ABSTRACT

Objetivo. Revisar los resultados clínicos y radiológicos de una serie de artroplastias de rodilla con plataforma móvil (T.A.C.K®, Link). Material y método. Estudio retrospectivo de 40 artroplastias intervenidas en nuestro centro por cuatro cirujanos distintos usando la misma técnica quirúrgica. Fueron incluidas en el estudio todas aquellas prótesis implantadas entre marzo del 91 y diciembre del 92. A los 33 pacientes que acudieron a control se les valoró el arco de movilidad, estabilidad articulary la presencia de dolor según la prueba HSS (Hospital for Special Surgery) y se les solicitó una radiografía de control. Resultados. Cinco prótesis (15%) han sido revisadas, una por infección profunda que precisó artrodesis, otra por disfunción femoropatelar, dos por incorrecta colocación de los implantes y una por indicación incorrecta. Estas tres últimas se resolvieron con una prótesis constreñida (Rotatoria®, Link). Sólo se ha constatado desgaste grave del polietileno en un caso. Conclusión. Con este tipo de prótesis, en ausencia de mala técnica y seleccionando adecuadamente el paciente, no es de esperar desgaste del polietileno en los primeros 10 años


Objective. Review of the clinical and radiographic results of a series of knee replacements using the mobile-bearing knee prosthesis (T.A.C.K.®, Link). Materials and methods. A retrospective study of 40 knee replacements was carried out at our center by four different surgeons using the same surgical technique. All prosthesis implanted between March 1991 and December 1992 were included in the study. In the 33 patients who were followed up, the range of movement, joint stability, and presence of pain using the Hospital for Special Surgery (HSS) score were evaluated and a radiographic was made. Results. Revision surgery was performed on 5 prosthesis (15%), one for deep infection requiring fusion, one for patella femoral dysfunction, 2 for implant misalignment, and one incorrect indication. The last three resolved with a rotational hinge prosthesis (Rotatoria®, Link). Severe polyethylene wear occurred in only one case. Conclusion. With the mobile-bearing knee prosthesis, good technique, and proper patient selection, there should be no polyethylene wear in the first 10 years

ABSTRACT

Se presenta un caso de fractura de apófisis coracoides asociada a una luxación acromioclavicular y rotura de ligamentos coracoclaviculares en un varón de 28 años de edad. La reducción abierta y fijación interna de la apófisis coracoides, de la articulación acromioclavicular y la sutura directa de los ligamentos coracoclaviculares ha condicionado un resultado excelente (AU)

ABSTRACT

Se presentan 2 casos de coxartrosis rápidamente progresiva. Se trata de una coxopatía poco frecuente y de etiología aún desconocida, que radiológicamente puede ser similar a otras coxopatías agudas y subagudas. Su diagnóstico viene dado fundamentalmente por la clínica, y la rápida y agresiva evolución hacia la destrucción articular. Ambos casos precisaron sustitución protésica de la cadera afectada (AU)

ABSTRACT

As there is little evidence of the efficacy of 25-hydroxyvitamin D3 (25-HCC) in reducing the risk of new fractures in osteoporotic women, we performed an open, prospective study with a follow-up of 1 yr in 58 females over 65 yr of age with osteoporosis and proximal femoral fractures. The patient group received 1 g calcium per day and 10 640 IU 25-HCC per week, while the control group received 1 g calcium daily. Biochemical markers of bone remodelling, serum calcium and parathyroid hormone were determined. Bone mineral density was assessed in the lumbar spine and in the proximal femur by two methods. After 1 yr of treatment, 25-HCC corrected secondary hyperparathyroidism, increased urine calcium excretion, and increased bone mass in the femoral neck, but had no effect upon the appearance of new fractures.
