ABSTRACT

UNLABELLED: New Caledonia is a French territory of Pacific Ocean, where frequent dengue outbreaks occur. In 1995 and 1996, 3042 cases (including 18.3% children) were diagnosed in Pasteur Institute of Noumea. PATIENTS AND METHODS: This work was a clinical and biological study of 68 in-patients of different ethnical groups in children. Among these young patients, 14 cases of dengue hemorrhagic fever occurred. RESULTS: The children were admitted to hospital after an average of 2.7 days of complaint, and during 6.7 days. The most important symptoms were fever at 39 degrees C (100%) during 2.2 days, sweatings (100%), malaise (57%) and headache (50%). Forty children had, at least, one hemorrhagic symptom. Leucopenia was noticed in 42 children, on an average of 4.2 days. A thrombocytopenia (mean 114,746 platelets/mm3) occurred in 5.3 days in 34 children (six cases with less than 10,000 platelets/mm3). A bacterial infection arised in 23.5% of children. CONCLUSION: Dengue is an important problem of public health, which occurs in all population, including the young children. The fight against dengue fever by a rapid destruction of larvae and adult mosquitoes led to the decrease of the outbreak.

ABSTRACT

PURPOSE: This paper relates a technical and functional study of teleradiology network after two years of duty in Mont-de-Marsan's general hospital. MATERIALS AND METHODS: We studied from november 1995 to november 1997, the following items: the number of transmitted files, the medical departments involved, the degree of emergency, the purpose of the transmission, the number and type of images, if patient's transfer is wished or not, and the way the answer is given. RESULTS: During this 2-year period, 94 files were transmitted mainly to the University Hospital. The radiology department was the main user (36%), followed by the emergency department (29%). A notion of emergency occurred in 63% of the cases. Most of the cases where for radiodiagnosis (48%), and therapeutic advice (42%), mainly in neurosurgery. Each file consisted of 3 to 4 images of which 77% were CT-Scan. The patient transfer was wished for in 13 files. Answer was given by computerised methods in 43% of the cases, and otherwise by telephone. CONCLUSION: Despite a better medical management of patients, this network is underused. The main reasons being the lack of information, the lack of time and the fact the network has not yet found its place in our professional environment.

ABSTRACT

The author reports a personal series of ten congenital male urethroceles to recall the fact that this is only a relatively rare anomaly, that its surgical treatment performed before the stage of urinary abscess gives good results as it is performed on healthy urethral tissue, and finally, to emphasise the diagnostic difficulties, as these patients present varied and misleading clinical symptoms and signs.

ABSTRACT

The authors relate one case of obstruction by emboly in the renal artery. The diagnosis were too late for conservative surgery. In this case, they insist on the particulary signs of that nephretic colic and they show the diagnosis measures which are able to permit a conservative surgery in urgency.
