ABSTRACT

INTRODUCTION: Prechtl's method (GMA) is a test for the functional assessment of the young nervous system. It involves a global and a detailed assessment of the general movements (GMs) and has demonstrated validity. Data on the reliability of both assessments in the preterm period are scarce. This study aimed to evaluate the inter-rater reliability for the global and detailed assessments of the preterm writhing GMA. MATERIALS AND METHODS: The study participants were 69 infants born at <37 gestational weeks and admitted to the neonatal intensive care unit. They were randomly assigned to five pairs of raters. Raters assessed infants' GMs using preterm videos. Outcome variables were (a) the GMs classification (normal versus abnormal; normal versus abnormal subcategories) and (b) the general movements optimality score (GMOS), obtained through the global and detailed assessments. The Gwet's AC1 and the intraclass correlation coefficient (ICC) were calculated for the GMs classification and the GMOS, respectively. RESULTS: The global assessment presented an AC1 = 0.84 [95% CI = 0.54,1] for the GMs binary classification and an AC1 = 0.67 [95% CI = 0.38,0.89] for the GMs classification with abnormal subcategories. The detailed assessment presented an ICC = 0.72 [95% CI = 0.39,0.90] for the GMOS. CONCLUSIONS: Inter-rater reliability was high and substantial for the global assessment and good for the detailed assessment. However, the small sample size limited the precision of these estimates. Future research should involve larger samples of preterm infants to improve estimate precision. Challenging items such as assessing the neck and trunk, poor repertoire GMs, and tremulous movements may impact the preterm writhing GMA's inter-rater reliability. Therefore, ongoing training and calibration among raters is necessary. Further investigation in clinical settings can enhance our understanding of the preterm writhing GMA's reliability.

ABSTRACT

Congenital viral infections are believed to damage the developing neonatal brain. However, whether neonates exposed to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) show manifestations of such damage remains unclear. For neurodevelopment evaluation, general movement assessments have been shown to be effective in identifying early indicators of neurological dysfunction, including the absence of fidgety movements. This study compared the early motor repertoire by general movement assessment at three to five months of age in neonates who were or were not prenatally exposed to SARS-CoV-2 to determine whether infants prenatally exposed to SARS-CoV-2 are at risk of developing neurological disorders. Fifty-six infants, including 28 in the exposed group of mothers without vaccination who had no need for intensive care and likely had SARS-CoV-2 infection close to the time of pregnancy resolution and 28 infants in the nonexposed group, were videotaped to compare their detailed early motor repertoires, in which a motor optimality score-revised (MOS-R) was calculated using Prechtl's method by using the chi-square or Mann-Whitney U tests. In the exposed group, 3 (11%) infants showed the absence of fidgety movements with a total MOS-R<14 points, and 3 (11%) other infants showed abnormal fidgety movements. Between groups, atypical body symmetry (p = 0.009) and MOS-R values were significantly lower (Z = -3.08, p = 0.002), with a large size effect (Cohen's d = 0.97). The consequences of this new virus go beyond the health of the pregnant mother, and these consequences in some of the infants in the exposed group are likely not transitory because of the absence of fidgety movements between 3-5 months; thus, these babies are at increased risk of developing a serious neurological disorder.

ABSTRACT

The Prechtl General Movement Assessment (GMA) has become a cornerstone assessment in early identification of cerebral palsy (CP), particularly during the fidgety movement period at 3-5 months of age. Additionally, assessment of motor repertoire, such as antigravity movements and postural patterns, which form the Motor Optimality Score (MOS), may provide insight into an infant's later motor function. This study aimed to identify early specific markers for ambulation, gross motor function (using the Gross Motor Function Classification System, GMFCS), topography (unilateral, bilateral), and type (spastic, dyskinetic, ataxic, and hypotonic) of CP in a large worldwide cohort of 468 infants. We found that 95% of children with CP did not have fidgety movements, with 100% having non-optimal MOS. GMFCS level was strongly correlated to MOS. An MOS > 14 was most likely associated with GMFCS outcomes I or II, whereas GMFCS outcomes IV or V were hardly ever associated with an MOS > 8. A number of different movement patterns were associated with more severe functional impairment (GMFCS III-V), including atypical arching and persistent cramped-synchronized movements. Asymmetrical segmental movements were strongly associated with unilateral CP. Circular arm movements were associated with dyskinetic CP. This study demonstrated that use of the MOS contributes to understanding later CP prognosis, including early markers for type and severity.

