ABSTRACT

This before-after experimental study evaluated the impact of dental treatment on the oral health-related quality of life (OHRQoL) in children aged 6-8 years from Paranoá, DF, considering the presence or absence of cavitated dentin carious lesions pre- and post-treatment. The responsiveness and sensitivity of the questionnaires were also investigated. Caries was detected by using the Caries Assessment Spectrum and Treatment (CAST) instrument, while the impact of oral health on the children's health-related quality of life was assessed using the Brazilian version of the Child Perceptions Questionnaire (CPQ8-10), which was completed by the children and the Brazilian version of the Early Childhood Oral Health Impact Scale (B-ECOHIS), which was completed by their parents. Sociodemographic characteristics were also assessed. After the examinations, the children were categorized into two groups according to the presence (treatment/n = 34) or absence (control/n = 34) of cavitated dentin carious lesions. Restorative/curative care was provided to the treatment group, while the control group received preventive measures. OHRQoL was assessed at baseline and at four weeks post-treatment. No significant sociodemographic differences were observed between the groups. In the treatment group, the children and their families reported a greater impact of oral health on their OHRQoL in both questionnaires (p < 0.05). However, there was a significant reduction in the impact of oral health, with differences between the pre-treatment and post-treatment phases (p = 0.001). Good sensitivity and responsiveness were observed for both questionnaires. Dental treatment was found to reduce the negative impact of dental caries on OHRQoL in 6-8-year-old children, which was detected by both questionnaires (B-ECOHIS and CPQ8-10).

ABSTRACT

Uma perfeita harmonia do sorriso nos padrões estéticos atuais vai além de dentes em posição, restaurados e íntegros. A busca por dentes brancos tornou-se queixa frequente no consultório dental. Assim, o objetivo do presente trabalho é relatar a resolução estética e funcional de um caso clínico onde a paciente apresentava sorriso desarmonioso, dente desvitalizado escurecido e restaurações anteriores. O tratamento utilizado foi clareamento dental com peróxido de hidrogênio, clareamento com perborato de sódio e laminados cerâmicos de dissilicato de lítio. O resultado estético alcançado mostrou que o clareamento interno do dente desvitalizado foi insatisfatório e evidenciou a necessidade de tratamentos multidisciplinares. (AU)


A perfect smile harmony in today's esthetic standards goes beyond teeth in position, restored and intact, the search for white teeth has become a frequent complaint in the dental office. Thus, the objective of the present study is to report the aesthetic and functional resolution of a clinical case where the patient presented a disharmonious smile, darkened devitalized tooth and previous restorations. The treatment used was dental bleaching with hydrogen peroxide, bleaching with sodium perborate and ceramic laminates of lithium disilicate. The aesthetic results showed that the internal bleaching of the devitalized tooth was unsatisfactory and evidenced the need for multidisciplinary treatments. (AU)

ABSTRACT

OBJECTIVES: To evaluate whether an association exists between school performance, quality of life, dental caries, and dental pain in children between 6 and 8 years of age. METHODS: All 6- to 8-year-old children enrolled in the second year of the six public schools in Paranoá-FD were invited to participate in this cross-sectional study. In total, 374 consented. Dental caries was recorded according to the Caries Assessment Spectrum and Treatment (CAST) index by two trained examiners. Dental pain was assessed during clinical examination. The Child Perceptions Questionnaire (CPQ) was used to assess the oral health-related quality of life, and sociodemographic data were collected. The child's school performance was measured through Provinha Brasil according to the regulations of the Ministry of Education. Descriptive statistics, bivariate analysis, and a multivariate regression model were used for data analysis. RESULTS: 185 boys and 189 girls participants with a mean age of 7.2 (±0.5) years were included in the study. The prevalence of dentin carious lesions for this group was 40.6 and 49.2 percent, including enamel-only lesions. The prevalence of dental pain was 2.8 percent. A high performance in reading (63.6 percent) and in mathematics (79.4 percent) was observed and were not associated with the presence of cavitated dentin carious lesions or dental pain. A significant association was found between school performance and the final CPQ score. CONCLUSION: School performance as measured by the Provinha Brasil examination was not associated with carious lesions or dental pain. Students with lower school performance had higher CPQ scores.

ABSTRACT

PURPOSE: The purpose of this systematic review was to verify whether child and adolescent oral health affected academic performance. METHODS: A literature search conducted in March 2017 on PubMed, Lilacs, Web of Science, and Scopus databases identified 2,009 papers, six of which were included in the final review. Quality appraisal and risk of bias were evaluated using the quality assessment tool for observational cohort and cross-sectional studies. RESULTS: Two papers were classified as being of good quality, one as fair, and three as poor. In four publications, oral health conditions were measured by taking only dental caries into account, while in two others treatment needs and dental trauma were also considered. Although four papers concluded that children's academic performance and poor oral health were associated, the results were not considered reliable because of the high risk of bias. The two papers classified as being of good quality did not show an association between oral health and academic performance, unless mediated by socioeconomic factors. CONCLUSION: Further well-designed studies are required to demonstrate whether children's oral health can have a negative influence on their academic performance.

ABSTRACT

Neste relato de caso clínico, uma paciente de 63 anos de idade apresentou-se totalmente edêntula e usuária de prótese totais há mais de 40 anos, com mordida cruzada posterior unilateral e insatisfação estética. Após uma explanação sobre as opções de tratamento existentes, a paciente optou pelos implantes dentários e novas próteses totais fixas. Após a realização dos exames radiográficos e levantamento bilateral do seio maxilar para enxertia com osso do ramo mandibular, oito implantes foram colocados nas regiões posteriores, e cinco implantes colocados entre os forames mentuais, sendo as próteses reembasadas com material macio. Após a osseointegração, foram realizados os procedimentos de transferência e montagem dos dentes artifi ciais. Ambas as infraestruturas foram desenhadas para a individualização das coroas, sendo fundidas em liga de Ni-Cr, e recebendo o suporte labial em porcelana rosa. Após a prova intraoral, o aspecto metálico foi mascarado com agente opacificador, as embocaduras dos parafusos selados com fita teflon, e as coroas cerâmicas de dissilicato de lítio cimentadas com agente resinoso dual. Além da possibilidade de higienização e troca individual das coroas em momentos de falhas, o que evita um custo elevado se a infraestrutura fosse completamente em cerâmica, esta modalidade de tratamento resgata com segurança a harmonia e a naturalidade do sorriso.


In this clinical case report, a 63 years-old patient presented with complete conventional dentures (more than 40 years of use), having posterior unilateral crossbite and esthetic failure. After explaining the available treatment options, dental implants and new complete fixed prostheses were selected. Upon radiographic and bilateral sinus grafting with autogenous bone from the mandibular ramus, 8 dental implants were placed at the posterior maxillary regions and 5 dental implants between the mental foramina, being the prostheses relined with a soft material. After the osseointegration period, the transferring procedures were made for artifi cial tooth setup. Both maxillary and mandibular infrastructures were designed to receive individual crowns, cast in Ni-Cr alloy, and receive labial support with pink porcelain. After intra-oral try-in, the metallic aspect was masked with opaque paste, the screw access channels sealed with Tefl on tape, and the lithium disilicate crowns cemented with a dual resin agent. Besides the chance for homecare oral cleaning and individual crown replacement in case of failures to avoid excessive costs compared to complete ceramic infrastructures, this treatment modality provides safety and harmony for a more natural-looking smile.
