ABSTRACT

Background: We investigated the effects of a 12-week home-based exercise program delivered with virtual or minimal supervision on the physical and cognitive function of community-dwelling older adults in the context of the COVID-19 pandemic in Brazil. Methods: The study was registered on the Brazilian Registry of Clinical Trials platform (code: RBR-8qby2wt). Thirty-eight older adults (81% female and 68±7 years old), non-disabled, and without cognitive impairment or dementia, were randomly assigned to a 12-week home-based exercise program: 1) virtual supervision (classes remotely delivered through video conference by trained staff), or 2) minimal supervision (once-weekly contact to touch base through standardized text messages). The participants initially performed two sets of 10 repetitions three times a week, with a 60-second interval. The volume and complexity of the exercises were progressively increased. (e.g., the number of sets increased to 3 and later to 4). At baseline and follow-up, we collected remote measurements of physical function (muscle strength and power, functional muscular fitness) and cognition (processing speed, inhibitory control, verbal fluency). Results: Participants in the minimal supervision home-based exercise group significantly improved the Stroop test (-1.6 sec, 95% CI = -3.20; -0.09). No significant between-group differences were observed for physical and cognitive outcomes. Conclusion: A home-based exercise program delivered with virtual or minimal supervision can produce similar effects, and may help to maintain physical and cognitive capabilities among healthy, high-functioning older adults who experienced mobility restrictions due to the COVID-19 pandemic in Brazil.

ABSTRACT

AIM: The objective of this study was to compare the effects of virtual supervision and minimal supervision in a 12-week home-based physical exercise program on the mental health and quality of life of community-dwelling older adults. METHODS: This study presents a secondary analysis of a randomized clinical trial conducted during the COVID-19 pandemic. The sample comprised 38 elderly men and women (81.6% women, mean age 68 ± 6.4 years; mean weight 69.8 ± 12.1 km) without physical and/or cognitive impairments. The volunteers were divided into two groups: one group received virtual supervision (monitored via videoconference), while the other group received minimal supervision (monitored through weekly text messages). Both groups followed the same home-based physical exercise program, consisting of three sessions per week, differing only in the supervision strategy. Mental health was assessed using the Depression, Anxiety, and Stress Scale (DASS-21) in conjunction with the POMS questionnaire, which evaluates mood, and quality of life was assessed using the WHOQOL-OLD questionnaire. RESULTS: Our findings demonstrated statistically significant superiority of virtual supervision compared with minimal supervision in relation to depression (-2.92, 95% confidence interval = -5.22 to -0.63). Conversely, minimal supervision exhibited superiority over virtual supervision regarding quality of life (-6.70, 95% confidence interval = -11.66 to -1.73). CONCLUSIONS: Virtual supervision yielded better outcomes for the depression indicator, while minimal supervision favored the quality of life of older adults at the conclusion of the 12-week home-based physical exercise program. ABSTRACT

O artigo objetivou relatar as experiências de uma monitora extensionista no Programa Segundo Tempo Paradesposto (PSTP) com a identificação e análise de situações de conflitos e as estratégias de mediação dos mesmos. Trata-se de um artigo de relato de experiência referente às atividades realizadas em fevereiro de 2019 a março de 2020. Foi feita uma discussão dos relatos de experiência do monitor extensionista, referente ao olhar crítico às vivências compartilhadas com crianças e adolescentes com deficiência intelectual. Foi identificado que os conflitos foram de natureza intrapessoal, interpessoal e grupo, e que a mediação dos conflitos foi feita principalmente pela mediação preventiva. As estratégias adotadas se mostraram efetivas na melhora substancial do comportamento dos beneficiados.


The article aimed to report the experiences of an extensionist monitor in the Second Half Parasport Program (SHPP) with the identification and analysis of conflict situations and their mediation strategies. This is an experience report article referring to the activities carried out from February 2019 to March 2020. A discussion was made of the experience reports of the extensionist monitor, referring to the critical look at the experiences shared with children and adolescents with intellectual disabilities. It was identified that the conflicts were of an intrapersonal, interpersonal and group nature, and that the mediation of conflicts was done mainly through preventive mediation. The strategies adopted proved to be effective in substantially improving the behavior of the beneficiaries.


