ABSTRACT

Traumatic brain injury (TBI) can produce temporary biochemical imbalance due to leaks through cell membranes or disruption of the axoplasmic flow due to the misalignment of intracellular neurofilaments. If untreated, TBI can lead to Alzheimer's, Parkinson's, or total disability. Mild TBI (mTBI) accounts for about about 90 percent of all TBI cases. The detection of TBI as soon as it happens is crucial for successful treatment management. Neuroimaging-based tests provide only a structural and functional mapping of the brain with poor temporal resolution. Such tests may not detect mTBI. On the other hand, the electroencephalogram (EEG) provides good spatial resolution and excellent temporal resolution of the brain activities beside its portability and low cost. The objective of this paper is to provide clinicians and scientists with a one-stop source of information to quickly learn about the different technologies used for TBI detection, their advantages and limitations. Our research led us to conclude that even though EEG-based TBI detection is potentially a powerful technology, it is currently not able to detect the presence of a mTBI with high confidence. The focus of the paper is to review existing approaches and provide the reason for the unsuccessful state of EEG-based detection of mTBI.

ABSTRACT

The cuffless blood pressure (BP) measurement allows for frequent measurement without discomfort to the patient compared to the cuff inflation measurement. With the availability of a large dataset containing physiological waveforms, now it is possible to use them through different learning algorithms to produce a relationship with changes in BP. In this paper, a novel cuffless noninvasive blood pressure measurement technique has been proposed using optimized features from electrocardiogram and photoplethysmography based on multivariate symmetric uncertainty (MSU). The technique is an improvement over other contemporary methods due to the inclusion of feature optimization depending on both linear and nonlinear relationships with the change of blood pressure. MSU has been used as a selection criterion with algorithms such as the fast correlation and ReliefF algorithms followed by the penalty-based regression technique to make sure the features have maximum relevance as well as minimum redundancy. The result from the technique was compared with the performance of similar techniques using the MIMIC-II dataset. After training and testing, the root mean square error (RMSE) comes as 5.28 mmHg for systolic BP and 5.98 mmHg for diastolic BP. In addition, in terms of mean absolute error, the result improved to 4.27 mmHg for SBP and 5.01 for DBP compared to recent cuffless BP measurement techniques which have used substantially large datasets and feature optimization. According to the British Hypertension Society Standard (BHS), our proposed technique achieved at least grade B in all cumulative criteria for cuffless BP measurement.

ABSTRACT

Following hospital discharge, millions of patients continue to recover outside formal healthcare organizations (HCOs) in designated transitional care periods (TCPs). Unplanned hospital readmissions of patients during TCPs adversely affects the quality and cost of care. In order to reduce the rates of unplanned hospital readmissions, we propose a real-time patient-centric system, built around applications, to assist discharged patients in remaining at home or in the workplace while being supported by care providers. Discrete-event system modeling techniques and supervisory control theory play fundamental roles in the system's design. Simulation results and analysis show that the proposed system can be effective in documenting a patient's condition and health-related behaviors. Most importantly, the system tackles the problem of unplanned hospital readmissions by supporting discharged patients at a lower cost via home/workplace monitoring without sacrificing the quality of care.