INTRODUCCIÓN: El instrumento COPCORD permite identificar el dolor músculoesquelético y enfermedades reumáticas como artrosis, artritis reumatoide, lumbalgia. El objetivo de esta investigación es validar y adaptar transculturalmente el instrumento COPCORD en la población indígena como prueba de tamizaje para la detección de estas enfermedades. MÉTODOS: Se trata de un estudio descriptivo, el universo fue de 210 indígenas mayores de 18 años que residen en Saraguro ­ Loja, Ecuador. Durante el periodo del 1 diciembre de 2016 al 30 de enero de 2017. El COPCORD se ajustó al lenguaje español, se realizó adecuación y validación transcultural al contexto del grupo indígena. La información fue ingresada en el programa estadístico Stata Versión 11, se analizó: alfa de Cronbach, matrices de correlación con la prueba de Spearman. Se correlacionó las variables con el diagnóstico establecido por el reumatólogo. RESULTADOS: La edad promedio fue 46.1 años, 64.7 % del género femenino, la prueba de tamizaje para detectar una enfermedad reumática tiene sensibilidad del 92.3 % y especificidad de 57.9 %. La consistencia interna del cuestionario presentó unidimensionalidad en los apartados de carga biomecánica y capacidad funcional; y multidimensionalidad en la trayectoria del dolor músculo - esquelético y comorbilidades. Se observaron correlaciones significativas del COPCORD comparado con la evaluación del reumatólogo. CONCLUSIONES: Al realizar la validación y adaptación transcultural del cuestionario COPCORD se demostró su utilidad como prueba de tamizaje para la detección del dolor músculo - esquelético y enfermedades reumáticas en la población aplicada


BACKGROUND: The COPCORD instrument allows the identification of musculoskeletal painand rheumatic diseases such as osteoarthritis, rheumatoid arthritis, low back pain. The aim of this research is to validate and cross-culturally adapt the COPCORD instrument in the indigenous population as a screening test for the detection of these diseases. METHODS: It is a descriptive study, the universe of 210 indigenous people over 18 years residing in Saraguro - Loja, Ecuador. During the period from December 1, 2016 to January 30, 2017. The COPCORD was adjusted to the Spanish language, a transcultural adaptation and validation was made to the context of the indigenous group. The information was entered into the statistical program of Stata Version 11, analyzed: Cronbach's alpha, correlation matrixes with the Spearman test. The variables were correlated with the diagnosis established by the rheumatologist. RESULTS: The average age was 46.1 years, 64.7 % of the female gender, the screening test to detect a rheumatic disease has sensitivity of 92.3 % and specificity of 57.9 %. The internal consistency of the questionnaire presented a dimensionality in the biomechanical load and functional capacity sections; and multidimensionality in the path of musculoskeletal pain and comorbidities. Significant correlations of the COPCORD were observed compared with the rheumatologist's evaluation. CONCLUSIONS: By carrying out the cross-cultural validation and adaptation of the COPCORD questionnaire, its usefulness could be demonstrated as a screening test for the detection of musculoskeletal pain and rheumatic diseases in the population of Saraguro.of approach including the laparoscopic approach.

RESUMENOBJETIVO: Determinar las características socio-demo-gráficas asociadas a la presencia de dolor músculo-esquelético (MSQ) en personas mayores de 18 años que viven en el Cantón Cuenca.PACIENTES Y MÉTODOS: Estudio transversal analítico, realizado en su-jetos del área rural y urbana, mayores de 18 años, utilizando la metodología COPCORD (Community Oriented Program for the Con-trol of Rheumatic Diseases), adaptada para Ecuador. Se identificaron a los individuos con síntomas reumáticos, casa por casa. La revisión para confirmar el diagnóstico realizó un médico Reumatólogo.El análisis estadístico fue univariado y multi-variado. RESULTADOS:Participaron 2500 personas encuestadas, el 59,6% correspondió al sexo femenino, con una edad promedio de 42,8 años, el 25,5% de los participantes realizaban actividades domésticas. El dolor MSQ se presentó en el 32,2% de las personas, el cual se asoció sig-nificativamente a vivir en el área rural, tener menor escolaridad, ingresos económicos por debajo de 340 dólares, carga física ma-yor de 8 libras, mayor discapacidad funcional y el cocinar con leña en el sector rural. CONCLUSIÓN: El dolor MSQ tiene un alta prevalencia en la población de Cuenca, Ecuador; producien-do discapacidad especialmente en el área rural. se asocia con menor nivel de escola-ridad, ingresos económicos bajos y mayor carga física.


OBJECTIVE: To determine the socio-demographic cha-racteristics associated with the presence of musculoskeletal pain (MSK) in people over 18 years who are living in Cuenca.PATIENTS AND METHODSAn analytical cross-sectional study was con-ducted in subjects of rural and urban areas, who are over 18 years, using the metho-dology COPCORD (Community Oriented Program for the Control of Rheumatic Di-seases) adapted to Ecuador. We identified individuals with rheumatic symptoms, house by house. The revision to confirm the diagnosis was made by a rheumatologist. The statistical analysis was univariate and multivariate. RESULTSA total of 2500 people were surveyed. The 59.6% were female, with an average age of 42.8 years, the 25.5% of participants perfor-med domestic activities. The MSK pain oc-curred in 32.2% of people, which was signi-ficantly associated with living in rural areas, have lower education, lower income to $ 340, increased physical load of 8 pounds, more functional disability, and cooking with firewood in the rural area. CONCLUSIONThe MSK pain has a high prevalence in the population of Cuenca, Ecuador; producing disability especially in rural areas. It is asso-ciated with lower levels of education, lower income and increased physical load