El artículo tuvo como objetivo relatar las experiencias de un monitor extensionista en el Programa Segundo Tempo Paradesposto (PSTP) con la identificación y análisis de situaciones de conflicto y sus estrategias de mediación. Este es un artículo de relato de experiencia referente a las actividades realizadas de febrero de 2019 a marzo de 2020. Se discutieron los relatos de experiencia del monitor extensionista, referente a la mirada crítica a las experiencias compartidas con niños y adolescentes con discapacidad intelectual. Se identificó que los conflictos eran de carácter intrapersonal, interpersonal y grupal, y que la mediación de conflictos se hacía principalmente a través de la mediación preventiva. Las estrategias adoptadas demostraron ser efectivas para mejorar sustancialmente el comportamiento de los beneficiarios.

ABSTRACT

Inherited thrombophilia (e.g., venous thromboembolism, VTE) is due to rare loss-of-function mutations in anticoagulant factors genes (i.e., SERPINC1, PROC, PROS1), common gain-of-function mutations in procoagulant factors genes (i.e., F5, F2), and acquired risk conditions. Genome Wide Association Studies (GWAS) recently recognized several genes associated with VTE though gene defects may unpredictably remain asymptomatic, so calculating the individual genetic predisposition is a challenging task. We investigated a large family with severe, recurrent, early-onset VTE in which two sisters experienced VTE during pregnancies characterized by a perinatal in-utero thrombosis in the newborn and a life-saving pregnancy-interruption because of massive VTE, respectively. A nonsense mutation (CGA > TGA) generating a premature stop-codon (c.1171C>T; p.R391*) in the exon 6 of SERPINC1 gene (1q25.1) causing Antithrombin (AT) deficiency and the common missense mutation (c.1691G>A; p.R506Q) in the exon 10 of F5 gene (1q24.2) (i.e., FV Leiden; rs6025) were coinherited in all the symptomatic members investigated suspecting a cis-segregation further confirmed by STR-linkage-analyses [i.e., SERPINC1 IVS5 (ATT)5-18, F5 IVS2 (AT)6-33 and F5 IVS11 (GT)12-16] and SERPINC1 intragenic variants (i.e., rs5878 and rs677). A multilocus investigation of blood-coagulation balance genes detected the coexistence of FV Leiden (rs6025) in trans with FV HR2-haplotype (p.H1299R; rs1800595) in the aborted fetus, and F11 rs2289252, F12 rs1801020, F13A1 rs5985, and KNG1 rs710446 in the newborn and other members. Common selected gene variants may strongly synergize with less common mutations tuning potential life-threatening conditions when combined with rare severest mutations. Merging classic and newly GWAS-identified gene markers in at risk families is mandatory for VTE risk estimation in the clinical practice, avoiding partial risk score evaluation in unrecognized at risk patients.

ABSTRACT

Este artigo foi elaborado a partir da pesquisa intitulada "Poéticas e marginalidade: experiência no Projeto Cidadãos Cantantes". Ele aborda parte da experiência coletiva vivida no Projeto Cidadãos Cantantes, focalizando os modos pelos quais se produz grupalidade, formas de estar junto ou de estabelecer convivência, e apresenta uma análise sobre tais procedimentos. O Projeto Cidadãos Cantantes é, atualmente, composto pela Oficina de Coral Cênico e pela Oficina de Dança e Expressão Corporal, e surgiu a partir do movimento da luta antimanicomial, em 1992. Através da participação no projeto e de uma parceria de oito, as autoras elaboraram narrativas que conduzem a análise da experiência em questão. Compreende-se que, ao buscar promover encontros entre pessoas quaisquer e não a partir de uma definição ou diagnóstica, o Projeto pode agregar pessoas de forma desejante e cooperativa. Desta forma, os modos de relações vigentes nas oficinas do Projeto fortalecem a produção de encontros pautados na potência, na criação e no respeito às diferenças.
