ABSTRACT

Los hemangiomas son los tumores hepáticos benignos más prevalentes, por lo que su hallazgo incidental en estudios radiológicos de pacientes oncológicos es muy frecuente. Presentamos el caso de una paciente con diagnóstico de teratoma ovárico inmaduro de alto grado y lesiones hepáticas con criterios radiológicos típicos de hemangiomas que, en el curso del tratamiento quimioterápico adyuvante con BEP (cisplatino, etopósido y bleomicina), experimentan una disminución significativa de su tamaño. La respuesta de los hemangiomas hepáticos inducida por determinados quimioterápicos puede simular la respuesta de las metástasis hepáticas, y puede inducir a una interpretación errónea de las imágenes. El objetivo de este artículo es documentar la existencia de una «respuesta» de los hemangiomas inducida por la quimioterapia, en concreto cuando se usa la terapia combinada BEP, ya que ha sido escasamente publicada en la literatura y, según nuestro conocimiento, nunca en el curso del tratamiento de un teratoma inmaduro del ovario


Liver hemangiomas are the most common benign primary tumors of the liver. Due to their high prevalence, incidental finding of hemangiomas during radiological workup of oncologic patients is not uncommon. We present the case of a patient diagnosed of an ovarian immature teratoma and hepatic lesions with typical radiological appearance of hemangiomas that decreased in size during adjuvant chemotherapy with BEP regimen (cisplatin, etoposide and bleomycin). Chemotherapy-induced shrinkage of hepatic hemangiomas can mimic the response of liver metastases to chemotherapy and may induce erroneous interpretations. The purpose of this report is to describe a chemotherapy-induced response of liver hemangiomas, especially when BEP combination is used. To date this observation has been barely reported in the literature and, to our knowledge, this is the first documented case of chemotherapy-induced response of liver hemangiomas in a patient diagnosed of an ovarian immature teratoma

ABSTRACT

Sequences encoding DUF1220 protein domains show the most extreme human lineage-specific copy number increase of any coding region in the genome and have been linked to human brain evolution. In addition, DUF1220 copy number (dosage) has been implicated in influencing brain size within the human species, both in normal populations and in individuals associated with brain size pathologies (1q21-associated microcephaly and macrocephaly). More recently, increasing dosage of a subtype of DUF1220 has been linked with increasing severity of the primary symptoms of autism. Despite these intriguing associations, a function for these domains has not been described. As a first step in addressing this question, we have developed the first transgenic model of DUF1220 function by removing the single DUF1220 domain (the ancestral form) encoded in the mouse genome. In a hypothesis generating exercise, these mice were evaluated by 197 different phenotype measurements. While resulting DUF1220-minus (KO) mice show no obvious anatomical peculiarities, they exhibit a significantly reduced fecundity (χ(2) = 19.1, df = 2, p = 7.0 × 10(-5)). Further extensive phenotypic analyses suggest hyperactivity (p < 0.05) of DUF1220 mice and changes in gene expression levels of brain associated with distinct neurological functions and disease. Other changes that met statistical significance include an increase in plasma glucose concentration (as measured by area under the curve, AUC 0-30 and AUC 30-120) in male mutants, fasting glucose levels, reduce sodium levels in male mutants, increased levels of the liver functional indicator ALAT/GPT in males, levels of alkaline phosphatase (also an indicator of liver function), mean R and SR amplitude by electrocardiography, elevated IgG3 levels, a reduced ratio of CD4:CD8 cells, and a reduced frequency of T cells; though it should be noted that many of these differences are quite small and require further examination. The linking of DUF1220 loss to a hyperactive phenotype is consistent with separate findings in which DUF1220 over expression results in a down-regulation of mitochondrial function, and potentially suggests a role in developmental metabolism. Finally, the substantially reduced fecundity we observe associated with KO mice argues that the ancestral DUF1220 domain provides an important biological functionthat is critical to survivability and reproductive success.

