ABSTRACT

Bevacizumab plus paclitaxel therapy for recurrent breast cancer did not prolong overall survival(OS)in clinical trials, but it was efficacious for treating life-threatening HER2-negative recurrent breast cancer. This regimen is often used in daily clinical practice by breast surgeons. However, bevacizumab therapy results in unique adverse events, of which proteinuria and hypertension are relatively frequent. Moreover, the symptoms often improve on reducing the dose or discontinuing the drug. In this case, bevacizumab administration caused delayed wound healing, making subsequent anticancer treatment difficult, and consequently we could not prolong the patient's life.

ABSTRACT

BACKGROUND: Accessory breast carcinomas of the axilla of males are rare, and primary breast neuroendocrine tumors (BNETs) are rare as well. We present a case of a BNET arising in the axilla of a man. CASE PRESENTATION: A 64-year-old Japanese man presented with a hard 15-mm mass in the axilla and axillary lymph node swelling. Histopathological examination of the incisional biopsy specimen revealed a neuroendocrine carcinoma. Therefore, wide radical excision of the axillary tumor and axillary lymph node dissection were performed. Hematoxylin and eosin staining showed that the solid tumor was mainly located in the subcutaneous adipose tissues and appeared to invade the skin. The tumor phenotypes were positive for CAM 5.2, synaptophysin, estrogen receptor, progesterone receptor, and GATA-binding protein 3; they were negative for human epidermal growth receptor 2. The neuroendocrine component comprised more than 90% of the tumor, and the Ki-67 index was 21%. These results indicated that the tumor was a BNET. This patient underwent adjuvant chemotherapy, endocrine therapy, and radiotherapy. CONCLUSIONS: BNET cases in males are rare. The clinical and histological criteria as well as treatment for these rare cases are discussed.

ABSTRACT

We assessed the feasibility of MR digital subtraction angiography (DSA) using parallel imaging and keyhole data sampling in the diagnosis of cerebrovascular diseases (CVDs) in 11 patients. Their diseases included arterial trunk stenosis/occlusion ( n=4), aneurysm ( n=3), arteriovenous malformation ( n=2), venous angioma ( n=1), and sinus thrombosis ( n=1). The technique depicted not only anatomical features, comparably to MR angiography ( n=10/11), but also hemodynamics such as collateral flow at a temporal resolution of 1.68 s/frame. When compared with conventional angiograms ( n=7), details were missed in four patients (incomplete demonstration of aneurysmal neck in two and poor separation of AVM components in two). Although inferior to conventional angiography, this technique can provide both anatomical and hemodynamic information of CVDs.

ABSTRACT

MR digital subtraction angiography (DSA) visualizes intracranial vasculature using a rapid T1-weighted sequence and a bolus injection of gadolinium. Although two-dimensional sequences are most frequently used, we applied a three-dimensional technique in combination with a fast method of k-space filling to improve both the temporal and spatial resolutions. In this preliminary study, we assessed the feasibility of using this technique for the diagnosis of brain tumors in 21 patients by reviewing the obtained images and, in 10 patients, comparing the images with conventional angiograms. MR DSA visualized a tumor stain in 11 patients, a tumor mass effect in 9, and tumor-related vessels in 5. In 9 of the 10 patients for whom conventional angiograms were available, the two kinds of angiograms corresponded well. Three-dimensional MR DSA using our technique is a useful adjunct to conventional MR imaging for the visualization of tumor hemodynamics and, in some cases, tumor-related vessels and mass effects.

ABSTRACT

We evaluated the feasibility of MR cisternography by the balanced fast-field-echo (bFFE) sequence, comparing with that by a turbo-spin-echo (TSE) sequence, for cerebellopontine angle lesions on a 1.5-T imager (Gyroscan Intera, Philips, Best, The Netherlands). The bFFE MR cisternograms depicted target cranial nerves with less cerebrospinal fluid pulsation artifacts than TSE cisternograms and visualized an acoustic schwannoma in 6 of 44 patients with suspicion and a causative vessel of hemifacial spasm in all of 3 patients in a short scanning time (1 min 53 s). The bFFE sequence can be promising for MR cisternography in the diagnosis of cerebellopontine angle lesions.

ABSTRACT

We describe the diffusion-weighted (DW) MR imaging findings in three cases of spinal cord infarction using a recently developed single-shot fast spin-echo (SSFSE) technique. The SSFSE-DW MR images, which were obtained 20 hours, 3 days, and 18 days, respectively, after the ischemic event, demonstrated conspicuous areas of hyperintensity in the affected portions. Follow-up DW MR images, obtained in two of the patients at 17 days and 3 months, respectively, showed persistent decreased apparent diffusion coefficient values. SSFSE-DW imaging of the spinal cord may provide additional information for assessment of ischemic changes.

ABSTRACT

The advent of a multi-detector row helical CT has made it possible to attain images over a broader area with good spatial resolution. We assessed whether postmyelographic CT scans obtained using this system provided more information than conventional imaging techniques. Postmyelographic CT scans were preoperatively obtained in 46 patients using a multi-detector row helical CT system. Reconstructed images in the sagittal and coronal planes in all patients and curved reformation images along the dural sac in 37 patients were compared with myelograms and conventional postmyelographic CT scans. In 34 patients comparison was also made with MR images. The multi-detector row CT images demonstrated deformities of the dural sac more clearly than the other modalities in 39 of the 46 patients. They also provided the best visualization of nerve root abnormalities in 24 of the 46 patients and clearly revealed the presence of spurs in all 22 patients with spinal canal stenosis. Postmyelographic CT scans made using a multi-detector row helical CT system provide more information on the dural sac, nerve sleeves, and their contents than other imaging techniques.

ABSTRACT

PURPOSE: The advent of multidetector-row computed tomography (MDCT) has enabled images with good spatial resolution to be obtained over a wide range in a short scanning time. Our purpose was to determine whether CT angiography using the MDCT system could effectively depict extracranial-intracranial (EC-IC) bypass routes. METHOD: Helical CT angiography was performed using an MDCT scanner in 12 patients who had undergone EC-IC bypass surgery: 10 patients had undergone superficial temporal artery-middle cerebral artery (MCA) anastomosis, 1 patient had undergone an encephaloduroarteriosynangiosis procedure for the treatment of moyamoya disease, and 1 patient had undergone an external carotid artery-MCA anastomosis using a graft. The resulting CT angiograms were visually evaluated for their depiction of the EC-IC bypass route. Conventional angiograms were available for comparison in all 12 patients. RESULTS: The EC-IC bypass was visualized to be patent at the site of anastomosis in all 12 patients. Branches of the MCA secondary or more to those connected to a donor artery were demonstrated in 9 patients, whereas MCA branches immediately distal to the anastomosis were demonstrated in 3 patients. The CT angiography findings corresponded well with the conventional angiography findings in all patients. CONCLUSION: Helical CT angiography using the MDCT technique is an effective method for visualizing EC-IC bypass routes.
