ABSTRACT

The paper discusses the advantages offered by two regimens of therapy of Hodgkin's disease in children and adolescents (139)--DAL-HD (versions 87 and 90) (83) and SPbHD-05 (56). Survival rates were fairly high (OS--94.3%; DFS-- 90.0% and EFS--81.2%); they differed depending on risk group. Both the potential of a largen choice of prognostic criteria used in risk grading and the hazards from akylating drugs and anthracyclines were demonstrated.

ABSTRACT

Based on an analysis of 60 cases the authors have shown that identification and target investigation of signal lymph nodes using intraoperative staining with special dyes allows correct assessment of the stage of malignant disease. The method also allows treatment of the patient with an adequate, corresponding to up-to-date standards scheme of combined and complex treatment, gives possibilities of exact intraoperative diagnosis of lymphogenic metastasis and, as a result, allows extended operative intervention up to radical volumes.

ABSTRACT

A prognostic score was developed in the course of treatment of 551 pediatric patients with Hodgkin's disease stage I-IV (1967-2001). It took care of such significant factors of uni-and multivariate analysis as age (11 years or more), peripheral lymph node involvement (5cm or more), systemic symptoms (B), histological pattern for lymphocytic depletion, IVB stage and/or extranodal disease (E) and biological rate of tumor process ("b"). As a result, three groups at risk were identified: favorable prognosis (65.8%: overall survival - 92%, relapse-free - 67%), medium risk (17.8%: overall survival - 67%, relapse-free 48%) and high risk (16.4%: overall survival - 38%, relapse-free 25%). Our data may be used to estimate the scope of risk-adapted therapy for Hodgkin's disease.

ABSTRACT

Effects of operation of carotid endarterectomy (CE) on indicators of quality of life were investigated in patients with ischemic disease of the brain (IDB). The quality of life was assessed in 45 patients at the preoperative stage and in the remote postoperative period with the help of questionnaire SF-36. All the patients were divided into two groups according to the degree of the initial neurological symptoms. The first group included 25 patients with an "asymptomatic" or "minimally symptomatic" course of IDB. The second group had 20 patients with a stable neurological defect resulting from "great" ischemic stroke. It was shown that in the first group of patients there was a reliable improvement of the life quality after CE in 6 out of 8 indicators. In the second group a reliable improvement of the life quality was noted in one of the 8 indicators only. The average values of the life quality indicators after CE in the first group patients were close to those in healthy population. At the same time in the second group they were considerably lower. The carotid endarterectomy gives the maximum effect of improving the patients' life quality if fulfilled at the early stages of IDB and is considered to be of probhylactic character.