ABSTRACT

A methodology to automatically detect potential retakes in digital imaging, using the Digital Imaging and Communications in Medicine (DICOM) header information, is presented. In our hospital, neither the computed radiography workstations nor the picture archiving and communication system itself are designed to support reject analysis. A system called QCOnline, initially developed to help in the management of images and patient doses in a digital radiology department, has been used to identify those images with the same patient identification number, same modality, description, projection, date, cassette orientation, and image comments. The pilot experience lead to 6.6% and 1.9% repetition rates for abdomen and chest images. A thorough analysis has shown that the real repetitions were 3.3% and 0.9% for abdomen and chest images being the main cause of the discrepancy being the wrong image identification. The presented methodology to automatically detect potential retakes in digital imaging using DICOM header information is feasible and allows to detect deficiencies in the department performance like wrong identifications, positioning errors, wrong radiographic technique, bad image processing, equipment malfunctions, artefacts, etc. In addition, retake images automatically collected can be used for continuous training of the staff.

ABSTRACT

Existe información contradictoria en relación al efecto de la carencia de hierro sobre respuesta inmune. La interpretación de los datos inmunológicos obtenidos en pacientes con anemia ferropriva varía según edad, estado nutricional, presencia de infecciones y asociación con otras deficiencias nutricionales. Para definir el efecto que ejerce la anemia ferropriva sobre la respuesta inmunitaria celular se seleccionaron 15 lactantes con anemia ferropriva y 11 controles adecuadamente nutridos con hierro, entre 15 y 19 meses de edad, eutróficos, sin infecciones ni deficiencias nutricionales asociadas. Se determinó la capacidad de sensibilización a antígenos que evocan respuestas cutáneas de hipersensibilización retardada con 2-4 dinitroclorobenceno y PPD, como también la capacidad proliferativa de sus linfocitos al estímulo con fitohemaglutinina. No se observaron diferencias significativas en la respuesta inmune celular entre pacientes y controles antes y al cabo de tres meses de tratamiento con 5 mgù kgù día de hierro elemental en forma de sulfato ferroso. Se sugiere que la anemia ferropriva leve no alteraría la respuesta inmune celular

ABSTRACT

Existe información contradictoria en relación al estado nutricional de zinc en pacientes con fenilquetonuria. El objetivo de este estudio fue confirmar la existencia de manifestaciones indirectas de deficit de zinc, a través del efecto que ejerce la suplementación de este mineral sobre la respuesta inmune celular. Se determinó la reacción cutánea de hipersensibilidad retardada y la capacidad linfoproliferativa frente a fitohemaglutinina antes y después de suplementación con acetato de zinc, en 8 pacientes fenilquetonúricos. Dos de ocho pacientes presentaron reacción cutánea positiva antes de la suplementación, aumentando a seis el número de sujetos con respuesta positiva después de la ingesta de zinc (p<0,05 prueba de Mac Nemar). La capacidad linfoproliferativa aumentó al término de la suplementación con zinc (p<0,05 test de t pareado). Estos resultados sugieren que existe un déficit de nutrición de zinc en pacientes con fenilquetonuria, puesto que la administración de este mineral traza estimularía la respuesta inmune celular

ABSTRACT

Con la finalidad de identificar la existencia de casos urbanos de Hidatidosis Pulmonar en la ciudad de Lima Metropolitana, se diseño un estudio epidemiológico de tipo descriptivo. En una primera etapa del estudio se identificaron los casos de hidatidosis pulmonar en base a los registros hospitalarios de los pacientes operados en el Hospital Hipólito Unanue, entre loa años 1980 y 1986. Los casos identificados fueron clasificados en "metropolitanos", "provinciales", "migrantes" y "transeúntes", según el lugar de procedencia. La segunda etapa del estudio consistió en visita domiciliaria a los casos "metropolitanos", a fin de confirmar dicha situación. Se estudiaron 261 casos de hidatidosis pulmonar, de los cuales 57 (21.8 por ciento) eran "metropolitanos". En el 72.2 por ciento de los casos ubicados por visita domiciliaria se confirmó la condición de "metropolitanos". En estos casos se observó un promedio de edad menor que en el resto de los casos, no habían ganaderos, agricultores ni matarifes, el nivel educativo era principalmente secundario y superior, el promedio de tiempo de enfermedad también fue menor que en los grupos restantes. No hubo diferencias en las características clínicas. Este estudio constata la existencia de casos urbanos de hidatidosis en Lima Metropolitana. Sin embargo, no permite determinar la prevalencia e incidencia de esta enfermedad en la ciudad, siendo indispensable continuar investigando en este campo.

