for risk stratification and treatment of cardiovascular disease in patients with rheumatoid arthritis.