ABSTRACT

PIP: 5 different types of IUDs were used in 1020 women after placental expulsion: T Cu220 in 101 cases; Lippes Loop in 407 cases; T Cu200 in 180; Progestasert in 196; and the Multiload 250 in 136. IUD insertion was accomplished by 3 different techniques: manual, with Forester's forceps, and with an experimental applicator. There were more expulsions and more pregnancies when the applicator was used. Expulsion and pregnancy rates were higher with the Progestasert. The T Cu220C showed the lowest number of expulsions and pregnancies and the highest continuation rate. Removals for medical reasons were similar among all IUD types. (author's modified)^ieng

ABSTRACT

Se analizan los resultados obtenidos en 1020 casos de aplicacion, inmediatamente despues del nacimiento de la placenta, de cinco tipos diferentes de dispositivos intrauterinos. En 101 casos se utilizo T Cu 220 C, en 407 Asa de Lippes, en 180 T Cu 200, en 196 Progestasert y en 136 Multiload 250. La aplicacion se hizo mediante tres tecnicas: a) manual, b) con pinza de Forester y c) con un aplicador en experimentacion. Cuando se utilizo el aplicador se observo mayor tasa de expulsiones y por tanto de embarazos. Las tasas de expulsiones y embarazos fueron mayores con el Progestasert. Con la Teu 220 C se observo el menor numero de expulsiones y de embarazos, asi como la mayor continuidad de uso.Los retiros por causas medicas fueron semejantes para todos los dispositivos intrauterinos

The determinations of the amino acid pools, including tryptophan (by fluorescence spectrometry), in the arterial and venous blood of the mother, the arterial and venous blood of the fetus (cord blood), the amniotic fluid and the placenta was done in eight women at the moment of delivery. Only nine amino acids (asp, try, met, phe, ser, cys, lys, gly, thr) were significantly retained and four (arg, glu, pro and glu-NH2) were significantly released by the fetal tissues. In contrast with this behavior most amino acids were retained by the maternal tissue, try, phe and hist showing the highest retention. When the amino acids are grouped as essential and nonessentials, the maternal tissues retained both categories without apparent discrimination, while the fetal tissues retained essential amino acids preferentially. Our results emphasize the importance of the placenta as the regulating system of the fetal milieu under normal conditions. Thus human fetal blood levels of amino acids are patterned after the placental ones and not after the maternal values obtained at the same time. It is apparent that the placenta seems to function as a nonspecific retention filter for outgoing amino acids, but that its function is selective in respect to the release of amino acids into the fetal circulation.

ABSTRACT

Se realiza una investigación sobre mortalidad en pacientes de 65 años y más en el municipio de Palma Soriano durante el período comprendido desde el 1ro de enero de 1984 hasta el 31 de diciembre de 1993, a fin de determinar las primncipales causas de muerte según sexo, edad, y lugar de defunción. Las enfermedades cardiovasculares, los tumores malignos y los accidentes fueron las principales causas de muerte, con predomnio en pacientes del sexo mascvulino. Se pone en evidencia un número importante de fallecimientos en pacientes no hospitalizados, fundfamentalmente debido a las enfermedades del corazón entre éstas el infarto del miocardio. La mortalidad por accidentes, suicidios, diabetes mellitus y tumores malignos muestran una tendencia ascendente con mientras que las enfermedades del corazón y cerebrovasculares se han mantenido estacionarias en los últimos cinco años (AU)
